Community Coalitions for Change is a three year cooperative agreement grant funded by the Centers

for Disease Control. The purpose of the project is to build on county-level networks and resources to

conduct evidence-based intervention strategies. Specific strategies will improve physical activity and

nutrition, reduce obesity and prevent and contact chronic disease.

The grant has three primary strategies:

Strategy 1: Provide Education and Promotional Support to Environmental Approaches

Provide community-wide extension and outreach services to children and families to increase

healthy behaviors such as consumption of healthy food and beverages.

Engage with and support community coalitions that support healthy food and beverage consump-

tion and physical activity to prevent obesity and support health

Strategy 2: Implement evidence or practice-based strategies to increase consumption of healthy food

and beverages as recommended by the Dietary Guidelines for Americans

Implement food service guidelines/nutrition standards where foods and beverages are available

Increase access to and promote healthy food retail

Strategy 3: Implement evidence or practice-based strategies to increase opportunities for physical ac-

tivity

Create or enhance, and promote access to safe places for physical activity

Promote joint use agreements

Implement and promote Safe Routes to School or other walk/bike to school programs

Promote Complete Streets or other safe streets/community design initiatives

Community Assets were compiled through the assistance of the UT/TSU Agents and Program Assis-

tants, with input from the C3 Coalitions. This document contains a baseline of assets as of Spring 2015.

We will complete a final asset map upon the completion of the grant.
