If you are a resident or fellow and would like to receive the discounted rate, you must submit a signed letter from your program director verifying that you are still in training. The registration fee includes course tuition; a course syllabus; luncheon buffets; a continental breakfast on Saturday and Sunday; and refreshment breaks throughout. In addition, each participant will leave the course with written evidence of CME credits received. A letter of confirmation will be sent upon receipt of payment and completed registration form.
CANCELLATION POLICYShould you choose to cancel your registration, a request must be submitted in writing and postmarked no later than Friday, August 22, 2014. All requests postmarked by this date will receive a full refund less a processing fee of $50. No refunds will be granted after August 22, 2014.
