  • PREFACE

    This is the sixth edition of the Year Book for Graduating Medical and Dental Students of the College of Medicine, University of Ibadan. Our ability to produce this edition against all the odds encountered during the session confirms the comment in the preface to the fifth edition that we have overcome the "risks of Childhood mortality".

    We have maintained the general format of the book, with only a slight modification in the presentation of the jokes and popular saying to reflect the various aspects of life in the medical school.

    In the past one year, quite a number of our teachers have left the services of the University and it appears the trend will continue for some time to come. We shall continue to call on the government of this country to save medical education in Nigeria by ensuring that our high calibre teachers remain in the country through the provision of conditions of service that are commensurate with their status in the society

    The acceptance of our medical teachers all over the world is a testimony to their professional competence of which we are very proud. No nation can ignore the problems attendant on mass exodus of her best brains in time of National calamities as Nigeria is doing now. The situation must be rectified as a matter of urgency for the benefit of generations unborn.

    The economic crunch has continued to affect us negatively. The production cost of this edition is higher than the cost at which it is being offered to students. The last edition was poorly patronised because the students could not afford the unexpected increase in its price over that of the 4th edition.

    We are greatly indebted to those who have contributed to the cost of production of this edition. We are particularly grateful to the medical director of St. Vincent Medical Centre Bodija Ibadan for his unflinching support for our course.

    The members of the Year Book Committee have maintained the high sense of responsibility required for the successful collation, editing and final publication of this book. I am confident that the production of this book has come to be a permanent feature ot this College, the administrators of which have been most encouraging in times of difficulty.

    All too soon, another year has gone and today we have another set of new medical and dental practitioners taking the oath of allegiance at the end of their long training in the Medical and Dental Schools of this great University. Today also represents a landmark, for these new dodtors and dentists are the first to be trained under a new curriculum begun five years ago. This new curriculum appears to represent a great improvement on the old one. However, final judgement wil l have to await a scientific review which has already begun.

    Let me at this stage welcome and thank the Vice-Chancellor of this University for his support to me and the College in the past year. He is a great friend of this College, a honest, patient listener and our ardent supporter. I thank him once again for continuing to demonstrate his love to us by attending and chairmaning this ceremony of admission to the medical and dental professions. I also welcome Professor P. O. Bodunrin, another great friend to this occasion, the first since he assumed duties as the Deputy Vice - Chancellor of this University. Finally, I welcome the Registrar and the Chairman of the Oyo State Branch of the Nigeria Medical Association and thank them for coming.

    Our Special Guest, Dr. H.E.B. Green has already been introduced and he has delivered his words of wisdom. As you would have noticed, he doubles as a medical and dental practitioner, for he holds both MBBS and BDS degrees, although he is better-known as a distinguished dental practitioner. There is no doubt that he is a great Nigerian and a source of inspiration to all of us, especially these young men and women beginning their life-long career today. I thank him for accepting to be our Special Guest.

  • This College of Medicine has now trained a total of 3,234 MBBS and 138 BDS graduates since 1960 when it started training doctors and dentists fully here in Nigeria. Our graduate output in the last five years are as follows:

    There are 200 students in this year's MBBS class and 23 in the BDS class. Our College of Medicine therefore continues to be the largest and most important producer of doctors for the country, thus fulfilling the role for which it was set up in 1948 and justifying its designation in 1976 as a centre of excellence in Medicine in Nigeria. The Cortege has, as before, continued to provide assistance to other Medical Schools in Nigeria. In the area of health care delivery, our teachers have not only continued to provide high-quality curative services to Nigerians in their role as consultants to the University College Hospital, but have played a crucial role in the training of personnel for the successful take-off of primary health care in Nigeria. I will therefore like to pay special tribute to all members of our staff who continue to work with integrity, vigour, and devotion to duty in order to maintain the high standard for which Jbadan is well-known even in the face of dwindling resources and immense frustration.

    Funding was again, one of the major problems this session. The level of funding of this University and consequently its Medical and Dental Schools has become so critical that there is now a serious threat to our training programmes especially the quality of our graduates. This last year was really desperate as shown in the amount of money made available for Goods and Services to our 44 Departments and units between 1980 and 1987.

    Actual receipts for this year were in fact much less than the amount allocated. Thus, in practical terms. Departments now have available to them between N100 — N200 a month to provide teaching, practicals, and research facilities. The stage has now been reached where members of staff donate part of their monthly salaries towards the running of the Departments, a very deplorable situation indeed. The

  • consequences as excepted, include run-down, outdated equipment and lack of reagents and chemicals to run practicals. The departments in Dentistry are the worst hit since they require a lot of materials and equipment to function. Needless to say, morale among staff is very low.

    Added to the above is the increased exodus of staff from our Medical and Dental Schools. Over the past 4 years, 43 members of Academic Staff have left or indicated their intention to leave our services. Of these, 25 are of Professorial status and only 9 attained the age of 60 years before leaving. 23 of the 43 members of staff are either in Europe, Canada, U.S.A. or the Middle East countries. And, reports reaching us show that up to 15 members of staff plan to leave between now and next year. Needless to say, this must not be allowed to happen, for that would deal a heavy blow to medical education not only in this University but in the country. The most disturbing aspect is that this country is losing experts at an alarming rate in the less attractive specialities of Medicine — anaesthesia, radiology, otorhinolaryngology, pathology — where staff training and recruitment is difficult world-wide.

    To summarise, the situation in the Medical schools in the country is as follows: Funding is poor, morale is consequently very low and teachers are leaving for

    greener pastures outside the country at a very high rate. Thus, medical education and the provision of specialist care for the populace are seriously threatened.

    A solution to the current high exodus of staff can only be found if a dispassionate examination of its causes is made. Up till the early part of this decade, the remunerations and conditions of service of medical specialist doctors had compared favourably with those of other countries of the world. Nigeria was therefore able to retain its special.ists and even attracted other highly qualified doctors from other countries. With the devaluation of the Nigerian currency, salaries were in real terms lowered to between 200 — 300 US dollars a month. In the meantime, average pay in other countries has risen and in the Middle East countries, average pay is up to 5,000 US dollars a month, almost equal to what a specialist doctor earns in a year in this country. Moreover, in those countries, salaries are tax free, housing, electricity and water are free, and all hospitals are provided with the most modern equipment available. Job satisfaction is therefore guaranteed in contrast to the situation in this country. There must be very few people in the world who wil l not accept employment in any country on such good terms, and I do not think it wil l help appealing to the patriotic instincts of such people.

    No one, of course, expects Nigeria to match those countries in conditions of service to doctors but the time has come when a conscious effort must be made to retain those remaining in the country, and I believe the best way of doing this is to provide some incentives to this category of doctors. I therefore suggest the following measures:

    (i) efforts should be continued to rehabilitate and restore our Teaching Hospitals to what they should be — centres of excellence in all their ramifications. A lot is currently being done by the present Administration in the country to correct the years of neglect of our teaching hospitals but the effort must be accentuated;

    (ii) official establishment of facilities for private practice in our teaching hospitals. This will have the effect of not only increasing the revenue of our teaching hospitals, but also allow private funds to be injected into provision of modern

  • equipment in these Hospitals. Furthermore, geographical practice will ensure speedy treatment for it conserves and even attracts foreign exchange and reduces clandestine private practice by members of staff of the teaching hospitals;

    (iii) increase the remunerations and benefits of specialist doctors through

    (a) an increase in clinical supplementation payable to these doctors; (b) distinction awards for dedicated, hardworking staff; (c) provision of a yearly grant for research as done in countries like Cameroun

    and (d) official sponsorship of specialist doctors to attend a scientific conference

    overseas in his/her discipline once every three years. This will ensure that such doctors are current in their knowledge and practice.

    (iv) the National Postgraduate Medical College needs to be strengthened to ensure adequate output of specialist doctors for the country, particularly in the less attractive specialities. The Government may also wish to re-examine the ban placed on one-year abroad training for our specialist doctors. Even if it is not in a position to fund such training for all doctors, government may allow a few in critical and difficult specialities like neuro-surgery, cardiothoracic surgery, cardiology etc. to benefit from such sponsorship. This, I believe, will ensure adequate supply of such experts for the country.

    Finally, I wish to address specifically, rejoice with and congratulate our new doctors and dentists. You are today joining the over 3,000 doctors and dentists which this College has produced, men and women who have proved their worth in different spheres of life, making us proud of our College and our University. Such men include your teachers; leaders in Government, Universities and Industry; specialists in various hospitals in this country and abroad; doctors serving selflessly in both urban and rural areas; men and women whose expertise are being used not only by Government but by notable world bodies like the World Health Organisation. These are the people you will be joining as medical colleagues.

    Your qualifications give you a unique priviledge — that of knowing the innermost secrets of people who come to you as patients. This priviledge bestows on you a very great responsibility which must not be misused. You must always remember that in all you do, you will ultimately be accountable in your actions to your conscience and to God. This College has given you good basic training in the art and science of medicine and dentistry. We are now using this ceremony to remind you of the great responsibilities ahead of you by asking you to take the oath of allegiance which we hope will guide you in all your future endeavours. It is our hope and belief that you will not disappoint us. I also need to remind you that your alma mater is presently going through hard times, bordering on survival, as a result of chronic underfunding. It is our hope that you will at all times continue to remember this great Institution, assist her in whatever way you can and seek to protect its image, interests and welfare, for it has given you a chance in life. Many Universities outside this country derive their funds not from government but from private sources and from alumni. It is our hope that we too shall reach that stage in the nearest future.

  • AN A D D R E S S OF "WELCOME TO THE MEDICAL A N D DENTAL PROFESSIONS" PRESENTED BY DR. H.E.B. GREEN TO THE 1 9 8 8 GRADUATING STUDENTS OF THE COLLEGE OF MEDIC INE, UNIVER-

    SITY OF IBADAN ON FRIDAY, 29TH JULY, 1 9 8 8 .

    The Vice Chancellor, The Provost and Deputy Provost of the College of Medicine, The Secretary to The College, Deans, Sub-Deans and Heads of Departments, Dear Colleagues and Graduands of 1988, Distinguished Ladies and Gentlemen.

    The Honour is indeed rare and the pleasure great. I therefore thank the Provost for his kind invitation to address the Graduating Classes of our young and eager Doctors and Dental Surgeons who have today been numbered amongst us and to partake in the practice of the Art and Science of our noble Professions. It is gratifying to note that, for the first time, the Dental profession has been accorded the rare honour of actively participating in the Ceremony by inviting one of its members to deliver the Welcome Address. Although I am privileged to have both the Medical and Dental qualifications, the greater part of my professional career has been devoted to the advancement of Dentistry. This not withstanding, my address this morning will naturally embrace both professions. The theme of my address, "FIGHT THE GOOD FIGHT WITH ALL THY MIGHT", has been consciously and deliberately chosen at a period in the history of our profession where there has been series of Doctor bashing by the Press and the Public whenever there is agitation for reforms in the Health Care Delivery to our citizens. There has also been public outcry and condemnation whenever Doctors go on strike or threaten to go on strike because of demand for improved financial rewards.

    In the face of all these, we must in all honesty look inwards and reapraise our professional role in the Society in which we live and practise. Are the public criticisms justified?, or are we failing in the high degree of professionalism expected of us? I believe that the answer to the latter question may be relevant to whatever the profession may be passing through today. From time immemorial the profession of Medicine has been accepted as an honourable one; it commanded the respect of the Society which appreciated its value. But how honourable have we been in recent times? Our young colleagues, especially, do not fully appreciate the meaning of honour, dignity and dedication as the pillars on which the profession stands; and if we must accept the assertion that there are differences in the degree of moral obligation among men, then the moral obligation of the professional man must be greater than that which rests upon any other. Fjy_jhjg_Sfi whom he serves are dependent upon his moral integrity and his skill to a degree that is greater than the obligation of any other^ The persons who enter upon a profession as their life's work are supposed to devote themselves to the welfare of man and to the Communities in which they live. The professional man sells no goods. In general terms his equipment is his mental endowment supplemented by his training, which has become an integral part of himself. The professional man does not advertise or sell; he serves; he uses his mental endowment and special training to relieve the difficulties and distresses of

  • men, and for the benefit of Society. The question then to be asked is can we legitimately and proudly claim to be grouped among these men? What we see and hear today of the professions make us hesitant as to our legitimacy to this claim,

    i Allegations of professional negligence, sub-standard professional ethics, lack of confidentiality to our patients, breach of trust to both our patients and our colleagues, lack of respect to senior colleagues and lack of dedication and total commitment to one's moral obligation to the very people who look up to us for succour, relief and consolation, are causing the profession great concern; and one wonders whether the important subject of MedicalJurisprudence is again taught in our Schools these days. It is astonishing to find that very many young Doctors and Dentists have never seen or read that invaluable little document published by the Nigeria Medical Council — though not comprehensive enough — and captioned "Rules of Professional Conduct for Medical and Dental Practitioners in Nigeria". I am ofthe opinion that present day societal norms and diminished commitment on the part of our teachers may be partly responsible. Which ever it is, it is our avowed duty to defy ignorance and despondency in all their ramifications. We must rise above any detracting factors and your morale and that of your patients must-be-paramount and be kept afloat. You must endeavour to remember always that demeanour, personal attention, perception, integrity, good intentions and dependability will always influence a patient's response to whatever treatment is given much more than the drugs prescribed or gadgetry that characterise contemporary medicine in these days. Recovery of your patient must be the paramount goal above everything else. I therefore charge you all to be above pettiness and misplaced pride. You and therefore before God and man, taken the great and solemn to consecrate your lives to the service of humanity; to give to your teachers the respect and gratitude which is their due; to practise your profession with conscience and dignity; the health of your patient wil l be your first consideration; to respect the secrets which are confided in you, even after the patient has died; to maintain by all means in your power the honour and noble traditions of the profession; your colleagues will be your brothers; never permit consideration of Religion, Race, Party Politics or Social standing to intervene between your duty and your patient; to maintain the uttermost respect for human life from time of conception; never to use your professional knowledge, even under threat, contrary to the laws of humanity; and finally these promises you made solemnly, freely and upon your honour. "So help your God" (sic). These are not words of fun which are to be repeated lightly only to be forgotten and discarded as you proceed into the much wider world to practise your profession. The intelligent Society is aware of your pledge and will not hesitate to condemn you when you violate it. That condemnation will be the penalty of your obligation. The respect accorded you and the profession is a trust which you must bear very high like a blazing torch. You must have been warned by your teachers that your professional calling involves both mental and physical sacrifice. Your hours of duty will be tedious and long with moments of disappointment and-joy. After all, what profession has the rare ability to revive the sick and the mentally disturbed? What profession has the rare opportunity of achieving its goals through so much of human interplay? The present feverish quest for money and materialistic wealth has patently been responsible for some of the justifiable criticisms levied against us.

  • Therefore, anyone of you who thinks that our profession, executed with the right mental attitude, and according to your pledge this morning, will make you a millionaire should immediately start thinking of an alternative. That is not the subject of my address. But you will achieve fame, comfort and full spiritual satisfaction. You will be numbered among men who prize honour and virtue above the mere external advantages of fortune.

    To achieve this, you must fight the good fight with all your might and retrieve the falling image of the profession. Distinguished Ladies and Gentlemen, before concluding this address, I must crave your indulgence to express my concern, and perhaps that of others, on the many inter-relations between Medical and Dental Practice which are not sufficiently appreciated. As a result, the patient does not always benefit from the co-operative administration of these two professions. It is indeed unfortunate that the fraternal Medico-Dental relations which should and must exist are not found more universally in practice. Dentistry is a young profession compared to Medicine; but as a profession it is in no way inferior in purpose, achievements or service to humanity. Today the Dental Student receives training in the basic Sciences equivalent in scope and detail to that of the Medical Student. He attends lectures and clinicals in general Medicine and Surgery which give him an understanding of the common diseases, their oral manifestations and relations, the associated laboratory findings and the possible complications that may arise. Therefore, our present Dental Graduate can speak and understand the language of the Medical Practitioner, and can co-operate with him intelligently in the manage-ment of his patients. This, regretably, is not the case with the Medical Graduate whose basic knowledge of Dentistry is limited all too frequently to his personal experiences which many times are unpleasant!! Few people — including Medical Practitioners — regard Dentistry as a true Health Service Profession which requires the same broad physiophathological background, specialised training, good clinical judgment and technical ability as any of the Medical specialities. Complete or partial denture prostheses be they removable or fixed, or the restoration of diseased teeth by any of the highly skilled methods, or the reduction, immobilisation and fixation of fractures of the facial bones, or the surgical removal of tumours and tumorous lesions of the hard and soft tissues of the oral cavity is no more a mechanical procedure than is an appendicectomy or the reduction, immobilisation and pin fixation of a fractured femur. All require an understanding of the Anatomical structures and the physiological functions of the affected parts and call for a high degree of technical skill for their successful management. You will, therefore, agree that the non-inclusion of a well presented course in Dentistry and a basic knowledge of oral diseases of general Medical significance to our Medical Students is a serious defect in the Medical curriculum which has been responsible for the lack of better understanding between the two professions. It is important that the Medical Practitioner must receive adequate undergraduate Dental instructions which include clinical demonstrations of common oral diseases; and only through this can the two professionals foster that good interaction which has been a stumbling block to the successful management of our mutual patients.

    Finally, dear colleagues, as you depart to your various destinations to be with your families and loved ones, our prayers are with you; and always remember that a man's

  • true greatness does not depend upon the largeness of the sphere of his work- it is measured by his CHARACTER and SERVICE to his Fellow Men. Thank you.

    This is a formidable list considering the fact the services of these specialists are lost to this country.     While we hate to lose their experience, we cannot but accept the need for individuals to plan their lives as best they can. We wish all of them the best in all their future endeavours.
