1. Employer chooses what expenses will be reimbursable

A. Deductible/Coinsurance Only: unreimbursed deductibles and coinsurance within a health plan (least expensive)

B. Deductible Plus: Unreimbursed deductibles, coinsurance, and co-pays (more expensive)
