There are 5 main menus at SAIS Main Page: • Garis Panduan Permohonan Aktiviti – Link to student’s activity

submission flow chart page. (Please refer faculty HEP office for any new application procedure).

• Permohonan Aktiviti – Link to Login Page to access student’s activity

application form. • Senarai Kursus FPP – Link to FBM webpage to view list of FPP program. • Muat Turun Borang – Link to FBM webpage to download any HEP

related form. • Hubungi Kami – Link to HEP Contact Information Page.

(C) Student’s Login Page

Please register as SAIS users by clicking the Pendaftaran hyperlink. Only FBM students are allowed to access into SAIS.

(D) Student Registration Page

Please active your account by clicking the URL link in your email notification.

The link will direct you to Login Page. To login, used your student ID as your username and password that you have created. (Please click Lupa Kata Laluan hyperlink if you have forgotten your password).

(E) Student Module Page

There are 7 modules at Student Module Page: • Garis Panduan Permohonan Aktiviti – Link to student’s activity

submission flow chart. (Please refer faculty HEP office for any new application procedure).

• Permohonan Aktiviti – Link to Application Category Page for student to

submit their activity application. • Muat Turun Proposal – Link to page where student can upload their

proposal for HEP view (The list will be appeared in the table if the student did not upload the proposal yet).

• Muat Turun Surat Pelepasan Kuliah – Link to page where student can

upload Surat Pelepasan Kuliah (Only applicable to Program Akademik and Program Persatuan). (The list will be appeared in the table list if student did not upload the Surat Pelepasan Kuliah yet).

• CV Penceramah – Link to page where student can upload CV

penceramah. (Only applicable to the activity that invites external speaker). • Semak Permohonan – Link to page where student can view their activity

application status and activity information. • Kemaskini Maklumat Diri – Link to update form page where student can

update their profile.

(F) Permohonan Aktiviti Page

Please fill up all the information correctly. Please ensure your application has been discussed with your program advisor. **IMPORTANT - Date of activity should be not less than 30 days from date of application. Please leave the Maklumat Penceramah Jemputan Luar as blank if the program does not invites any external speaker.

Please fill up the Maklumat Perbelanjaan correctly and accordingly. Get advise from your program advisor/society advisor or HEP coordinator. Please refer Borang C HEP for the rate of student’s activity application. The form can be downloaded from FBM website.

Please upload your program proposal using the template provided by program society or HEP office.

Please upload Surat Pelepasan Kuliah if the program is program Persatuan or Akademik and the program was executed during class time (Monday-Friday, 8 am-6 pm).

Please upload the speaker’s CV if the program invites external speaker. You can choose to upload Kemudian. (The list of the application will be appeared in the table inside related module).

If any related document was not upload yet in the system, the notification *1* will be appeared on the related module. **IMPORTANT - Your application will not be processed if any related document was not uploading in the system.

Please click the Muat Naik to upload speaker’s CV at column CV Penceramah.

The list of the particular application will be disappeared after the necessary document was uploaded. This indicates that your application was completed. Your course coordinator/HEP coordinator will receive an email notification for them to review your application and endorse your application for Head of Department/HEP approval.

(G) View Permohonan

Please click the Senarai Permohonan module to view your list of activity application.

You can check your application status at this module and view your application by clicking the activity name at column Nama Aktiviti.

The Rujukan Aktiviti will be your application reference number. Please keep this reference number for your claim processes and report purposes.

Please alert of any documents that need to be submitted to faculty HEP office before or after activity. The documents are final report, evaluation form, claims form, sponsorship approval letter and Surat Ke Luar Negara approval letter (if any related).

You will receive an email notification if your application was approved by HEP office. Please get your activity approval letter at faculty HEP office before proceed with the activity or any necessary action to be taken before activity started.

The above figure is the sample of email notification to show that your application was approved.
