No.:v03.3Issued :09 October 2013TSSR Document Guidance and minimum standards of working safely and technically.1Vendor and/or Vendor Approved Service Provider (Subcontractor) must us qualified, competent, trained, certified personnel in the performance of the Technical Site Survey, and production of the Technical Site Survey Report (TSSR).2Vendor and/or Vendor Approved Service Provider (Subcontractor) must work in accordance with International best practices for Occupational Health and Safety, and all personnel going on sites must work in accordance with safe working practices, at a minimum in alignment with Indosat and Ooredoo group policy, manual, procedures and safety instructions.All personnel must be fully certified at a minimum to Indonesian regulatory standards and requirements, and furthermore, to Indosat, Vendor own and any additional requirements, and have been provided OJT, and certified competent to perform the job both technically and safely by the Vendor.3Vendor and/or Vendor Approved Service Provider (Subcontractor) must provide at a minimum, the Indosat specified minimum tools, equipment, PPE and First Aid kit required for TSSRs, and furthermore additional as recommended by the Vendor, and as required per the Site Specific Safe Working Method Statement (SWMS), and Job-Safety Analysis, both of which also must be performed and approved by the Vendor prior to any personnel going on any sites to perform the TSSRs.4All photographs taken per the requirements of the various sections of the TSSR must be both included in the TSSR from a report perspective, in the specified sections, and additionally, must be uploaded in the appropriate directory allocated to the TSSR within the Site Specific directory in Site Binders part of Share-point. Guidance on the sub-directories required to be created and photos to be uploaded is contained within the Required Photos sections of this document, as well as the specified sections for photo placement. Where in doubt, more photos should be taken, and uploaded.5All photographs must be taken with the minimum required capability camera, including the ability to geo-tag, time-stamp, and date-stamp the photos, with such properties preserved in the raw photo upload to Share-point.6All photographs must be properly labelled, and for the raw photo upload, properly oriented for storage in Share-point, without loss or modification of Geo-tagging, timestamp and datestamp properties.7Panoramic View (30, 60, 90, 120, 150, 180, 210, 240, 270, 300, 330, 360) Panoramic views for tower/monopole site must be taken either from top of tower/monopole or above the existing antenna under survey. Panoramic views for rooftop pole site must be taken from the center of building at highest point if possible. If it is not doable, panoramic views must be taken from each building corner correspond to compass north reading.
