Advance Planning for Quality Care at End of Life - Action Plan 2013–2018

Launched by the Minister for Health in 2013, this Action Plan provides the way forward for NSW Health implementing advance care planning in the NSW public health system over the next 5 years. It also identifies strategic partnerships with other government agencies and sectors to improve this aspect of planning for end of life care in primary, acute and aged care settings. This Action Plan seeks to normalise Advance Care Planning and improve end of life care by integrating patients’ wishes into the management of chronic life-limiting illness.

The End of Life Decisions, the Law and Clinical Practice Website is a resource for health care professionals containing comprehensive information about the law in NSW, advance care planning and end of life decision making. The site also contains guidance and contacts for health professionals seeking further information and advice on medico legal issues. The site will strengthen health care professional's knowledge about the law in NSW to facilitate lawful decision making and support patient involvement in end of life choices

Capacity Toolkit The Capacity Toolkit is a guide to assessing a person's capacity to make legal, medical, financial and personal decisions. The Capacity Toolkit was created in response to requests from lawyers, medical professionals, health workers, carers and advocates who required more information about capacity, some general capacity principles and guidelines on assessing a person's

capacity to make decisions.
