Assignment Status Includes:1. Assignment Category

2. If this is a temporary assignment, include an assignment end date where the red arrow is below

Payroll Information includes: Work Hours (i.e. number of hours the employee will be scheduled to work), Salary Basis, Salary Actual Pay Amount, and Payroll Name, If this is a commission employee

Assignment Details: Please reference your Store data sheet with details for your Organization, Location, and Door number. Information such as Job title and Grade will change depending on the type of hire.

Returning New Hire Documentation Paperwork

Manager may sit down with the candidate to review packet information.

Candidate reads the Code of Conduct, reviews and signs required new hire packet pages and completes state tax form. The Manager either collects required pages including state tax form, or candidate submits on their own.

Manager sends completed New Hire form and required candidate paperwork to Employee Services.

Completed packets can also be mailed to (with physical signatures only):

Tip: The candidate can submit all paperwork electronically or the Manager may choose to scan the completed packet pages with physical signatrues, and send via e-mail directly to Employee Services

*Not e: If information is not returned together onboarding may be delayed. If anything is missing, EmployeeServices will follow up directly with the hiring manager and it will be the manager’s responsibility to follow up with

the candidate.

Instructions to return completed New Hire Packet:

• Packet includes bookmarks for each section, for easy navigation

• Auto-Population feature after the candidate provides field once for:− Name, DOB , SSN

• Electronic Signature capability now exists in fillable PDF format

New Hire: Helpful Tips

• Please send completed New Hire form to Employee Services at least two weeks prior to the desiredstart date

• If an employee is transferring to/from a different brand within ELC, or changing employment status(i.e. Freelance > PT/FT) this would be a Job Data Change and not considered a new hire.

For example: • If a freelance/on-call employee is moving to a PT/FT role, this would be a Job data

Change completed in Manager Self-Service (MSS)

• To expedite New Hire processing send complete, accurate and signed forms to Employee Services:– New Hire Form

Note: Please identify if this new hire had previously worked for ELC on the New Hire Form(Rehires will receive adjusted service date)

– GIS Pre-Employment Authorization Forms (If required – PT/FT Regular employees only)– I-9 Form

• You will receive a confirmation from HR Connect when your inquiry is received, and when the NewHire has been processed, providing you with the new Employee ID number

• Employee Services will keep you posted on the status of your submitted New Hire and follow-up ifanything is missing

• Reference your “HR Connect Store Details Sheets’ with specific data or required fields for your location

• The fillable New Hire Packet can be completed and returned by the candidate electronically orthe Manager can also scan the pages with physical signatures and submit to ES via e-mail
