Objective
• Understand the updates of the US2014 form on the list of medical services in the Summary tab of Individual Details page
• Record HA (Health Assessment) Results for a Normal medical case using the new US2014 form
• Validate the US2014 form of a Normal medical case

Mandatory – fields that will always be mandatoryConditional Mandatory – fields that are mandatory depending on the medical services provided:
• Radiology if X-ray Chest is a scheduled/assigned medical service• Syphilis if Syphilis Screening is a scheduled/assigned medical service
• The following date fields cannot go beyond one year into the future from the current date (e.g. 01-Jan-2041)– Exam Date– Panel Physician Signature Date– Physical Exam Date
• The Panel Physician Signature Date cannot be earlier than the Exam Date

• System updates the Medical Services in the Summary tab of the Individual Details page based on updates made in the US2014 form (including the Status)
• Set All No/Negative/Normal function can populate most of the data for you, but it is still best to do QA/QC
