Admission Agreement Be On Time: Be sure children arrive to camp on time on field trip days, as the bus is on a schedule and cannot wait. Dress Code: NO short shorts are allowed. Children should arrive each day in camp t-shirts and must wear walking / Bermuda shorts. Open toed shoes may only be worn on water field trips. Please make sure your child has an extra pair of shoes in his/her backpack on those days. Closed toed shoes must be worn at all other times. Please apply sunscreen before camp. What To Bring: Sunscreen, complete lunch (meal & drink) and two daily snacks with drink (am & pm). No microwaveable meals please. Provide disposable sack lunch products on field trip days. Some field trips include lunch - pay close attention to Weekly Camp Calendars for details. Pool/Beach Days: Children should come dressed in swim attire and have towel, disposable lunch/snack, sunscreen, hat (optional) and flip flops. Note on days with water activities: We will apply/reapply sunscreen to children periodically throughout the day. Sign-In / Sign-Out: All children must be signed in and out each day by parent or guardian. Prohibited: No cell phones, MP3s, iPods, iPhones, tablets or electronic games allowed at camp, unless instructed by Camp Director(s). Behavior: Children must maintain a courteous manner to counselors and peers. The camp has four rules paramount to overall camp discipline: 1. Respect to God. 2. Respect to others. 3. Respect to property. 4. Respect to self. Disrespect or misbehavior will not be tolerated. Misbehavior or disrespect may result in your child being suspended from camp with no refund.

I understand and agree to the following: Payment Options: Check or Automatic Payment via authorized credit or debit card. No cash will be accepted. All Payments Are Non-Refundable: The $75 non-refundable Registration Fee is due upon registering. Tuition Is Due: Always on a Monday one week prior / in advance to the registered camp week. Payments received after Monday could result in your child's loss of camp for that week. Changes: No changes will be made once I have registered and paid for camp dates. Camp fees are non-refundable. Upon registration all Weekly and Daily Sign Ups are final. There will be no refunds for cancellations and no rescheduling of days and weeks. Feel free to add days or weeks but any additional dates must be requested one week prior to the week of camp wanted. Thank you for your cooperation and understanding. Non-Payment / Late Payments: If I neglect to pay the amount due by the Monday one week prior to camp attendance, (which would also include any late fees incurred during that time), this would result in the loss of camp for my child the week of non-payment. Payments received after Monday of the camp week being paid, may be assessed a $10 late fee. Insufficient Funds (NSF): Any payment returned for non-sufficient funds will be assessed an additional $25 fee & future payments may be required in money order form. Pick Up: If my child has not been picked up by the closing time 6:00 pm, I will be charged $1 per minute for every minute thereafter. We refer to MVCS time/clocks when checking in and out. Crusader Summer Camp closes at 6:00pm. Financial Responsibility: I am financially responsible for the Crusader Summer Camp weekly tuition for the number of weeks that I have reserved, regardless of attendance. MVCS CRUSADER CLUB SUMMER CAMP AUTOMATIC PAYMENT PLAN

Automatic credit card payments for MVCS Crusader Summer Camp are recommended. Payment by check is acceptable. To activate the automatic payment plan, please complete the form below.

AUTOMATIC PAYMENT PLAN TERMS AND CONDITION FOR CREDIT CARDS / DEBIT CARDS: I give permission to MVCS to automatically charge the amount due to my authorized credit or debit card on Monday prior to each week my child is enrolled in / signed-up for camp, unless I clearly communicated to MVCS in writing before 10:00am that Monday prior to the camp date that my child would not be attending. I am also aware that if any changes occur to my child’s attendance, I must and will communicate this in writing to MVCS 7 days prior to the Monday camp date. If my card is declined or does not go through and MVCS is unable to process the charge, my child will not be able to attend Crusader Summer Camp. MVCS will contact me by phone and/or email immediately, which will give me the opportunity to make another payment option, such as providing an updated credit / debit card number.

As parent/guardian, I understand that promotional pictures and videos (individual and group) will be taken throughout my child’s participation at Mission Viejo Christian School Crusader Summer Camp.
