New as of January 06, 2006.} Maintenance (Recare) patient: a patient who completed treatment in our facility and is returning after 3-6 months. You do not need to be the original clinician. *ASA PHYSICAL STATUS CLASSIFICATION SYSTEM* ASA I: Normal, healthy patient. No organic, physiologic, or psychiatric disturbance; excludes the very young and the very old; healthy with good exercise tolerance. ASA II: Patients with a mild systemic disease or a risk factor for a systemic disease, for example, tobacco use, alcohol abuse, mild obesity. No functional limitations; has a well-controlled disease. Patients who may be categorized as ASA II are those with controlled hypertension or type 2 diabetes without systemic effect, cigarette smoking without chronic obstructive pulmonary disease (COPD), mild obesity, pregnancy. * Medical Emergencies Essentials for the Dental Professional, 2nd Edition, Ellen B. 3- Assess-confirms level of readiness to quit 4-Assist-appropriate resource materials are selected, referral options are discussed

5-Arrange -schedule follow-up contact, preferably within the first week after the quit date.

6-For those who are not interested in quitting, one or all of the 5As are attempted

LOCAL ANESTHESIA: Mental Block Injection Procedure Student Name:__________________________________ Date:______________________ Patient Name:__________________________________ Instructor Signature:___________________ Competency: Provide each patient with personal dental hygiene care including assessment, diagnosis, planning, implementation and evaluation to achieve and maintain oral health and wellness. Goal: By following the stated protocols, when provided with the appropriate materials, the student will demonstrate the proper technique for administering a mental block injection.

Procedure Errors

Does the student: 1 2 3 Assume the correct operator position (Horizontal approach, Right-handed: 8 or 9 o'clock, Left-handed: 4 or 3 o'clock Vertical approach, Right- or Left-handed, 12 o’clock or 1 o’clock)?

Asked the supine patient to open their mouth and then retracted the lower lip, pulling the tissue taut?

Located the mental foramen by placing cotton tip applicator in the depth of the mucobuccal fold in the area of the mandibular first molar and then palpated anteriorly until a depression is felt?

Prepared the alveolar mucosal tissue in the vestibule anterior to the mental foramen in the depth of the mucobuccal fold?

Communicate with the patient as topical anesthetic is placed and during the injection?

Using a 25/27-gauge short needle in the syringe in the dominant hand, oriented the bevel of the needle away from the large window of the syringe to ensure bevel orientation toward the bone?

Stabilize/fulcrum the syringe? *Place the syringe with the large window facing the operator? Horizontal Approach: Direct the syringe from the anterior of the mouth to the posterior in a horizontal manner? Insert the needle to the depth of the mucobuccal fold, directing the needle anterior to the mental foramen?

Vertical Approach: Establish a fulcrum using the thumb of the retraction hand, then directed the syringe vertically with the patient’s cheek, insert toward the injection site anterior to the mental foramen?

* Verbalize Aspiration within two planes?

If negative aspiration, slowly deposited approximately 0.6 mL (one-third of cartridge) over 30 to 60 seconds? (Ensured that the anesthetic was not injected too rapidly?

*Carefully withdrew the syringe and immediately recapped the needle using a recapping device?

Rinsed the patient’s mouth and waited approximately 2 to 3 minutes until anesthetic took effect before starting treatment?

Maintain the “aseptic chain” throughout the competency? Accurately document administration of local anesthetic including utilization of topical, type and amount of anesthetic, type of injection, area(s) of administration and patient response.

Procedure Errors

Does the student: 1 2 3 Assume the correct operator position depending on the area anesthetized?

Ask the supine patient to open their mouth and then retract the lip, pulling the tissue taut?

Communicate with the patient as topical anesthetic is placed and during the injection?

Using a 25/27-gauge short needle in the syringe in the dominant hand, oriented the bevel of the needle away from the large window of the syringe to ensure bevel orientation toward the bone?

Stabilize/fulcrum the syringe? *Place the syringe parallel with the long axis of the tooth with the large window facing the operator?

Insert the needle at the height (or depth) of the mucobuccal fold, approximately one–fourth the length (approximately 5 mm) or until the bevel is slightly superior (or inferior) to the apex of the tooth?

* Verbalize Aspiration within two planes? If negative aspiration, slowly deposited approximately 0.6 mL (one-third of cartridge) over 30 to 60 seconds? (Ensured that the anesthetic was not injected too rapidly?
