Pursuant to DO 88, s. 2010 re Revised Manual of Regulations for Private Schools in Basic Education and DO 4, s. 2014 reAdoption ofModified School Forms for Elementary and Secondary Schools, this office announces the monitoring of the Division M&E Field Technical Assistance Team for Private Schools for the SY 2019 -2020.
2. Attached is the schedule of monitoring and evaluation. Likewise, the tool attached should be accomplished and be presented on the day of evaluation for validation purposes.

MONITORING AND EVALUATION TOOL FOR PRIVATE SCHOOLS (Pursuant to DO 88, s. 2010, DO 4, s. 2014)
This Monitoring and Evaluation Tool is basically designed to assess the extent of compliance with the DepEd Minimum Quality Standards as well as to evaluate the school's characteristics, its qualitative traits of excellence, and its level of performance. This is a quality assurance tool for private schools aimed at building effective and high performing schools through continuous focuses improvement efforts. It guides both the school and certifiers in evaluating the different areas to achieve quality student performance and organizational effectiveness.
Rating Scale and Compliance Status The rating of each statement must be based on verifiable facts and evidences, interviews with concerned stakeholders, and a rater' s personal observations, assessment and evaluation. Each statement shall be rated on a scale of 0 to 4 with 1 being the least desirable state and 4 being the most desirable condition. The 0 indicates that minimum standards Is not complied with at all.

How to Compute? 1. The Area Mean: Add the ratings of all the statements in the area. Divide the sum by the number of
answered items. 2. The Overall Mean: Multiply each area mean by the respective area weight (refer to the "Areas" and
their equivalent "Weights" in page 1). Add up these products and divide the sum by 50.
Directions: Based on documentary analysis, interviews with concerned stakeholders, and one' s personal observations and evaluation on the indicators (Column 1), encircle the number that best describes the extent of compliance (Column 2) with DepEd Standard, N/A if not applicable. Use the comments/remarks box (Column 3) for any quantitative details, if necessary.
