Tony Kuhlman’s CSS Premachining team surprised him with a re-tirement celebration. In addition to his cake, he was presented with a “Participation Award” by the area engineers!

It’s About More Than Coordination.

We All Can Slip, Trip or Fall!

Slips, trips and falls are some of the lead-ing causes of injuries on the job and at home. At GM, Fall Hazards – falling from heights and lower or same levels – are the top trending sentinel event. Remember, sentinel events are accidents or unsafe conditions that have the potential to be fatal. GM has recorded 641 Fall Hazard sentinel events through mid August 2019.

We are all at risk, and the consequences range from sprains that make simple tasks tough, to broken bones that put us out of action, or worse. Reducing the risks is in our control.

Pre-task planning and preparation are the best ways to avoid many fall hazards. When working from heights, let’s be sure to use the correct Personal Protection Equipment (PPE) for the job, and to an-chor properly. Be sure you are trained in the Fall Hazard Control program and nev-er work alone. And let’s work together to make sure pits and open spaces are properly lit and barricaded, including af-ter hours.

That’s only part of the story, though. We don’t need a hazard to fall — people trip over their own feet all the time. The risk is increased when we walk while distracted, such as walking and texting or while hav-ing an intense conversation. Walking too quickly can be a hazard, especially when tired. Or when we take the stairs and don’t use the handrail.

There are hundreds of common hazards that contribute to slips, trips and falls. Our goal is “no injuries” -- a goal we can achieve if we take it one day and one task at a time. Start today with a commit-ment to not get injured. Then, make a new commitment tomorrow.

And when you see colleagues increasing their safety risks, stop and talk to them about your concern. A helpful suggestion to a coworker or family member might save them from injury -- or worse.

PLANT SAFETY REVIEW BOARD/August 28, 2019SAFETY UPDATE Reviewed safety chart for July final. It has been 3 years, 6 month and 26 days since we lost our team member. -Reviewed Sentinel Event Tracking. At 13 YTD. Had 2 in January, 3 in February, 3 in March, 1 in April, 1 in May, 1 in June and 2 in July. -Reviewed total injuries through July. Had 47 injuries, 46 near misses, 1 LWD and 0 PDI’s. -Reviewed the 2019 Defiance Injury “Deep Dive” chart. The high-est incidents were Sprain/Strain at 15, Contusion/Abrasion at 9, Non-Injury/Illness at 6, Exposure at 4, Foreign Body at 4, Dermati-tis at 4 and Hearing Loss at 2. -Reviewed the Green Cross chart for August. Have 2 recordables, 6 first aid cases, 5 near misses and 1 lockout violation. EMPLOYEE SAFETY CONCERNS-Air quality Finishing: They are running both units. The duct work rebalance is complete. Employee is ready to sign off after the fan install is complete. -Air quality Core Room: Employee would like to observe the area once they get the fan in secondary running. -Water coolers: Reviewed new cleaning schedule and identifica-tion system. EE wants to monitor effectiveness before signing off. -Robot: Have process in place when changing EOAT. The employ-ee on sick leave. Review with him when returns. -Air flow: They added duct work. Vent cut off shortening the duct work and welded a plug on the end to provide more directed air to the operators. Next step add another fan. -Sanitary lift station: Reviewed current plan to correct ESWP is-sues the week of September 9. Will start the decommissioned equipment by the end of the 3rd quarter. Asked for money for a new sanitary lift station. OPEN H&S PM’S July is at 100%. At around 90% for August. Will get these completed by Saturday. GLOBAL CALL TO ACTION STATUS Open: 18-20: Working on sur-vey. Identified 13 to 14 platforms. 19-08: Working on this with Saginaw. Received the clarification on this. WORKPLACE SAFETY SYSTEM Reviewed 4.1 Monitoring and Measurement/current overview. -Highlights of Element: oDevelop a process to track all GM required metrics that show both leading and lagging indicators.-Evidence of implementation:oLeadership’s Monitoring and Measuring of Safety Data•Level 1 Quad Charts: Fatality, # Recordables, # Near Misses, # Lost Work Days, Sentinel Events, Pareto of Injuries•Level 1 BPD & PSRB: Fatalities, PDI’s, Recordable Rate, # of Re-cordables, # of Lost Work Day Cases, Rate of Lost Work Day Cases•PSRB & Weekly Staff meeting: Call to Actions and Sentinel Events closed on time.Reviewed Performance Standard 13 Chemical Exposure Assess-ment and Control. Dr. Eby is the champion, Carl is the salary sup-port and Cliff is the UAW support. Reviewed current overview:-Highlights of Performance Standard: Industrial Hygiene, Health Center, Incident Investigation, Emergency Response Plan-Leading Indicators for 2019: Sample plan additions/results, Med-ical evaluations for overexposures (7 YTD)-Evidence of implementation of all departments and shifts: SOT deficiencies, IH sample plan covers all shiftsReviewed Performance Standard 16 Facility Safety Management. Reviewed the current overview:-Highlights of Performance Standard: Drinking water, Health Cen-ter, Elevators/Escalators, Piping Systems, Asbestos, Biological con-tamination of water systems- Evidence of implementation of all departments and shifts:

Converting existing procedures and processes-Improvement/upcoming changes for next 12 months: Complete written program, Identify gaps and establish, CAP’s to correct deficiencies.H&S TRAINING UPDATE Reviewed the training update. -ESWP – Have 15 red and 30 yellow. Get classes scheduled.-Rigging – Have 5 red and 4 yellow. -Fork truck – Have 1 red and 16 yellow. Nine are scheduled for training in the next week. -First Aid & CPR (Elect) – Have 9 red and 4 yellow. Four are sched-uled for training in the next week. -Respirator fit – Have 33 red and 60 yellow. Twenty are scheduled for training in the next week. Thanked everyone for their help with this. -Roof – Have 20 red and 28 yellow. Twelve are scheduled for training in the next week. -Reviews this weekly in the OPS meeting. Asked if we could get all 93 people trained in respirator fit within the next 2 weeks. Will work out a plan. - Defiance has been put back on lockout probation since a lockout violation occurred 8/1/19. Will be required to perform lockout audits each quarter for 12 months.ERGONOMICS UPDATE Reviewed Ergonomics for 2019. Zero QRP’s in Medical, from the floor, and open issues. INDUSTRIAL HYGIENE UPDATE Reviewed the 2019 sampling plan. Total of 281 samples taken. Have 27 results pending. Reviewed August action plans: - Precision Sand core machine deep clean – Silica & MDI (Isocyanate), Precision Sand raw sand and return sand belt areas – Silica, SPM coremakers – Sulfur Diox-ide, Mod 2 Op 20 personal samples – DMIPA, Petroleum Distil-lates and Naphthalene, SPM new resins – Trimethylol Propane Triacrylate, Formaldehyde, Continue DMIPA core machine area samples as needed, Continue any additional area and personal sampling as neededFOLLOW-UP Reviewed follow-up items from the PSRB meeting on 8/14/19.-Visitor orientation Working to update our video. -Roof integrity near Plant 1 ladle repair – The plan is to not put this unit in service this winter. Will barricade the roof access.-Add to Sentinel Event for Timcor mowing incident: Greg to fol-low-up with Ed & Steve on making the conduit neater, Working on conduit for additional corrugated tubing, Added barricade blocks to protect the panels and wiring, Weed mowing complete, need to put stone down -Lockout probation and lockout refresher requirement – Schedule is to have SafeStart “Mind on Task” 2-hour training followed by 1-hour lockout refresher in September.-MVV cooler cleaning TIS provided, quarterly report out to PSRB.-Gate intercom not working – Security got working after PSRB on 8/14/19. Sytem to monitor, waiting for the mic to come in. -New safety green lock communication:oSafety performed some testing, locks did not hold up. Working with Global Safety and Masterlock to address. oInitial feedback from Masterlock - bad fusion welding batch and will replace all locks. More to come. oLaminating the pictures on the locks. -Reviewed the current SafeStart schedule for September. Kip will be in the plant Tuesday’s, Wednesday’s and Thursday’s. ROUNDTABLENeed warning tags with trucks. Yesterday a truck didn’t have the proper tag. The tag is a 2-part tag. Will test new locks in heat and places where they think they might not work. We can use the old locks as long as they work.
