    SCHEDULE D PERQUISITES

    Sick Leave Short & Long Term

    The Employee is entitled to Sick Leave as follows:

    First 4 months salary continuance at 100% of salaryTo age 65 66.7% of pre-disability earnings through to age 65 on a non-taxable basisUp to $8000 of benefit will be provided with no medical evidence to qualify

    Other Features Offer a partial disability benefit so partially disabled persons qualify Offer an EAP program tied to the disability for dealing with the disability Add the Best Doctors program to get first in line service Enhanced definition of disability Change the definition of disability to Own Occupation to age 65 By implementing the individual plan, if the employee terminates, she will take up to $3000 with her as

    permanent disability coverage to age 65

    Benefits on Early Retirement

    If the Employee retires and has not yet reached age 65 and is in receipt of Employers pension plan benefits,the Employee will be provided semi-private, extended health care and dental benefits on the same basis as isprovided to active employees. The Care Group will contribute the same portion towards the billed premiums ofthose benefit plans as is currently contributed to the billing premiums of active employees. The early-retirement Employees share towards the billed premium of the insured benefit plans will be deducted from theEmployees monthly pension cheque.

    Health Spending Account

    The Care Group will provide the Employee with a $2,000 Health Spending Account on an annual basis, to payallowable medical and dental expenses accordingly to CCRA guidelines.

    This money will be utilized to pay medical and dental expenses up to this limit. Once the total expenses for theemployee reach the annual limit listed above, there will be no further reimbursement of expenses in that year.If the employee did not fully utilize the limit in a given year, it can be rolled to the next years Health SpendingAccount. Any monies rolled to the next years Health Spending Account MUST be used in the following year othey are lost.

    The Health Spending Account utilizes the same claims payor as the fully insured plan and as long as theexpenses are allowed, they will be reimbursed until the maximum is reached.

    Vehicle/Parking Allowance

    The Care Group will provide the Employee with an annual Vehicle Allowance of $300 per month and paid

    parking.

    Memberships

    The Care Group will pay the Employees annual dues associated with professional associations.

    Education

    The Care Group will provide the Employee with the resources to allow the employee to participate inappropriate Professional Development.
