May 5, 2015 Dear Dr. Natividad, I would like to convey my sincere and deepest a pology to you for not making it to my scheduled Physical Exam last April 30, 2015. I must offer my deepest apologies for the inconsideration I displayed by missing our appointment without calling you and explaining the circumstances. It was unprofessional and impolite of me to do so and I hope that it did not cause you any problem . I was obligated to attend all my internship make-up duties at Quezon City General Hospital last May 2, 2015 to May 3, 2015 because the University of Santo Tomas was compelling us to finish our make-up duties on or before the scheduled submission of grades. May 4, 2015 was also inconvenient for me due to our requirement to accomplish our hospital clearance which also has its deadline. Once again, I apologize for the inconvenience and thank you sincerely for your time and concern.

my scheduled dates for enrollment last April 20-30, 2015.

I must offer my deepest apologies for the inconsideration I displayed by missing our

appointment without calling you and explaining the circumstances. It was unprofessional

and impolite of me to do so and I hope that it did not cause you any problem.

I was obligated to attend all my internship make-up duties at Quezon City General

Hospital last May 2, 2015 to May 3, 2015 because the University of Santo Tomas was

compelling us to finish our make-up duties on or before the scheduled submission of

grades. May 4, 2015 was also inconvenient for me due to our requirement to accomplish

our hospital clearance which also has its deadline. Because of the said situations, I was

strained to finish my Physical Exam on May 5, 2015. Evidently, the said date is beyond the

listed dates for enrollment. In this state, I mistakenly assumed that I am no longer qualified

for enrollment. My notion was only altered when I gave your office a call and I was given

possible options that may fix these problems.

Once again, I apologize for the inconvenience and thank you sincerely for your time

and concern.
