Charting a Course for Tomorrow’s Health Care. Influenced by the rapid changes in today’s health care environment, Montana’s health care leaders are embarking on the task of charting new courses for the delivery of health care services. Major components of the new direction will include: improving population health, improving the patient experience, and reducing health care costs.

To be most successful in this endeavor there will need to be more collaboration and integration among providers. And, as patients and families become more informed and educated about their health and the care they receive, they will become an integral part of the effort to chart a new course.

The future direction of health care will require all of you (health care leaders, providers, and consumers) to come together to navigate today’s challenges, while maintaining focus on the need to meet the expectations of the patient, now and in the future.

This year’s keynote speaker will share his personal experience with a grave medical condition and his efforts to seek the best information with which to make decisions about his treatment. Health

care organizations will benefit from having individuals like him at the table during key discussions to help ensure that patients’ needs are met.

Facilitators for our breakout sessions will address the changing health care landscape and how all health care workers can play a role in addressing the challenges in our future.

Joe Lupica Newpoint Healthcare Advisors

Mr. Lupica pioneered an award-winning community collaboration model to build consensus when not-for-profit or public-sector hospital leaders consider commu-nity objectives and evaluate options. He has also provided guidance to help local hospital leaders stand firm against mergers that would not advance their institu-tions or the communities they serve. In addition to Joe’s experience as an invest-ment banker, attorney and development officer, he served in the Reagan White House as Special Assistant for Intergovernmental Affairs, and as Special Assistant to the Secretary of Housing and Urban Development. During his federal service – as now – he has focused on local and regional issues faced by community officials and the people they serve. He will present his session titled, Weighing the Options: How to Determine the Best Path for Your Hospital at two separate times. He will explore best practices for hospital and system leaders to determine what level (if any) of independence, or interdependence, on that spectrum of affiliation structures will help their organization thrive.

Ms. Adams has practiced in a large variety of health care settings over the last 50 years with emphasis during the last 35 years on home health, hospice and home care. She has also held a wide array of positions including staff nurse, supervisor, administrator, Di-rector of Quality Improvement, and Director of Compliance and Accreditation prepa-ration. In 2009, she opened her own home care consulting and education company and since that time has presented hundreds of in-person, webinar and audio-conference educational sessions on diagnosis coding, OASIS, regulatory, compliance and general clinical issues for individual home care agencies, state associations, and national com-panies primarily across the eastern half of the United States. In addition, Ms. Adams has contributed to a variety of national home care publications and authored resource books and articles related to diagnoses coding and other home care and hospice topics. She has served as a subject matter expert to the Board of Medical Specialty Coding and Compliance since 2008.

Demetria (Demi) Haffenreffer, RN, MBA, has made long-term care her profession since 1973, first as a Director of Nursing and for the last 35 years as a consultant. She is found-er and President of Haffenreffer & Associates, Inc., an Oregon consulting firm supporting skilled and community-based care providers in the delivery of person-centered, compas-sionate care. Haffenreffer & Associates, Inc. provides educational and hands-on assistance with the implementation of quality systems and corporate compliance programs nation-wide. In 2011, Demi assisted the Colorado Foundation for Medical Care with a CMS grant to publish the Model Program for Quality Performance called “QAPI.” Demi is a facilitator for the Leadership Excellence Self-Assessment System and has served on the Oregon State Resident Safety Review Council, the Steering Committee of MOVE (Making Oregon Vital for Elders, an outreach of the Pioneer Network), as a member of Oregon Patient Safety Commission, and as a Master Examiner for AHCA’s Quality Award. She has taught workshops nationally and internationally on a variety of subjects pertinent to long-term care and has authored five policy and procedure manuals. In addition, Demi is retained regularly by nationally known law firms as an expert on regulatory compliance issues.

Dave deBronkart, known on the internet as e-Patient Dave, is the author of the highly rated Let Patients Help: A Patient Engagement Handbook and one of the world’s leading advocates for patient engagement. After

beating stage IV kidney cancer in 2007 he became a blogger, health policy advisor and international keynote speaker. An accomplished speaker in his professional life before cancer, he is today the best-known spokesman for the patient engagement movement, attending nearly 300 conferences and policy meetings internationally,

including testifying in Washington for patient access to the medical record under Meaningful Use.

Eric Coleman University of Colorado Denver Anschutz Medical Campus

Eric A. Coleman, MD, MPH, is Professor of Medicine and Head of the Division of Health Care Policy and Research at the University of Colorado Anschutz Medical Campus. Dr. Coleman is the Director of the Care Transitions Program, aimed at improving quality and safety during times of care “handoffs.” Dr. Coleman serves as the Executive Director of the Practice Change Leaders-a national program to develop, support and expand the influence of organizational leaders who are committed to achieving transformative improvements in care for older adults. Dr. Coleman was recognized with a 2012 MacArthur Fellowship for his work in bridging innovation and practice through enhancing the role of patients and family caregivers in improving the quality of their care transitions. His presentation titled Improving Care Transitions by Infusing True Person and Family Centered Care will be a bonus session on Wednesday September 24 from 8:30 am to 3:00 pm. Additional fees apply. Mr. Eaton has over 10 years of adult education experience specializing in the topics of leadership, management, coach-ing, personal development, and organizational psychology. He is currently a Doctoral Candidate and holds graduate degrees in Psychology and Business. D9 Financing the Future of (Tele) Health NHA

Jonathan Neufeld Telehealth is often discussed as a singular concept involv-ing the delivery of clinical care at a distance. In practice, however, telehealth programs are always implemented in the context of a clinical setting that operates within the existing health care system, and this context determines which of the multiple aspects of the telehealth mode of service delivery are likely to be critical to a program’s

success. This presentation will explore the three major domains in which telehealth programs function— Inpatient and Specialty Care, Integrated Primary Care, and Transitional Care—and the business models most often used to sustain these programs in the con-texts within which they function. Using relevant cost and utilization data along with conceptual diagrams and examples, this presentation will first outline the features of these three major domains, helping partic-ipants understand why it is important to identify the domain in which a given telehealth service functions or intends to function. The presentation will then go on to explore the relevant business models most often used to sustain telehealth programs in these three domains. Each model has critical strengths and drivers that must be recognized and addressed in order for the program to move toward sustainability. Finally, the presentation will briefly review historical factors that led to current practices, and then provide a detailed examination of the current financial and political forces shaping the ways in which telehealth services are likely to develop and provide value to various stakeholders for the foreseeable future. Dr. Douglas WaldoJoin Dr. Douglas Waldo as he presents an overview of Mission: Lifeline Montana. Mission: Lifeline focuses on

saving lives by increasing the number of patients who are rapidly transported to hospitals capable of opening the blocked coronary artery. The most effective treat-ment is PCI (percutaneous coronary intervention), also referred to as heart catheterization or angioplasty. If PCI is not an option, the next line of treatment is the use of fibrinolytic therapy (clot-busting drugs). Currently, 30% of STEMI patients do not receive any therapy. Many more do not receive therapy within the 90-minute time-frame recommended by the scientific guidelines.

The Goal:Through Mission: Lifeline, we will create and help exist-ing statewide system of care to improve the quality of care and outcomes for every STEMI patient. Plans are being developed that provide pre-hospital identification of STEMI-type heart attacks, alert the nearest, most appropriate hospital and activate the catheterization lab to treat the patient immediately upon arrival.
