About the Template

•There are 80 identified Academic programs

•Faculty from each program will fill in the template for that program

•Template consists of 26 questions over 10 criterion areas

•ATF members will use a scoring rubric to evaluate and score the templates

•The goal is to assess program contribution to success of OC

Instructions to report writers

•Using the provided template, fill in responses as shown in this training–Enrollment and Financial data provided will be entered into

a table–250 word limit on written responses; 500 word limit on

Criterion 9

•You may save a draft of your template at any time. Use the “Finalized” check box to submit your final report.

•Each question response will be evaluated by ATF members as: Not Applicable Minimal/Limited Moderate Exceptional

•The ATF will combine individual scores into a group score for each question.

•The weighted question scores will be combined into a score for your program
