1. Improve Health Equity

Examine all Community Health Improvement Plan strategies through an “equity lens” to reduce disproportionate impacts

Raise awareness and understanding of health disparities among elected officials and other community leaders

Engage diverse communities in policy initiatives Increase the capacity of diverse populations to participate in

health improvement activities Increase the availability of culturally competent and

nontraditional health workers Improve data collection systems to improve monitoring of health

2. Prevent and Reduce Tobacco Use

Increase the tobacco tax Protect people from exposure to second hand smoke by
