You have the opportunity to earn up to a maximum of $1300 for each wellness year as follows:

Participation in 6 or more hours of LED programs $80

**To be eligible for the incentive, you must complete the biometric screening and wellness consultation. If you do not complete this step, you will not be eligible for the other incentives.

Any value earned will be placed into your Health Savings Account.

MOST OF THE PROBLEMS IN LIFE AREBECAUSE OF TWO REASONS:

Team AwardStep 1To be eligible for the Team Wellness Incentive, each employee that participates in the Team Wellness incentive must complete annual blood draw and wellness consultation.

Step 270% of your team must complete 7 out of 9 of the incentive items (non-tobacco use, BMI <40, BMW LED 6 hrs, LDL, HDL, BMI <28, triglycerides, glucose & blood pressure). Completion of a Reasonable Alternative would meet the requirement.

If team member does not want to participate with the team, he / she will need to inform the team BMW Rep. The BMW Rep will notify the BMW Staff to remove the individual from their team.

The Team Award will qualify you for a $75 gift card, which will be distributed mid-December.
