1:15 to 2:15 Hospital at Home: Per Danielsson, Executive Medical Director & Director of CareTransformation at Swedish Medical Center, authored a proposal for the new high touch high tech program to take care of hospital-level patients at home. Dr. Danielsson will provide an overview of this program which is slated to roll out in early 2017.

2:25 to 3:25 Provider Alignment to Achieve Improved Outcomes: Joan Valentine, RN, MSA, VP of Quality and Clinical Integration at US Medical Management,

will discuss the importance of aligning all the providers in the patient’s continuum of care to improve outcomes. Joan has significant experience

in working with CMMI (Center for Medicare and Medicaid Innovation) as well as both a Pioneer ACO, a Shared Savings program and a member of the CMMI post- acute care continuum task force.

Home Health Care Services- Tracy Mroz, PhD, OTR/L, Assistant Professor, Department of Rehabilitation Medicine, University of Washington and Susan Skillman, MS, Deputy

Director, University of Washington Center for Health Workforce Studies. Tracy and Sue will share findings from three related studies on home health care in rural America. Topics include “views from the field” about barriers to access to home health care in rural areas; and studies of the impact of community factors and service provision on quality outcomes for home health care among high-risk, rural Medicare beneficiaries. They will focus on what home health providers need to know about current research as well as ask for feedback from providers on interpretation of study results and next steps for research.

alignment across settings and evidence that the home is the preferred setting for best clinical outcomes.

Breakout Session #1B: Rural Home Health - talk with Andrea Manningabout problem solving access to home health care for critical access hospitals.

Breakout Session #2: Face to Face- Jennifer Warfield, BSN, COS-C, HCS-D, Education Director, PPS Plus. The importance of accurate and complete Face to Face documentation became very evident in Round One of the Probe and Educate audits. With Round 2 soon to be underway, and with

Face to Face being an important aspect of Pre Claim Review, this presentation is important and timely. Jennifer will not only provide information on the documentation requirements, but also will give examples of where to find the documentation. While PCR is not piloting in our state, it is highly likely it could be introduced beyond the pilot states in the near future.

Breakout Session #3A: Business Strategy for Today- Donna Cameron, will be speaking on the importance of developing and nurturing a culture of kindness in the workplace. Studies have shown a 20% increase in performance and that employees are 87% less likely to leave with the

kindness culture!

Breakout Session #3B: Grass Roots Initiative - talk with Board member Brent Korte about starting a “grass roots” activity in getting your senator and representatives on a joint home visit!
