Thiscoursefocusesprimarilyonthebasicprinciplesofepidemiology:(1)introductiontoepidemiologicthinking,(2)measuresofdiseasefrequencyandassociation,(3)basicstatisticsrelevanttoepidemiologymeasurement,and(4)anoverviewofstudydesign(especiallycross-sectionalsurveys,case-control,andcohortstudies).Astimeallowswewillalsocoveraverybasicintroductiontobias,confounding,andeffectmodification.

Thisintensivecourseintroducestheprogramplanningprocesswithaspecificfocusondevelopinghealthpromotionprograms.Bytheendofthecourse,studentsshouldbeconversantwiththeelementsoftheplanningprocess,includingproblemanalysis,strategy/interventionselection,settingobjectives,programimplementationandprogramevaluation.ThePRECEDE/PROCEEDmodelwillbeusedasanorganizingframeworktoguidestudentsthroughtheprocessofdevelopinghealthpromotionprogramsinavarietyofsettingsandpopulations.Throughtheapplicationoftheoryandwithconsiderationofsocial,cultural,economicandpoliticalfactorsthatinfluencehealth,studentswilldevelopamulti-levelinterventionprogram.

ProgramPlanningThissessionwillprovideanoverviewontheresponsibilityoftheUnitedStatestoprovidehealthcaretoAmericanIndiansandAlaskaNativesunderthefederaltrustrelationship.ItwillincludeanoverviewontheIndianhealthsystembyreviewingitshistoryandpoliciesthroughthedevelopmentofimportantlegislation,suchastheIndianSelf-DeterminationandEducationAssistanceActandtheIndianHealthCareImprovementAct.Thecoursewillcoverthefinancing,delivery,andpaymentofhealthcareserviceswithintheIndianhealthsystem.Animportantaspectofthecoursewillbetodiscusspastandpresentchallengesandpotentialsolutionstothefinancing,structure,staffing,andservicedeliveryoftheIndianhealthsystem,includingadiscussionabouthowhealthreformwillimpactthesystem.Areviewofcurrentandpastlegislation,healthpolicy,andresearchissueswillbeincluded.AttentionwillalsofocusonanychangestotheAffordableCareActstructurethatmaybeenactedorcontemplatedbythetimeofthecourse.

Thiscourseinvolvesamoredetailedexplorationofepidemiologicstudydesigns,primarilycross-sectional,case-control,andcohortstudiesaddressingquestionsrelatedtochronicdiseasesandcancercontrol.Wewillincludediscussionsofsomeoftheculturalconsiderationsinthedesignandimplementationofthesestudies.Thiscoursewillalsoexamineinmoredetailtopicsofimportancetothedesign,implementation,andevaluationsofobservationalepidemiologystudies:(1)systematicerror(selectionandinformationbiases),(2)confounding,and(3)effectmodification.Readingassignmentsforthiscoursewillincludejournalarticles,aswellaschaptersfromtheassignedtext.Prerequisite:EpidemiologyI(orequivalent).

Thiscourseisdesignedforhealthprofessionalsandstudentswithaworkingknowledgeofepidemiologyandstudydesign.Itwillcoverhowtoplan,design,anddevelopabriefoutlineforanNIHresearchproposal(abstract,aims,researchdesign,outcomes,budgetlineitemfor1category,budgetjustification,etc.).Participantsshouldhaveagrantideaortopicpriortoattendingtheclassforthecoursetobeofsignificantvalue.Theoutlineprovidesthebasisforworktheparticipantwillneedtoexpandaftertheclassisover.Thecourseisheldinthecomputerlabbuteachdayhashomeworkandparticipantsneedtohaveaccesstoalaptopcomputertocompletetheirdailyhomework.

ResearchDesign&GrantWritingThiscoursewillbeareviewoftheepidemiologicprinciplesandskillsusedaswellascommonjurisdictionalissuesencounteredduringanoutbreakresponse.Wewillundertakeaninteractivereviewofdiseasesurveillance,detectinganoutbreak,creatingcasedefinitions,selectingastudydesign,andchoosingappropriateoutcomemeasuresandtheirstatisticaltests.Wewillspendthelatterpartofeachafternoonworkingtogetherthroughcasestudiesthathighlightthesetopics.Thecourseistailoredtopracticing,MPH-levelepidemiologistsandcoverstopicsoftennotincludedindoctorateprogramssuchasmedicalschool.Thiscoursemayalsobeusefulforphysicianswhoareworkinginappliedpublichealthsettings.

ThissessionwillprovideanoverviewofFocusGroupmethodologyandwillincludeadiscussiononthemethod’sstrengthsandlimitations.Theinstructorswillprovideexamplesofwhenfocusgroupsareutilizedto:informquantitativeresearchdesignandsurveydevelopment;explainandprovidecontextforquantitativeresults;and/orintegratedintoamulti-methodevaluation.StudentswillparticipateinamockFocusGroupsessionandconsideranalysisofresultantdatainordertogainexperiencewiththismethodology.

ThiscourseexaminespreventionandtreatmentservicesforAmericanIndian/AlaskaNativepopulationswithsubstanceuseandbehavioralhealthdisorders.Classesexaminesocialdeterminantsofhealthandtherangeofsubstanceusedisorders(SUDs)anddescribehealthservicesresearchopportunities.PrevalenceandincidenceofSUDs,riskandprotectivefactors,culturallyrelevantinterventions,useofparticipatoryresearchmethods,andculturallydevelopedandsupportedinterventionswillalsobeexplored.Classmembersengageinactivediscussionrelatedtotheirownworkandcommunity.Servicesresearchstrategiesandpolicyinterventionsarealsoreviewed.Studentswillbeintroducedtopapersonkeyconceptsandwillparticipateingroupprojects,activities,andinteractivelearning.

SubstanceAbuseEpidemiologyTheaimofthiscourseistoprovideabriefoverviewofbasichealthdataliteracytopicsforthosewhowishtointerpret,presentandmakeuseofepidemiologicdatabutdonothaveastatisticsbackground.Studentswillleavewithanunderstandingofcoreepidemiologyconcepts,wheretofindAI/ANspecifichealthdata,bestpracticesforpresentingdataeffectively,andusingGISmappingtools.Pleasenotethatthiscourseisdesignedforthosewhowishtointerpretandmakeuseofhealthdata,butnotthosewhowishtolearnhowtoconducttheirowndataanalysisandsurveys(thesetopicsarecoveredintheEpidemiologyI&IIcourses).

Cancerremainsaleadingcauseofmorbidityandmortalityamongtribalpeople,andmanyofthecausesofcancerinNativepeoplearepreventable.Inthiscourse,wewilldiscusscanceretiology,cancerbiology,commonepidemiologicapproachestostudyingcancerandcancerprevention,surveillance,andwillpresentexamplesofsuccessfulcancerpreventionprojectsinIndiancountry.

Assovereignnations,AmericanIndianTribesareresponsiblefortheoverallhealthandwell-beingoftheirmembersalongwiththelandandenvironmentoftheirtribe.Tribesarebecomingincreasinglyinvolvedinmorepublichealthactivitiesandregulationanddeliveryofpublichealthservices(throughvariousfundingsources,grantsandcontracts),aloneorincollaborationwithothertribesandlocal,countyandstatehealthdepartments.InthiscoursestudentswilllearnaboutthelegalresponsibilityoftheUnitedStatestoprovidehealthservicestoAmericanIndians.ParticipantswillexaminethepublichealthandwellnessissuesfacingAmericanIndianandAlaskaNativecommunities;reviewhistoricalimplications,analyzelegislation,applyspecificfinancingrequirements,andgainanunderstandingoftheuniqueAmericanIndianpublichealthsystemandthecomplexsetofservices,activities,collaborationsandstakeholdersthatvariesbytribeandregion.Thecoursewillincludeexpertguestpresenterswhowilladdresstopicsofpublichealthimportancetotribalpeople,includingimmunizations,environmentaljustice,mediaadvocacyforpolicychange,infectiousdiseasesurveillanceinIndiancountry,andinjuryprevention.
