MANAGEMENT
•Mild or Absent epithelial dysplasia Influenced by the extent of lesion and oral
subsite
• Severe epithelial dysplasia - surgical excision- cryosurgery- laser surgery - non-surgical treatment : Vitamin A,
Retinoid, beta-carotene, vitamin E, bleomycin, alpha-tocopherol
CAROTENOIDS
•Beta-Carotene• Vitamin A precursor• Found in dark green, orange /yellowish
vegetables• Recommended to prevent oral
leukoplakia + possibly oral cancer• Effects related to antioxidizing action• Better response in smoker patients• Side effects : headache, muscle pain
CAROTENOIDS
• Lycopene• Carotenoid w/o provitamin A action, fat-soluble
red pigment• Found in some fruit + vegetables : tomatoes • Most efficient biological antioxidizing agent• Risk of development cancer + cardiovascular
diseases• Better absorbed in oil resin capsules +tomato
juice• No systemic significant toxical effect +side
effects
VITAMIN
• L-Ascorbic Acid (Vitamin C).• α-Tocoferol (Vitamin E).• Retinoic Acid (Vitamin A)• Fenretinide. Fenretinide (4-HPR) or N-(4-hydroxyphenyl)
• Found in kiwi, strawberries, papaya, mango• 100–120 mg/day for adults(>140 mg/day
for smoker)• pt. low plasma L-AA levels ≤ 25 μmol/l)
Presence petechias, oral leukoplakia and lichenoid lesions• L-AA alone for oral leukoplakia treatment
No efficacy
L-ASCORBIC ACID (VITAMIN C)
• Found in plant oil, margarine, green leaves• 10 mg/day for adult men • 8 mg/day for adult women
Α-TOCOFEROL (VITAMIN E)
• Includes all the natural and synthetic compounds• Found in meat, milk, eggs• Topical retinoids
I. Tretinoin :0.05% gel• Side effects : retinoic dermatitis, cutaneous
irritation, scaling, ardency, pruritusII. Isotretinoin : 1 %
• Systemic retinoidsI. Isotretinoin :0.06 mg/kg/dayII. 13-cRA retinoid :0.06-0.09mg/kg/day• Side effects : muscle pain, dry mouth
RETINOIC ACID (VITAMIN A)
• Systemic retinoids are not indicated in cases • pregnancy or probability of pregnancy• noncompliance with the use of contraceptives• breast feeding• hypersensitivity to parabeno• leukopenia• hypothyroidism (patients using bexarotene)• high levels of cholesterol and triglyceride• hepatic malfunction• renal malfunction
RETINOIC ACID (VITAMIN A)
• Vitamin A analogue• less toxic• Systemic use : 200 mg/day
FENRETINIDE. FENRETINIDE (4-HPR) OR N-(4-HYDROXYPHENYL)
BLEOMYCIN
• cytotoxic antibiotic• 0.5%/day for 12 to 15 days or 1%/day for 14 days• white patch peeled off raw suface was
epithelialized • significant reduction of dysplasia and keratinization• follow-up 3-4 years • 31.6% no clinically visible lesions• 11% malignant transformation
• Side effects : mucocutaneous reactions (stomatitis, alopecia, pruritic erythema, vesiculation of the skin)
PHOTODYNAMIC THERAPY (PDT)
• noninvasive• nonthermal photochemical reaction• photosensitising drug (photosensitiser)• Oxygen• visible light
• damage cell components structural proteins, enzymes, DNA, and phospholipids• Low systemic toxicity• good function and cosmetic outcome• PDT-ALA in topical concentrations from10 - 20%
DIAGNOSTIC PROCEDURES
FOLLOW UP
- No strict duration & frequency guideline- After surgical excision recurrence are not common.
- In general, 6-12 months- In case of non-surgical treatment should follow minimum duration 4 wks. ( size, texture, histopathological finding, changes of degree epithelial dysplasia )
- In non-treated pt., 3 months
