The connections...

Integration is a means towards an end, not an end in itself:

• Personalisation: A means towards ensuring that the health and care system is built around the needs and aspirations of people with care and support needs, which values their contribution and empowers them to exercise the choice and control that is right for them.

• Prevention: A means towards redesigning the system so that ill health and functional impairment can be avoided, or managed much more effectively, and people can live in stronger, healthier communities.

• Broader focus on health and wellbeing: A means towards tackling those issues outside the reach of health and social care, which undermine people’s health and wellbeing or can contribute to improved health and wellbeing in the future.

• Coproduction: A means towards engaging people in the design, delivery and evaluation of services, in assessing their needs and planning which treatments and supports would best meet them.

TLAP and the Shared Commitment

• TLAP welcomes the increased focus on integration and the commitment of national partners towards making progress at pace and scale – it’s about time!

• In particular, we welcome the grounding of this commitment in a Narrative of person-centred, coordinated care that is about people’s lived experience of the health and care system

• TLAP’s National Coproduction Advisory Group worked alongside National Voices to ensure the Narrative is well aligned with the vision spelled out in Making it Real

• We will be working to ensure that the perspective of people using services as well as that of provider organisations is well represented

• We will be offering support to Pioneers and others who have included integration at the level of the individual through personal budgets as part of their plans

• We are working collaboratively with National Voices on Making it Real Narratives for Mental Health, Children with complex needs and end of life care
