The Advisor Approval Form

If you are late……

• You might move to the bottom of the list for that day

• You might not see someone that day

• You might need to book another appointment

• You risk delaying your registration

Arrive on time

Is your paperwork complete?• Advisor Approval Form - You must fill in DEPT, Course Name & #, Section, Lab, Seminar as they

appear in the course catalogue e.g. GEOG 363, Lect X, Lab A

• Course Check List - Fill in your grade for completed courses and use I/P of courses In Progress

- Forms are to be completed in Ink, signed,& Dated

• Print copy of your student recordIf your paperwork is not ready at the time of your

appointment you are late.
