Exposure of A. fumigatus hyphae to antifungal drugs modu-lated VOC production, particularly sesquiterpenes: !-trans-bergamotene increased 10-fold from baseline with 12 hours ofmicafungin exposure and 3-fold with 12 hours of amphotericinexposure, followed by near-complete attenuation of all volatilemetabolites 36 hours later. In vitro voriconazole exposure, incontrast, reduced primary metabolite, monoterpene, and ses-quiterpene production at 12 hours, with attenuation of all vola-tile metabolites 36 hours later (Supplementary Figure 3).
Distinct VOC Pro!les in Other AspergilliEach Aspergillus species we assessed had a distinct VOC pro!le,consistent within biological replicates of each species anddistinct between species, with particular interspecies heteroge-neity in monoterpene and sesquiterpene metabolites (Figure 1).Aspergillus terreus had a particularly rich and abundant sesqui-terpene secondary metabolite pro!le, and A. calidoustus consis-tently produced !-sesquiphellandrene. Under these cultureconditions, A. !avus and A. niger produced alcohols and ke-tones in abundance, but no volatile secondary metabolitesother than limonene (Supplementary Figure 4). Beyond limo-nene, there was no terpene overlap between A. fumigatus andany of the other Aspergillus species assessed.

classi!ed as having possible IFD during his lifetime, with pul-monary nodules but unrevealing respiratory tract cultures andnegative fungal antigen testing. On autopsy, however, these pul-monary nodules contained invasive septate hyphae, identi!edas Aspergillus by immunohistochemical staining by the CDCInfectious Diseases Pathology Branch.
We found no association between contents of the meal priorto breath sampling, tobacco use, or concurrently administeredinhaled, oral, or topical medications and detection of these sec-ondary metabolites.
Although we did not discern a clear association between rel-ative abundance of Aspergillus breath metabolites and the sizeor number of lung lesions on chest imaging, the smallest IA le-sion detected through breath metabolite analysis was 0.88 cm3.
DISCUSSION
We identi!ed distinctive terpene secondary metabolites in thevolatile metabolome of the most common pathogenic Aspergil-lus species in vitro, and found that !-trans-bergamotene,"-trans-bergamotene, a !-vatirenene–like sesquiterpene, andtrans-geranylacetone comprise an Aspergillus metabolic signa-ture that can accurately discriminate patients with IA frompatients with other pneumonia, in a heterogeneous populationat risk for IA. Our results suggest that direct detection of exog-enous fungal metabolites in breath can be used as a novel,
noninvasive, species-speci!c approach to identify patientswith IA, potentially allowing more precise targeting of antifun-gal therapy and fewer invasive diagnostic procedures.
Although microbial VOC detection in the breath has beensuggested for the diagnosis of pneumonia, including IA [31],these studies have proposed biomarkers that are either primarymetabolites or catabolic products ubiquitous in many species orcompounds that lack biologically plausible synthetic pathways[32]. For example, 2-pentylfuran, a linoleic acid breakdownproduct, was proposed as an IA breath biomarker [23, 33] buthas since been shown to be widely present in food productsand ambient air [33], and not detected in other series [34] orin any breath samples in our study. Other studies have takenhypothesis-free feature classi!cation approaches without iden-tifying the biologic components distinguishing infected andnoninfected patients, increasing the risk of identifying signalin noise and limited reproducibility [31, 35].
In contrast, we took a biologically guided approach to bio-marker identi!cation. We identi!ed terpenes released duringA. fumigatus growth and modulated these metabolites with var-ious stress conditions and antifungal exposures, suggesting a bi-ologic relationship between the metabolically active organismand production of these compounds. Knowledge of theseunique metabolites informed identi!cation of these fungal me-tabolites and closely related sesquiterpenes in breath.
Others have previously described sesquiterpenes in the in vitrovolatome of A. fumigatus [20, 34, 36], and one group identi!ed asesquiterpene in the breath of 8 IA patients [34], although thedominant metabolite, !-trans-bergamotene, was misidenti!edas !-farnesene in these studies, given the similarity in fragmenta-tion patterns and the absence of !-trans-bergamotene from theNIST library. In our study, and in an independent assessmentof the in vitro A. fumigatus volatome [37], the NIST library ini-tially identi!ed !-trans-bergamotene as !-farnesene, but we ob-served perfect retention time and spectral alignment between thisendogenous sesquiterpene and !-trans-bergamotene. !-trans-bergamotene is putatively a precursor of fumagillin, a secondarymeroterpenoid with antibiotic and anti-angiogenic properties[17, 38, 39].
The biologic signi!cance of these sesquiterpenes, whether asend products or as precursors to other secondary metabolites,and their role in fungal pathogenesis are yet unde!ned. Whilenot required for primary growth of the organism, a substantialdiversion of resources from primary metabolism is required tosynthesize these metabolites, and many fungi have evolvedunique terpene cyclases with distinctive suites of sesquiterpeneproducts of great structural and stereochemical diversity [15, 16,40]. These products are believed to have roles in inter- and in-traspecies communication, deterring competing microorgan-isms in the environment and potentially contributing tosurvival of the organism in its hosts [15, 18, 19].
