Does the health examination focus on areas that

are really important?

Smoking is estimated to increase the risk —

- For coronary heart disease by 2 to 4 times

- For stroke by 2 to 4 times

- Quitting smoking cuts cardiovascular risks. Just 1 year after quitting smoking, your risk for a heart attack drops sharply.

- Within 2 to 5 years after quitting smoking, your risk for stroke could fall to about the same as a nonsmoker’s.

• Long-time employment supports quality shipping.

The shipping industry is used to long term investments in ships… Crews may also last 30 years in service!

The challenges of today - Focus on improvement on health status in general

What are the main health challenges among experienced seafarers today?

Lifestyle related diseases – cardiovascular disease, cancer,

diabetes and many others

• Sedentary lifestyle with insufficient physical activity.

• Smoking.

• Overweight and inappropriate nutrition.

The Philippines is one of the world’s emerging diabetes hotspots. Ranked in the top 15 in the world for diabetes prevalence, Philippines is home to more than 4 million people diagnosed with the disease

Diabetes is growing alarmingly in India, home to more than 65,1 million people with the disease, compared to 50.8 million in 2010.

Present status

We establish more and more sophisticated screening procedures because we want to avoid active seafarers getting sick while employed. We find some, who actually will be sick (true positive in the screening process) - But the majority will, despite positive screening markers, not be sick (false positive)

RESULT: Many highly qualified seafarers and fishermen are lost from the business

Conclusions:

• General health checks did not reduce morbidity or

mortality, neither overall nor for cardiovascular or

cancer causes.

• The number of new diagnoses increased.

• General health checks are unlikely to be beneficial.

- Screening is simply not the right tool to lift the mean health status of a group

Conclusions Present status - Pre-employment health examinations are a

part of the business and have come to stay.

- Statutory health examinations, especially the first one, will ensure the basic safety needs of the industry.

The future - Healthy seafarers will be increasingly

important to ensure safe shipping.

- Focus needs to move from selection towards creation of healthy seafarers by health promotion and the industry must create the necessary framework to support this.
