Ventricular Assist Device Program, Cedars Sinai Medical Center, Los Angeles, CA Constructed, directed and developed a VAD program from inception to production and growth. Developed clinical expertise in VAD implantation by creating a multidisciplinary team

IRB PROTOCOLS: PRINCIPLE INVESTIGATOR

1. “Clinical Evaluation of the Thoratec® Implantable Ventricular Assist Device (IVAD)”

Specific Aims:

To demonstrate that the Thoratec® IVAD, when used with either the Dual Drive Console or the TLC-II Portable Driver is safe and effective for either a bridge to transplant or VAD support pending myocardial recovery. Patients may be discharged to home with the IVAD in place. This is a multicenter, prospective controlled clinical trial conducted according to a common protocol. Ten U.S. study sites will be involved.

2. “Clinical Evaluation of the ACORN Cardiac Support Device Therapy in Patients with Dilated

Cardiomyopathy"

Specific Aims:

This is a prospective, randomized, unblinded, parallel group evaluation of the Acorn Cardiac Support Device therapy in patients with dilated cardiomyopathy of either ischemic or non-ischemic origin, who may or may not need concomitant mitral valve repair/replacement. The study is intended to assess the safety and efficacy of the Acorn Cardiac Support Device. 300 patients will be enrolled at 20 U.S. centers.

3. “Assessing Endothelial Dysfunction Utilizing Calibrated Cuff Plethysmography” Specific Aims:

This is a prospective, non-randomized single-center study to evaluate a medical device known as the Calibrated Cuff Plethysmograph. The device can measure endothelial dysfunction (ED) and provide an actual measurement as opposed to an estimate of ED. The device is a non-invasive , non-imaging device. 100 patients with known cardiovascular disease will be tested.

IRB PROTOCOLS: CO-INVESTIGATOR

1. “A Phase II Randomized Multicenter Clinical Study to Evaluate the Safety and Efficacy of the Tandem HeartTM pVAD System versus Conventional Therapy for the Treatment of Cardiogenic Shock.”

Specific Aims: To evaluate the efficacy of the Tandem HeartTM pVAD System in providing effective circulatory support to stabilize patients who are in persistent cardiogenic shock despite maximum medical management. Ninety patients will be randomized at up to 20 centers. Control patients will be supported with an IABP and medical management.

2. “Safety and Hemodynamic Effect of Nesiritide in Postoperative Cardiac Surgical Patients.” Specific Aims: This is an open label study of the administration of fixed dose Nesiritide on the hemodynamics of cardiac surgery patients. The major goal is to establish the safety of administering Nesiritide to post- cardiac surgery patients and the degree of benefit.

3.“A Prospective, Observational Evaluation of the SJM Epic Porcine Valve”

Specific Aims:

The purpose of this study is to monitor and record the performance and durability of a new porcine aortic valve which is processed with glutarldehyde and treated with ethanol in a process called Linx ™. 300 patients will be enrolled at 20 sites in the United States and Canada. Patients will be followed for five years.

4. “A Pharmacodynamic Approach to Immune Monitoring”

Specific Aims:

To define a quantitative relationship between cyclosporine blood levels and inhibition of peripheral and intragraft lymphocytes in recipients of cardiac and pulmonary allografts.

To define a relationship between blood cyclosporine levels and inhibition of the mixed lymphocyte

reaction in recipients of cardiac and pulmonary allografts.

5. “A Two-Year Randomized, Multicenter, Double-blind Study of the Efficacy and Safety of SDZ RAD Versus Azathioprine as part of a Triple Immunosuppressive Therapy Regimen in DeNovo Heart Transplant Recipients”

Specific Aims:

To compare the efficacy of two oral doses (0.75 and 1.5 mg/bid) of RAD001 (an oral analog of rapamycin) with azathioprine in addition to Neoral and steroids in de novo heart transplant recipients as measured by the incidence of death, graft loss/retransplant, biopsy proven acute rejection episode

Grade > 3A or any clinically suspected acute rejection episode associated with hemodynamic compromise in the first 6 months post transplant.

6. “Aortic Comparative Trial: A Randomized Comparison of Stentless Versus Stented Aortic Valve

Replacements”

Specific Aims:

To compare, in a randomized fashion, the hemodynamic characteristics and patient outcomes of both the Torono SPV stentless valve and stented pericardial or porcine valves. Patients will be followed by echocardiogram at 6 months, 1 year, 2 years and 5 years after surgery.

7. “Transmyocardial Revascularization (TMR) Using the Heart Laser CO2 System: A Post-Market Study”

Specific Aims:

To monitor the outcomes of patients treated with TMR in order to gain a better understanding of the risks associated with the treatment. Patients will be followed postoperatively at 30 days and 1 year. Patients included in the study will have stable CCS class 3-4 angina refractory to medical management.

They must also have objectively demonstrated coronary artery atherosclerosis, which is not amenable to direct coronary revascularization.

8. “Phase II Randomized Study of H5g1.1-Scfv: Outcomes in Patients Undergoing Cardiopulmonary Bypass”

Specific Aims:

Anti-human C5a has been shown to down-regulate pro-inflammatory activity in the animal model and dramatically reduce cardiac tissue injury and infarct size in ischemic myocardium. It similarly reduces cerebral swelling and infarct size. This study will compare the effects of H5g1.1-Scfv, an anti-human C5a complement antibody, to placebo in patients undergoing non-emergent heart surgery. Patients will be assigned to one of three groups to receive either a) placebo, or b) H5g1.1-Scfv IV over 10 min. at 2mg/kg or c) H5g1.1-Scfv IV over 10 min. at 2mg/kg followed by H5g1.1-Scfv infusion over 24 hours at 0.5mg/kg/hr. Routine blood tests will be obtained preoperatively and periodically during patient hospitalization and at 30 days following surgery with additional tests to measure the antibody. Neuropsychological tests will be administered pre and postoperatively and any adverse events will be monitored and recorded.

9. “Phase IV Multicenter, Randomized, Double-Blind, Placebo Controlled Trial to Evaluate the Efficacy of Fenoldopam Mesylate in Preserving Renal Function and its Potential Effects on Postoperative Morbidity”

Specific Aims:

The objective of this drug study is to evaluate the effect of fenoldopam mesylate in comparison to placebo on primary and secondary efficacy outcomes. The primary outcome is preserving renal function after CABG. Secondary outcomes are postoperative mortality, cardiac morbidity, stroke and cognitive function. We will also evaluate the safety of intravenous administration of the study drug in comparison to placebo.

The study drug is a known renal and systemic vasodilator. Effects of fenoldopam mesylate as a coronary vasodilator and its ability to increase cardiac output in cardiac surgery patients warrant study of this agent in heart failure and myocardial ischemia. The study drug may also reduce the effects of decreased blood flow to the brain during bypass and, by doing so, reduce mental dysfunction postoperatively.

10. “Acupuncture Following Cardiac Surgery: A Pilot Study”

Specific Aims:

Acupuncture is a known technique used for the treatment of a variety of conditions including pain and discomfort. The uses of therapies such as acupuncture are widely used in Asian countries and are now becoming increasingly practiced throughout the world including the United States. Although acupuncture is frequently used as a “sole” treatment, it is beginning to be used in conjunction with standard “Western” medical treatments. Once a suitable way of integrating acupuncture into the

clinical setting has been developed, we will embark on designing a larger study to assess the efficacy of acupuncture for specific patient needs.

11. “Guided Imagery Before, During and After Cardiac Surgery: A Pilot Study”

Specific Aims:

Guided imagery involves a variety of techniques. The use of imagery techniques is based on the assumption that relaxation associated with imagining a relaxing environment can, in part, alleviate physical tension and anxiety which directly influences a patient’s perception and tolerance to pain. Once a suitable way of integrating guided imagery techniques into the clinical setting has been developed; we will embark on designing a larger study to assess the efficacy of guided imagery for specific patient needs.

12. “The Ross Aortic Valve Replacement Procedure: A Postoperative Follow-Up Study”

Specific Aims: To document the long-term results of the Ross Procedure in patients who suffer from aortic valve disease. Patients will be followed post-operatively by echocardiography.

13. “The Effect of Elevated Preoperative Pulmonary Vascular Resistance on Heart Transplant Recipients: a

single center experience.”

Specific Aims:

The purpose of this study is to evaluate the effect of pre-transplant elevated pulmonary vascular resistance (PVR) on the clinic outcome of heart transplant recipients. Hemodynamic measurement of cardiac performance, valve dysfunction, rejection rates, readmission rates and exercise tolerance will be compared between patients with a PVR >3.0 Wood units and patients with a PVR <3.0 Wood units.
