This leaflet
This leaflet is to provide service users, carers, family and friends with information about
Teign ward at Fromeside medium secure unit.
Thanks to service users on Teign ward for help with photographs and ideas for content.

Coming into the unit Wherever possible, we will meet you before you are admitted.
When you arrive on the ward, we will welcome you and introduce you to the staff who
will be involved in your care. Both doctors and a nurse will meet with you to explain the
Admission process and complete the necessary assessments.
Refreshments will be provided.
Fromeside Teign Ward Teign ward is divided in to two parts—
The Enhanced Care and the Continuing
Care unit.
When you are first admitted, you will be
cared for within the Enhanced Care Unit
and the work towards moving to the
Continuing Care side. The ward
is explained in more detail later.
Teign ward uses an approach based on
Attachment Theory. Attachment theory
suggests that the relationships we form
as children with our families or
carers set a pattern that continues
throughout our lives.
These patterns are activated when we
are under stress. On Teign ward, we aim
to understand the effect of early
attachments on the relationships you
have now.
Our staff aim to help you feel secure and
to trust in the relationships you have with
your care team.
Fromeside is a purpose-built medium
secure unit situated within the
Blackberry Hill Hospital site in Bristol.
The unit supports people who because of
their mental health problems, may have
had contact with the legal system and
require a safe and secure environment that
enables them to receive a wide range of
treatments, therapies and care to assist in
their recovery and to reduce risk.
The unit is made up of 80 beds spread over
eight wards. The Women’s Service has one
ward for 12 women and is separate from
the Male Service. This women’s ward is
called Teign Ward.
All wards are on the ground floor and have
their own gardens.
Ward routine

You are asked to be dressed in day clothes by 10am in time for therapeutic activity.
There is a night-time curfew so that it is quiet for sleeping. From Sunday to Thursday
this is Midnight and on Friday and Saturday this is 1am.
Therapeutic activity
Meal Times
Breakfast 6.00am - 10.30am
Lunch 12.30 - 1.00pm
Tea 5.30 - 6.00pm
Supper 22.10-22.30
Takeaway food can be ordered on a
Saturday evening.
A menu is provided each week for you to
select their choices for lunch and supper.
You are encouraged to make your own
breakfast - provisions are available. Drinks and
snacks are available throughout the day.
Garden access
The garden area can be accessed
between 6.00am - 9.00pm. You can
ask staff if you wish to use the
garden between these times, and
they will arrange this if possible.
Smoking
You can use the garden for smoking
breaks every hour between 6am and
Curfew (12am on weekdays, and
1am on weekends and bank holi-
days.).
From curfew until 6.00am, you may
have one discretionary cigarette; but
you must have appeared to be
resting for one hour before this.
Every service user will receive support from staff to work out their own timetable of
activities to go alongside regular open groups that are held on the ward.
‘Plan the Day’ sessions are held every weekday morning at 9.45am for you to plan your
time and to make sure you get the support you need from staff. We will give you
information about activities occurring that day and notices about things like finance
opening times and visitors.
Enhanced Care Activities
There are activities held every weekday and led by nursing staff, occupational
therapists and psychologists.
Continuing Care Activities
You will always have a programme of activities on and off the wards as you progress
through your rehabilitation.

Meetings about care
Your clinical care team will include: a main nurse, an associate nurse, a support
worker, a consultant psychiatrist, a Ward doctor, a clinical psychologist, an
occupational therapist and a social worker. We call this the Multi-Disciplinary Team
(MDT). There is also a pharmacist based at Fromeside who can give advice about
medications.
Care plans: Care planning on Teign ward is done between you and your
clinical team. You will be given copies of your care plans and asked to sign them.
Care planning approach: Your care team will hold regular meetings with you to
discuss your care, treatment and progress. We call these Care Planning Approach
meetings or CPAs for short. They are held after the first three months and then every
six months. If you wish, your family members or carers can join these meetings.
These meetings gives everyone the chance to get together and think about all aspects
of your life, your support needs and your future care pathway.
Patient Care Reviews (PCRs) are held every week to discuss your needs and any
other things that arise. We will let you know what is discussed at these meetings every
week and you will be invited to take part in your PCR every fortnight.
The ward

Bedrooms - Everyone has their own en-suite bedroom
with access at all times. Bedrooms are your own
personal spaces. We will try to enter as little as possible.
Visitors and other service users are not allowed in
bedrooms.
Lounge - The lounge is an area for everyone to spend
time with a television, books and other social activities
such as board games.
Kitchen – The Enhanced Care Kitchen is locked but you
can use it with staff support. There are facilities for making
drinks in the lounge. You will need to have an assessment
before you can use the enhanced care kitchen.
When you are on the Continuing Care unit, you can use the
kitchen at anytime. You can make snacks and we will help
you prepare food and meals for yourself, as part of
your recovery.
Dining room – Everyone can use this area to eat
meals together. During meal times, the television is
turned off.
Garden – the Enhanced Care and
Continuing Care wards have separate
gardens which can be used at certain times
throughout the day.
Activity room – on both the Enhanced and Continuing
Care sides, there are rooms just for activities and
groups. Activities co-ordinators offer a variety of
activities and we encourage you to choose which things
to do.

Interview room – both the Enhanced and Continuing
Care sides have a private room available for time with
staff and visitors.
Laundry room – Everyone is expected to do
their own laundry but we can support you
if needed.
Gym – Teign has its own gym complete with a treadmill and space for activities such as
yoga, pilates and a Nintendo Wii.
Enhanced Care suite – this is a safe, private and quiet
space for you to reflect and receive support from staff.

Leave
Leave is an essential part of recovery. It is an important part of your rehabilitation.
Leave will be discussed at Patient Care Reviews and CPA meetings, so that you and
your clinical team can all put forward ideas and make decisions about your leave.
Escorted ground leave
At first , you will only be given access to the grounds of Blackberry Hill Hospital with a
member of staff supporting you. This is to make sure you are safe whilst helping build
your confidence.
Escorted community leave
When you are ready. you will be able to take trips into the community with a member of
staff. There are shops nearby in Fishponds and it is easy to get to the city centre.
Unescorted community leave
Going into the community without staff support will be granted gradually following a set
process, taking account of your needs and your family’s needs, where required.
For all leave, it is very important that you arrive back at Fromeside by the
given time.
Personal property

You will have a lockable drawer in their bedroom for personal or valuable items.
There are some things that may need to be kept locked in the property store in the team
office. These are things that could pose a risk to the safety of the ward such as
scissors, aerosols and nail varnish.
We will let you know what things need to be kept in the property store. Items in the
property store are safe and secure.
One nurse on each shift will act as the ‘security nurse’ and will hold the key to the store.
The security nurse will get items out of the store upon request.
Smoking Smoking
Smoking is permitted in the gardens of Teign ward. The garden area is open from
6.00am - 9.00pm. You can use the garden for smoking every hour.
After curfew until 6.00am, only one discretionary cigarette is allowed but you must have
appeared to be resting for a period of one hour before this.
We encourage service users to develop a healthy lifestyle. If you would like to stop
smoking, we have staff trained to help you do this.
For safety on the ward, lighters are held by staff who will give you ‘lights’ upon request.
Staff boundaries To keep therapeutic relationships within safe boundaries, it is important for members
of staff not to answer questions of a personal nature. This helps us make sure we
focus on service user care. Please don’t be offended if we can’t answer a question
that you ask.
We also have clear boundaries about how staff and service users should behave with
each other on the ward, so that everybody knows what to expect.
There are also policies, so that the staff know how to respond in different situations
and so service user know what will happen too. This is important in making sure
everybody feels safe.
If you have any questions about boundaries, please ask.

Laundry Towels and bed linen are provided and are available from nursing staff. You may bring
your own linen if you wish, as long as we have checked it meets fire regulations.
Washing powder is also provided.
Enhanced Care Laundry Facilities
You can use the laundry with support from staff. Staff may need to do ironing until risk
assessments have been carried out.
Continuing Care Laundry Facilities
The laundry is available at all times. Staff are here to support where needed but we
encourage you to take responsibility for this task. You can get the iron from the security
nurse and must do your ironing in the laundry room.
Having a say

Visitors
We value what service users have to say about the ward. There are fortnightly
Community Meetings for service users on the ward to get together and discuss how
the ward runs, any concerns about the ward and suggestions for making it better.
Advocacy.
We can organise for Advocates from Mind to support you to communicate your wishes
and concerns to doctors and other professionals. They also provide an Independent
Mental Health Advisor (IMHA) service, giving you information about your rights under
the Mental Health Act. Bristol Mind can be contacted on 0117 980 0386.
Patient Advice and Liaison Service (PALS)
PALS provides confidential advice and support, helping to sort out any concerns about
the care being provided. PALS can also signpost you to different services available.

We recognise the importance of contact with family and friends and will help you arrange
and plan for visits.
Home visits
If you have unescorted leave, you can visit family up to every six weeks.
Visitors are also welcome on the ward. We can help you use the telephone to arrange
visits, or visitors can call the ward to arrange a suitable time. However, we recognise
that the geographical area we cover is very large and will consider visits outside of the
regular times for friends and relatives who have to travel long distances.
Weekend visiting times are flexible as there are less scheduled activities and no
clinical meetings.
Visits on weekdays are often arranged for after 4pm to avoid meetings and scheduled
activities.
Under-18 year olds are not allowed on the ward, so extra planning is needed to arrange
visits with young people. Please speak to nursing staff and the social worker for more
information on arranging visits for under 18 year olds.
Family and friends are welcome to contact us at anytime, but we must have your
consent before we can share any information about your care.

What’s on offer? Teign ward has a dedicated medical team made up of psychiatrists, GPs, general
nurses, a physiotherapist and a pharmacist who you can ask to meet to discuss any
concerns about your medciation.
The Nursing Staff based on Teign ward offer a wealth of experience and expertise in
working with women who suffer from mental health problems. They are available 24
hours a day to offer support.
Teign ward has a dedicated team of Clinical Psychologists who are available on
he ward for service users to get to know them.
Psychologists will sometimes be involved in :
carrying out assessments
therapy
runnning groups.
The Malago Centre is run by the
Occupational Therapy team. The centre
offers a range of different activities to meet
therapeutic and rehabilitation needs.
You can get involved with woodwork, metalwork, cooking and many other activities.
There is also a garden attached to the Malago Centre which you can help with.
Occupational therapists can also help service users to access the community on leave for
specific activities and can arrange voluntary work to develop skills.
Possible topics for therapy may be:
ways to manage distress
relationship difficulties
alcohol and drugs work
managing distressing experiences
such as hearing voices
offence work
coping with trauma.
Rivers Café is located in Fromeside and is run by service users and Occupational
Therapists. The café is open during lunch times on weekdays for staff, service users and
visitors. Service users can get involved with Rivers Café and earn some money whilst
gaining valuable work experience. They will need to pass a food hygiene course.
The Arts Psychotherapy department offers individual and group work in art, drama
and music therapies.
The unit has a Chaplain who visits Teign ward and you can request an appointment for
spiritual needs. Teign ward respects and accommodates all religious and spiritual
beliefs. There is a room at Fromeside called The Sanctuary where you can have time
and space for reflection and prayer.
There is a Social Worker on Teign ward who is
available to offer support on all areas of social care
such as money, housing, the criminal justice system
and contact with family. They will liaise with external
agencies alongside other members of the clinical
team to look at services to provide support and ac-
commodation when you are ready to move on
from Teign.

Aggression and violence

Observation
It is very important that service users and staff feel safe whilst on Teign ward and do not
fear bullying, verbal aggression or violence.
Aggressive and violent behaviour is taken very seriously and will not be tolerated.
Any act of physical violence will be reported to the police.
Any incident of aggression or violence will result in service users being escorted to the
‘de-escalation suite’. This is to give them time and space to think about what led up to
the incident.
In some circumstances, staff are required to physically intervene to protect
service users, staff and others. Staff on Teign ward are trained in Prevention and
Management of Violence and Aggression (PMVA) techniques. Wherever possible,
female staff will carry out these techniques.
After an incident requiring PMVA, service users will be offered an opportunity to talk
about what happened.
For the safety of service users, staff and visitors, all service users are regularly
observed. This involves checking that you are safe.
There are different levels of observation. You and your clinical team will decide
which level is best for you.
Level one observation means that every hour staff will check that you are safe and
will record where you are and what you are doing.
Level two observation means that staff will regularly check that you are safe. How of-
ten will depend on your level of risk.
Level three observation means that staff will always be able to see you.
Level four observation means that one or more members of staff will remain very
close to you at all times.
Level 3 and 4 observations are usually used when a service user is at a high risk of
harming herself or others. Keeping constant observation helps to keep everyone on the
ward safe.
Observations are done at night as well as during the day. Staff try to be as discreet as
possible, especially at night.

Restricted items Ito keep everyone safe, we have to limit or deny access to certain items. These are things
that could be harmful to people or damaging to property.
These items cannot be taken onto Teign ward at any time:
weapons
drugs and alcohol
chewing gum
Blu-tac
broadcast or communication systems (i.e. modems)
laptop computers
umbrellas
cameras
matches
metal combs
magnets
As well as these items, visitors must not bring mobile phones or lighters on to the
ward.
There are lockers in reception for safe storage of property.
These items must be kept in the security cupboard:
scissors
razors
plastic bags
glass objects
solvents
DVD’s or VHS Tapes which are not officially produced
Tin/steel cans
computer software
domestic knives
tools
lighters
mobile phones
perfume, aftershave

Getting Here
By Bus
From Bristol City Centre—the number 5 leaves regularly and stops on Manor Road
outside Blackberry Hill Hospital.
By Train
If arriving at Temple Meads Railway Station in Bristol, you can get the number 49 bus
from Old Market Street. This stops on Channons Hill which leads on to Blackberry Hill
and Manor Road.
By Car
From the East take M4 to M32 and exit at J1. Take first exit at roundabout signposted
Yate. Follow signs to Frenchay Hospital, pass it on your left and continue over a mini-
roundabout. Go straight over some traffic lights and turn left at the next mini-roundabout.
Follow road to the ‘White’ entrance (as on map).
