A year ago I moved across country to Cleveland, Ohio. We had to nd a new internet/cable/phoneservice to use, so we selected one and had to have it installed into the temporary rental we moved into.When the technician arrived, we were made aware that our home was not properly set up for theirservices, and he would have to add a cable from the outside that comes into the inside, as well as add a back-up power supply for the phone, to be used when we might lose power. All was taken care of, andfor the past year, we used the services with no issues. A year later, just a few weeks ago, we moved to a new home, and of course had to have all of ourservices moved as well. We called the service provider, and scheduled a time for a technician to come
out and re-install into our new home. At this time, we were told to be sure to disconnect all hardware, and bring it to the newhome. We did this, and were ready with the technician arrived.
After a few bumps in the road with scheduling and miscommunication within theservice company, we nally got a technician arrive and told us we didn’t have tobring the back-up power supply to our new home, because it wasn’t needed. Wemoved forward with the installation and a few hours later were connected to theworld. The technician left our home, and we realized we still had this 10lb back-up power supply sitting in our tv room. My husband grabbed the unit, and ran out
o the technician’s van. He told us that he didn’t need the unit, and that we couldkeep it. Of course, we didn’t want the 10lb battery unit, and have no use for it,so insisted that he take it, but he told us that he doesn’t take it back, that we cankeep it, and if we don’t want it to just discard it ourselves.
Of course, my Reverse Logistics brain set off, and I was very confused as to why this service provider was not re-using theunit, or refurbishing the unit for other use, or discarding it themselves if they no longer needed it. Since this group has been amember of RLA, I contacted the Director of Reverse Logistics about the issue – and inquired about the unit, relaying the story.Fortunately, I got the response I was hoping for: “We had been collecting those BBU’s and recycling them at our Fort Worthfacility for reuse so I’m not sure why the technician gave you that direction. Please work directly with [Name removed] andName removed] copied on this mail.”
I was so happy to see this response, to see that this Service Provider has taken the proper steps and already taken this unit intoconsideration within Reverse Logistics. It seems there was a lack of communication or error on the side of the technician,which will get worked out. However, in whatever business you are in, we always want to remember that Customer Service isa part of Reverse Logistics – and each Reverse Logistics department needs to take into consideration what our customers aredoing with our “unneeded” parts, and consider making it our responsibility as well.

Neste artigo relatamos como alguns hospitais, naidade de São Paulo, têm lidado com os resíduosuímicos perigosos gerados nos processos diagnósticos,erapêuticos ou de pesquisa. Nosso objetivo é discutir,m que medida modelos e técnicas de gestão da cadeia deuprimentos e, em especial, as noções de logística reversa,êm sido percebidos e empregados como oportunidade de
melhoria no gerenciamento da disposição desses resíduos,seja na redução de custos, no marketing ambiental ou noatendimento às exigências legais. Mesmo reconhecendoque o emprego da logística reversa na gestão de resíduosé ainda hoje no Brasil uma prática pouco estruturada emuitas vezes até informal, consideramos alguns estudosque têm destacado as vantagens de uma abordagem
sistêmica, em que a geração dos resíduos, em qualquerestágio da cadeia, é entendida como parte do processo de produção.
Analisando os planos de gerenciamento de resíduos devinte hospitais de São Paulo, constatamos que apenas seisdemonstraram identicar corretamente os vários tiposde resíduos químicos perigosos gerados. Ao contráriodo que geralmente ocorre com os RSS (Resíduos deServiços de Saúde) comuns ou infectantes, cada tipo deresíduo químico se origina de um processo especíco,isso possibilita maior facilidade na identicação eanálise, em separado, de cada um desses processos,considerando as condições típicas da cadeia de produtose serviços envolvida na sua geração dos resíduos, como por exemplo: resíduos do setor de radiologia, da farmáciaou do laboratório de análises clínicas. O PAPEL DA LOGÍSICA REVERSAA logística reversa é, ainda hoje, considerada pormuitos apenas com um recurso para retorno demercadorias aos fabricantes, motivado por recalls ou pordefeitos cobertos por garantia. Uma outra modalidadetradicional da logística reversa tem sido o retorno deembalagens de transporte ou recipientes retornáveis,quando seu emprego ainda se apresenta como umaopção economicamente interessante. Nenhuma dessasaplicações tradicionais, no entanto, se fundamentana busca da maior sustentabilidade dos processos de produção ou consumo.

3) recondicionamento, remanufaturamenrenovação de produtos;

dessas cadeias especicamente, mas discutir aspectosgerais da gestão dos designados genericamente comoresíduos de serviços de saúde ou RSS químicos comênfase nas perspectivas e condicionantes das aplicaçõesda gestão da cadeia de suprimentos e da logística reversacomo ferramentas que viabilizem soluções de reduçãode custos e de impactos ambientais e, dessa maneira,contribuam para tornar a produção e consumo maissustentáveis no setor hospitalar.
2. CLASSIFICAÇÃO E RISCOS DOSRESÍDUOS DE SERVIÇOS DE SAÚDEA assistência hospitalar é uma atividade potencialmentepoluidora sob vários aspectos. No entanto, os resíduossólidos gerados nos
hospitais e outrose s t a b e l e c i m e n t o s
similares, têm-se
destacado enquanto
preocupação dasautoridades ambientais e
sanitárias, motivando aedição de regulamentosespecícos como asResoluções do ConselhoNacional do MeioAmbiente (CONAMAResoluções 05/1993 e283/2001) e da AgênciaNacional de VigilânciaSanitária (ANVISARDC 33/2003). Embora essas resoluções priorizassemoriginalmente a regulamentação do manejo dos resíduosde natureza infectante, especícos dos serviços desaúde, o aprofundamento dos estudos para classicaçãodos RSS destacou a importância de um outro grupo deRSS: os que apresentam riscos de natureza química.Embora gerados em quantidade menor, os RSS químicosapresentam, relativamente aos demais, maior potencialde dano ao meio ambiente.
Os resíduos sólidos gerados por serviços de saúde,assim como já vem ocorrendo com os resíduos deoutras indústrias com necessidades especícas, foramagrupados segundo uma regulamentação própria edenominados genericamente de Resíduos de Serviçosde Saúde. Essa denominação é utilizada nas normas elegislações mais recentes, em substituição ao antigotermo “lixo hospitalar”.
Dada a complexidade dos processos desenvolvidos naassistência a saúde, uma grande diversidade de resíduossão gerados em quantidades variáveis, conforme asatividades realizadas no estabelecimento de origem.
Além disso, reete-se na composição dos resíduos,quali ou quantitativamente, o porte da unidade, astecnologias empregadas e os recursos disponíveis, dessaforma, um mesmo produto ou serviço pode envolvera geração de diferentes resíduos conforme o modo de produção, a tecnologia e os materiais empregados, como por exemplo: se o processo é realizado manualmenteou é automatizado, se utiliza material descartável oureutilizável ou se a tecnologia empregada é mais antigaou recente.

mencionadas é o tipode risco que cada
grupo apresenta.S i m p l i f i c a d a m e n t edivisamos quatro grupos principais de RSS: GrupoA (GA) – Infectantes;Grupo B (GB) –Químicos; Grupo C (GC) – Radioativos e Grupo D(GD) – Comuns. Os três primeiros são resíduosque exigem cuidados
especiais e o quarto segue recomendações equivalentesàs aplicáveis aos resíduos domiciliares. De acordocom o enquadramento do resíduo num grupo de riscoserão denidas as medidas de segurança no seu manejoe as alternativas de tratamento e disposição nal maisadequadas e legalmente aceitas.
Os resíduos GB – Químicos são certamente os que
apresentam maior diversidade de riscos. Incluídosnesse grupo, podemos encontrar, entre outros, resíduostóxicos, resíduos inamáveis, reativos ou explosivos eresíduos farmacêuticos. Os resíduos tóxicos têm grande potencial de contaminação ambiental, envolvendo prejuízos para a saúde humana e dos demais seres vivos.Os inamáveis, reativos ou explosivos, além dos riscosambientais, exigem maiores cuidados para preveniracidentes nas etapas de estocagem e transporte. Para facilitar a realização deste estudo, os RSSQuímicos foram divididos em apenas quatro sub-grupos. A classicação simplicada adotada foidesenvolvida a partir do Regulamento Técnico proposto pela RDC 33/2003 da ANVISA (ANVISA, 2003) queoriginalmente adota oito sub-grupos. Devemos levarem consideração que a resolução da ANVISA aindanão havia sido publicada na época em que os hospitais
avaliados elaboraram os PGRSS. A classicação queutilizamos, e sua correspondência com a classicaçãoda ANVISA, são apresentadas na Tabela 3.
5. RESULADOSA Tabela 4 apresenta a distribuição das unidadesconforme o nível em que foram classicadas. Atravésda coluna que apresenta a proporção média de resíduosquímicos no total de RSS gerados, podemos vericarclaramente que nas unidades onde a identicaçãodos RSS do Grupo B foi considerada detalhada, a participação média dos resíduos químicos no total foisensivelmente maior. Os resultados obtidos reforçam ahipótese de que hospitais que não incluíram nos seusPGRSS sistemas de identicação, tais como: inventáriosde resíduos, identicação de processos geradores enormas de procedimento, tendem a registrar menoresquantidades de resíduos químicos, ou mesmo nenhumresíduo nessa categoria, embora realizem, em linhasgerais, os mesmos processos que os demais. Sem adevida identicação esses resíduos são classicadoscomo comuns ou infectantes e receberão destinaçãoincorreta, no sistema de limpeza urbana ou em aterrosclandestinos.
O desenvolvimento tecnológico na produção de bens eserviços gerou um impasse que hoje se apresenta comoo desao da sustentabilidade e da superação dos graves problemas ambientais e sociais da atualidade. Um dosaspectos mais graves dessa questão é o crescimentodos níveis de contaminação ambiental por produtos
químicos disseminados por quase todo o planque se evidencia na poluição das águas, do solar pelo lançamento irregular de euentes e emou disposição de resíduos tóxicos. Reverter atendência de agravamento dessa situação requer ee mobilização tanto dos setores produtivos comgovernos e da sociedade na articulação de soluçõenvolvem mudanças de comportamento, de valor
prioridades.Os exemplos que discutimos sugerem deciêncgerenciamento de RSS nos hospitais analisados de muitas das questões emergentes referentes à de materiais e processos já estarem presentes,que em forma embrionária. Entre abril de 1999 a novembro d e 2001, a Secretaria doMeio Ambiente do Município de São Paulo desenvolveuum projeto piloto de implementação do PGRSS – Planode Gerenciamento de Resíduos de Serviços de Saúdeque envolveu cerca de 20 hospitais. Esse projeto seapoiou nas diretrizes da Portaria SMA 102/99, criadaespecicamente para denir os critérios de elaboraçãodos PGRSS no âmbito daquela Secretaria.
Um primeiro grupo foi denido visando a obtenção deuma amostra sobre as condições de g eração de RSS noshospitais da Cidade de São Paulo. Para composiçãodesse grupo, foram selecionados doze hospitais, sendometade público e metade privado. Em cada uma dessas
metades, foram escolhidos, aleatoriamente, em uma lista
de todos os hospitais da cidade, dois estabelecimentosde cada porte, ou seja, pequeno, médio e grande porte.A denição de porte usou como base o número de leitos.Um segundo grupo de oito hospitais foi convocado aapresentar o PGRSS devido a denúncias ou reclamaçõesapresentadas pelo Departamento de Limpeza Urbana daPMSP.
Conforme podemos observar na Tabela 1, a amostraanalisada abrange 10,75% dos 186 hospitais em operaçãona Cidade de São Paulo e cerca de 26,43% dos leitosdisponíveis.
Os nomes, a localização, assim como outras informaçõesque permitissem a identicação dos hospitais pesquisadosforam omitidas.

2. Rating and Risks of Waste in Health Services
The Hospital Assistance is an active potential po lluter inmany ways. However, the solid waste generated in thehospitals and other similar establishments, have beenhighlighted by environmental authorities, motivating
the creation of specics regulations as resolutionsof (“National Council of Environment”) NationalSanitary Surveillance (RDC 33/2003). Although theseconclusions prioritizes the regulation of infectious waste,specically in health services, the deepen of studies toclassify RSS (Waste of health services) highlights theimportance of another group within it: the ones whooffer risks in chemical nature. Although generated in less
quantity, the RSS (Waste of health services) representsmore risks to the environment.
The solid waste generated by health services was
grouped following their own regulation and they arecalled RSS – Health Services Waste.
The quantity of waste is generated in variable quantities
because of the complexity of the developed process in
the health area and as the activities made in the hospi talsas well. Although, it reects the composition of wastein quality and quantity and it depends on the technologyand resources available. In this way the same productor service can involve the generation of different kinds
of waste following the production way, the technologyand material used on it as an example: if the process isrealized manually or automatically, it uses disposablesor reusable or if the technology used is older or not.
The main criteria used in the classications is the typeof risk that each group represents. There is (in a simpleway) 4 main g roups of RSS(Health Services Waste):

4. Analysis o the situationThere are no indicators that have an evaluation of the
environmental impacts by the RSS and there are somedifculties to development studies about the problemsgenerated by management the RSS (in its generation,transportation and nal disposal stages)
Between April of 1999 to November 2001 , theEnvironmental Secretary of São Paulo City developedone pilot project with the implementation of the PGRSS(Plan of Waste Health Services Management) thatinvolves about 20 hospitals. This project was supported by the law SMA 102/99, created specically to dene thecriteria of the PGRSS((Plan of Waste Health ServicesManagement).
The rst group was dened by aiming the acquisition of
one example of generation conditions in RSS ((Wasteof health services) in hospitals in São Paulo City. Forthe composition of these groups, 12 hospitals had beenselected (middle public and middle private). Each ofthese were chosen at random, within a list of all the city

hospitals. Two of them in any size (small, medium andbig ports) based on the number of beds.

To facilitate the reality of this study, the Chemical RSS
was divided into four subgroups.
The simplied classications adopted was developed based on the Technical Regulation proposed byRDC 33/2003 of Anvisa (Anvisa, 2003) that originallyadopts 8 subgroups. We should consider that the Anvisaresolution was not publicized yet in the time that theassessed made their PGRSS ((Plan of waste healthservices management). The classication that we usedare presented in the table 3 :

The results strengthen the hypothesisof the hospitals that have not included in their PGRSSsystems as identication, as : waste inventory, processgenerator and procedures or rules tends to registersmaller quantities of chemical waste or even nothing in
this category (although they realize the same process as
the others). Without the identication this kind of wasteare classied as common waste or infectious and theywill receive the wrong destination in the cleaner urban
system or in not legal landlls.
The technological development in production of goodsor services generated one deadlock that today representsa challenge for sustainability and to the overcoming of
serious environmental and social problems. One of themore serious aspects of these questions is the growingof environmental contamination levels by chemical
products widespread for all the planet mainly in thewater, soil and air by the releasing of efuents andemissions or disposal of toxics waste. Reverse this strongaggravation tendency requires effort of the mobilization
of the production sectors, governs the society to ndsolutions that involves behavior, changing values and
priorities.
The examples that we discuss suggests deciencies inthe management of RSS in the hospitals despite manyother questions referring to the management of materials
and processes that already exists even in the embryonicform. This is the beginning of the development of newlogistics ows – Reverse Logistics Chain and recyclingof the waste . Involving manufacturers and supplierswith their responsibilities and solutions
On the other hand, a clear part of this changing passeddirectly because of economic or legal pressures andare limited by reactive attitudes. It also still depends
on the increase in value of environmental aspects anddissemination of these values to the all society.
