One of many important areas that is crucialfor all of us to focus on is the huge reserveof untapped knowledge in lessons we are yetto learn from Lessons laying latently withinthe organization lessons that we as anorganization are not learning from for eachday that pass Are we able to concretelyidentify the incidents that have occurred onthe unit we are attached to because oflessons unlearned Are you able to recognizethe incidents you have succeeded inpreventing as a result of lessons LearnedThe complexity of our organization and thecriticality of the operations we are engagedin dictate the need for every lesson to belearned and not just some

Several incidents relating to fire injuries ondrillfloor and High Potential Near Misses inthe Saipem Drilling Offshore Divisionhappened partly because of some failure inadequately learning from lessons shared infor example related safety and technicalbulletins

These accidents could have been avoided if lessons were adequately shared and learnedfrom It is most likely that anybody reading this article knows about one or more events thatoccurred somewhere as a result of lessons unlearned

In some countries continuous learning and improvement is so fundamental an expectationfrom the authorities that it has been made a law and not just left to be yet anotherGuideline or Recommendation by an International regulatory body For Saipem through well-established programs like LIHS some fundamental terms like 5 stars Intervention LeadingBehaviors and Supervision have become very familiar terms for its employees The time hascome to expand the companyrsquos core glossary with terms such Continuous Learning andImprovement

Not only should we aim to hold continuous Learning and Improvement as an ideal it must beoperationalised made a deliverable and adequately monitored Everyone must make it partof hisher modus operandi to share learn and improve from their achievements mishaps aswell as that of others and also be able to proof so through agreed methods Nothing must beseen as more indefensible than incidents occurring due to organizational and individualfailure to share learn and improve

A way to entrench the organizational culture of continuous learning and improvement is thereforenecessary to put in place This could come through Corporate and Divisional Guidelines definingand describing methods and processes for the mandatory sharing and learning of lessons in asystematic and traceable way that will lead to improved productivity and above all improvedsafety for Saipem

I joined Saipem in November 2003 working as an Able Seaman onboard Perro Negro 4 I developed my leadership and teamworkskills for the following 9 years to the point I got promoted to Bosun in November 2012 Even now after 8 more years I still take proud in my everyday work striving to be an example for my team and my colleagues through positive conduit safety attitude and willingness to share my experience

What do you like most about your job

I start my day planning together with the Master the tasks needed to be done Morning meeting attendance ndash sharing the daily activities with everyone involved Getting together with my team organizing the work process in line with our safety commitment Carrying on the helicopter operations as HLO Ensuring the job completion dealing with any variation that may occur Best part about the job is being able to work as part of a team that feels like a large family and getting to return home safe and in good health at the end of every hitch

I joined Saipem in November 2003 working as an Able Seaman onboard Perro Negro 4 I developed my leadership and teamworkskills for the following 9 years to the point I got promoted to Bosun in November 2012 Even now after 8 more years I still take proud in my everyday work striving to be an example for my team and my colleagues through positive conduit safety attitude and willingness to share my experience

Describe a typical day at work in your position

What do you like most about your job

I start my day planning together with the Master the tasks needed to be done Morning meeting attendance ndash sharing the daily activities with everyone involved Getting together with my team organizing the work process in line with our safety commitment Carrying on the helicopter operations as HLO Ensuring the job completion dealing with any variation that may occur Best part about the job is being able to work as part of a team that feels like a large family and getting to return home safe and in good health at the end of every hitch

These accidents could have been avoided if lessons were adequately shared and learnedfrom It is most likely that anybody reading this article knows about one or more events thatoccurred somewhere as a result of lessons unlearned

In some countries continuous learning and improvement is so fundamental an expectationfrom the authorities that it has been made a law and not just left to be yet anotherGuideline or Recommendation by an International regulatory body For Saipem through well-established programs like LIHS some fundamental terms like 5 stars Intervention LeadingBehaviors and Supervision have become very familiar terms for its employees The time hascome to expand the companyrsquos core glossary with terms such Continuous Learning andImprovement

Not only should we aim to hold continuous Learning and Improvement as an ideal it must beoperationalised made a deliverable and adequately monitored Everyone must make it partof hisher modus operandi to share learn and improve from their achievements mishaps aswell as that of others and also be able to proof so through agreed methods Nothing must beseen as more indefensible than incidents occurring due to organizational and individualfailure to share learn and improve

I joined Saipem in November 2003 working as an Able Seaman onboard Perro Negro 4 I developed my leadership and teamworkskills for the following 9 years to the point I got promoted to Bosun in November 2012 Even now after 8 more years I still take proud in my everyday work striving to be an example for my team and my colleagues through positive conduit safety attitude and willingness to share my experience

Describe a typical day at work in your position

What do you like most about your job

I start my day planning together with the Master the tasks needed to be done Morning meeting attendance ndash sharing the daily activities with everyone involved Getting together with my team organizing the work process in line with our safety commitment Carrying on the helicopter operations as HLO Ensuring the job completion dealing with any variation that may occur Best part about the job is being able to work as part of a team that feels like a large family and getting to return home safe and in good health at the end of every hitch

Organizational Learning Emphasis on The Unlearned Lessons

These accidents could have been avoided if lessons were adequately shared and learnedfrom It is most likely that anybody reading this article knows about one or more events thatoccurred somewhere as a result of lessons unlearned

In some countries continuous learning and improvement is so fundamental an expectationfrom the authorities that it has been made a law and not just left to be yet anotherGuideline or Recommendation by an International regulatory body For Saipem through well-established programs like LIHS some fundamental terms like 5 stars Intervention LeadingBehaviors and Supervision have become very familiar terms for its employees The time hascome to expand the companyrsquos core glossary with terms such Continuous Learning andImprovement

Not only should we aim to hold continuous Learning and Improvement as an ideal it must beoperationalised made a deliverable and adequately monitored Everyone must make it partof hisher modus operandi to share learn and improve from their achievements mishaps aswell as that of others and also be able to proof so through agreed methods Nothing must beseen as more indefensible than incidents occurring due to organizational and individualfailure to share learn and improve

I joined Saipem in November 2003 working as an Able Seaman onboard Perro Negro 4 I developed my leadership and teamworkskills for the following 9 years to the point I got promoted to Bosun in November 2012 Even now after 8 more years I still take proud in my everyday work striving to be an example for my team and my colleagues through positive conduit safety attitude and willingness to share my experience

Describe a typical day at work in your position

What do you like most about your job

I start my day planning together with the Master the tasks needed to be done Morning meeting attendance ndash sharing the daily activities with everyone involved Getting together with my team organizing the work process in line with our safety commitment Carrying on the helicopter operations as HLO Ensuring the job completion dealing with any variation that may occur Best part about the job is being able to work as part of a team that feels like a large family and getting to return home safe and in good health at the end of every hitch

During drilling operations the subsea riser issubject to external forces due to vesselmotion amp subsea currents and internal forcesgiven by the mud flow in the riser

It is common practice to use a risermonitoring system which can estimate theriser state and communicate it to an on-board control unit to help engineers makingthe right decisions

Knowing the shape of the drilling riser stringis of primary importance to guarantee safeoperations and avoid disconnection to thewellhead

NEPTUNE SYSTEM is an innovative systemmade by software + hardware for the livereconstruction of the riser

I joined Saipem in November 2003 working as an Able Seaman onboard Perro Negro 4 I developed my leadership and teamworkskills for the following 9 years to the point I got promoted to Bosun in November 2012 Even now after 8 more years I still take proud in my everyday work striving to be an example for my team and my colleagues through positive conduit safety attitude and willingness to share my experience

Describe a typical day at work in your position

What do you like most about your job

I start my day planning together with the Master the tasks needed to be done Morning meeting attendance ndash sharing the daily activities with everyone involved Getting together with my team organizing the work process in line with our safety commitment Carrying on the helicopter operations as HLO Ensuring the job completion dealing with any variation that may occur Best part about the job is being able to work as part of a team that feels like a large family and getting to return home safe and in good health at the end of every hitch

I joined Saipem in November 2003 working as an Able Seaman onboard Perro Negro 4 I developed my leadership and teamworkskills for the following 9 years to the point I got promoted to Bosun in November 2012 Even now after 8 more years I still take proud in my everyday work striving to be an example for my team and my colleagues through positive conduit safety attitude and willingness to share my experience

Describe a typical day at work in your position

What do you like most about your job

I start my day planning together with the Master the tasks needed to be done Morning meeting attendance ndash sharing the daily activities with everyone involved Getting together with my team organizing the work process in line with our safety commitment Carrying on the helicopter operations as HLO Ensuring the job completion dealing with any variation that may occur Best part about the job is being able to work as part of a team that feels like a large family and getting to return home safe and in good health at the end of every hitch

I joined Saipem in November 2003 working as an Able Seaman onboard Perro Negro 4 I developed my leadership and teamworkskills for the following 9 years to the point I got promoted to Bosun in November 2012 Even now after 8 more years I still take proud in my everyday work striving to be an example for my team and my colleagues through positive conduit safety attitude and willingness to share my experience

Describe a typical day at work in your position

What do you like most about your job

I start my day planning together with the Master the tasks needed to be done Morning meeting attendance ndash sharing the daily activities with everyone involved Getting together with my team organizing the work process in line with our safety commitment Carrying on the helicopter operations as HLO Ensuring the job completion dealing with any variation that may occur Best part about the job is being able to work as part of a team that feels like a large family and getting to return home safe and in good health at the end of every hitch

I joined Saipem in November 2003 working as an Able Seaman onboard Perro Negro 4 I developed my leadership and teamworkskills for the following 9 years to the point I got promoted to Bosun in November 2012 Even now after 8 more years I still take proud in my everyday work striving to be an example for my team and my colleagues through positive conduit safety attitude and willingness to share my experience

Describe a typical day at work in your position

What do you like most about your job

I start my day planning together with the Master the tasks needed to be done Morning meeting attendance ndash sharing the daily activities with everyone involved Getting together with my team organizing the work process in line with our safety commitment Carrying on the helicopter operations as HLO Ensuring the job completion dealing with any variation that may occur Best part about the job is being able to work as part of a team that feels like a large family and getting to return home safe and in good health at the end of every hitch
