Welcome to Benefits Open Enrollment for 2019 Monday, September 10, 2018 through Friday, October 5, 2018 For Full-time and Reduced Contract Employees Open Enrollment is your once-a-year opportunity to learn your benefits options and to make additions, changes or deletions to your benefit elections including health, dental, vision, life insurance, flexible spending accounts, commuter benefits, and buy-up long-term disability insurance. Any elections made during Open Enrollment are effective and irrevocable as of January 1, 2019 and will stay in effect until December 31, 2019 (unless you have a qualified change in status and make a timely corresponding change). PERS Select Basic PPO Plan has adopted Value-Based Insurance Design (VBID): o The new PERS Select Plan will change co-pays for primary care, specialists, and urgent care; award credits to

reduce your annual deductible; eliminate hospital tiering, so you can access any hospital in your network at one coinsurance rate; eliminate coinsurance for inpatient delivery, after enrollment in Healthy Moms program.

o PERS Select VBID offers the following:

Available in all 58 California counties

Anthem will assign a primary care physician (PCP) to each insured to coordinate your care

Increase calendar year deductible

$1,000 for individual

$2,000 for family

Earn $100 credit/individual towards deductible if you get 1) Biometric screening (tested at your doctor or at Quest Lab) 2) Flu shot 3) Non-smoking certificate 4) Second opinion for elective surgeries by calling 888-361-3944 5) ConditionCare certification for engaging with a nurse manager for disease management if you

have asthma, diabetes, COPD, or heart conditions

Office Visits reduced to $10 with primary care physician (PCP)

Urgent Care copay increase $20 to $35

Specialist Visit copay increase $20 to $35

Hospital Coinsurance decrease 30% to 20%

Inpatient Maternity delivery coinsurance waived when using Healthy Moms program SpineZone, an online back and neck pain program, and Wisdom, a study to optimize breast cancer screening, will be

available for all PPO members. c

REMINDER: Designation of Emergency Contact and Beneficiary is now possible via Secova Online enrollment

A beneficiary is a person or legal entity that is entitled to receive the proceeds from an estate, trust, retirement account, life insurance policy, or transfer on death accounts. A beneficiary can be one or more individuals or organizations, such as a tru st or charity. You can designate two types of beneficiaries:

o Primary beneficiaries are first in line to receive the designated asset upon your death. o Secondary (or contingent) beneficiaries receive the asset if there are no surviving primary beneficiaries upon

your death.

IMPORTANT: Before going online to select your beneficiary, make sure you have your beneficiary’s Social Security number (SSN), date of birth, and mailing address.

Importance of Beneficiary Designations

o If you wish to name someone other than your spouse or domestic partner as beneficiary, if you have special estate planning needs, or if you wish to assign ownership of your life insurance to another person, seek legal and tax advice before completing your beneficiary designation.

o If your beneficiary is a minor (under age 18), you may wish to consult with a legal advisor to ensure you understand how to best protect the rights of your minor beneficiary.

Please note that employee death benefits are paid to the most recent beneficiary designation on file with the Benefits Office. Past paper enrollment records still retained until the information is fully populated by the employees and/or Benefits staff at a later date.
