Ability of Lab to detect
emerging antimicrobial resistance Most Labs were able to detect:
1. Methicillin/oxacillin resistance in S. aureus (MRSA)
2. Hight-level vancomycin resistance in Enterococci (VRE)
3. Resistance to extended-spectrum cephalosporins in
E.coli / K. pneumoniae (ESBL) Many Labs had difficulty in recognizing
Mettalo-beta-lactamase producing organisms Reduced susceptibility to penicillin in
Streptococcus pneumoniae Carbapenem resistance in Enterobacteriacea
The most difficult phenotype for Labs to detect was Reduced susceptibility to vancomycin in staphylococcus

Qualitative techniques (S,I,R)
Disc diffusion method ; inhibition zone (mm)
Quantitative methods :
MIC and MBC determination (ug/ml, mg/L)
- Broth dilution - Agar dilution
- E-test (Agar gradient ; MIC determination)
MIC = Minimum inhibitory concentration
MBC=Minimum bactericidal concentration

Results • The study supports the evidence that the hands of surgeons
and scrub nurses carry microorganisms identified as sources of microbial contamination. Common organisms including
coagulase-negative staphylococci (CNS), methicillin- sensitive Staphylococcus aureus (MSSA),
methicillin- resistant Staphylococcus aureus (MRSA), A. baumannii, P. aeruginosa, and Enterobacter species These common organisms may cause surgical site infections.
S. aureus and CNS are the leading causes of surgical site infections.
• The majority of flora isolated were gram-positive cocci, including CNS = 85.9% in pres-crub 77.2% post-scrub
MSSA = 9.8% pre-scrub 2.2% post-scrub Gram-negative bacteria 32.6% pre-scrub
7.6% post-scrub
• All health care workers in operating rooms should follow the recommended practice for surgical hand scrubs to prevent surgical site infection

Results of various laboratory tests are required for diagnosis of infection.
Therefore, the clinical laboratory has many important roles
in diagnosis and management of infectious disease.
Moreover, the laboratory has always played a key role in
controlling nosocomial infections.

Role of the clinical laboratory in infection control
The hospital laboratory
plays major role in care of patients with infection (diagnosis, treatment, surveillance, control and prevention)
These mission cannot be carried out effectively
without proper attention to communication with the medical staff who directly care for patients
The interface between clinician and microbiologist is critical
or logical in diagnosis and treatment of infectious diseases Rosenblatt JE; Am J Clin Pathol 1988;90:355-357.

Role of the clinical laboratory in infection control
Potential problems related to Lab activities in
Epidemiologic investigation
1. Misdiagnosis of outbreak (pseudo outbreak)
2. Inadequate quality control
3. Overuse of the Lab’s resources
![Page 59: Collaboration with Microbiology Laboratory in Investigation of N.I. …bamras.ddc.moph.go.th/userfiles/Collaboration with... · 2018-03-29 · and Staphylococcus spp., and Stenotrophomonas](https://reader033.vdocuments.net/reader033/viewer/2022041615/5e3b202900f62f7287408e11/html5/thumbnails/59.jpg)
Potential problems related to Lab activities in Epidemiologic investigation
1. Misdiagnosis of outbreak (pseudo outbreak)
* Clusters of ‘false infections’ or * False clusters of ‘true infections’ * Inaccurate or inconsistent microbiologic procedures * Ineffectively sterilized or contaminated reagents or equipments
![Page 60: Collaboration with Microbiology Laboratory in Investigation of N.I. …bamras.ddc.moph.go.th/userfiles/Collaboration with... · 2018-03-29 · and Staphylococcus spp., and Stenotrophomonas](https://reader033.vdocuments.net/reader033/viewer/2022041615/5e3b202900f62f7287408e11/html5/thumbnails/60.jpg)
Potential problems related to Lab activities in Epidemiologic investigation
2. Inadequate quality control
* Organisms involved in outbreak usually
are isolated at different time
* Lab results are particularly susceptible to day-to-day variation or batch-to-batch variation
* (IQC, EQA, HA, LA, ISO15189)
![Page 61: Collaboration with Microbiology Laboratory in Investigation of N.I. …bamras.ddc.moph.go.th/userfiles/Collaboration with... · 2018-03-29 · and Staphylococcus spp., and Stenotrophomonas](https://reader033.vdocuments.net/reader033/viewer/2022041615/5e3b202900f62f7287408e11/html5/thumbnails/61.jpg)
Potential problems related to Lab activities in Epidemiologic investigation
3. Overuse of the Lab’s resources
*Too many cultures from personnel or environment
* Basic infection control measures can solve
some outbreak without additional microbiologic data or
epidemiologic investigation
* Typing system must be used selectively, even during outbreak
![Page 62: Collaboration with Microbiology Laboratory in Investigation of N.I. …bamras.ddc.moph.go.th/userfiles/Collaboration with... · 2018-03-29 · and Staphylococcus spp., and Stenotrophomonas](https://reader033.vdocuments.net/reader033/viewer/2022041615/5e3b202900f62f7287408e11/html5/thumbnails/62.jpg)
Role of the clinical laboratory in infection control
Sources of pseudo-outbreaks of nosocomial infection
1. Clinical data or clinician • Wrong clinical diagnosis
• Positive cultures represent colonization rather than
infection (supportive Lab data may required)
• Failure to differentiate community-acquired from nosocomial infection
• Contamination during specimen collection
![Page 63: Collaboration with Microbiology Laboratory in Investigation of N.I. …bamras.ddc.moph.go.th/userfiles/Collaboration with... · 2018-03-29 · and Staphylococcus spp., and Stenotrophomonas](https://reader033.vdocuments.net/reader033/viewer/2022041615/5e3b202900f62f7287408e11/html5/thumbnails/63.jpg)

Guidance for Control of Infections with Carbapenem-Resistant/Carbapenemase-Producing Enterobacteriaceae
in Acute Care Facilities
Recommendations from CDC and (HICPAC) An aggressive infection control strategy, using Contact precautions Implementing CLSI guidelines for CRE detection In areas where CRE are not endemic:- 1) Review microbiology records preceding 6-12 months
to determine whether CRE have been recovered at the facility.
2) If the review finds previously unrecognized CRE,
perform a point prevalence culture survey in high-risk units to look for other cases of CRE.
3) Active surveillance cultures of patients with
epidemiologic links to persons from whom CRE have been recovered.
