 ACTIVI TIES AND PREPARATIONS BEFORE AND AFTER EMBASSY INTERVIEW I. ATTEND FINAL VISA BRIEFING AT FPI OFFICE ON ASSIGNED DATE AND TIME - Come in school uniforms with 1 parent - Bring final embassy documents in short plastic envelope ( primary documents) and short brown envelope ( secondary documents) - Bring printed copies of US visa application form DS-160 for checking and copy of Confirmation page for your embassy interview - Submit completed and signed TRAVEL ARRANGEMENT and WAIVER FORM - Fill up and sign a 2GO Letter of Authorization II. GOING TO THE EMBASSY - Attend embassy interview in school uniforms and ID - Parents NOT required for applicants 18 years old and above - Bring complete documents ( plastic and brown envelope) with printed copy of DS-160 CONFIRMATION PAGE with photo - Electronic gadgets ( cellphone, ipad, camera and the likes) are NOT ALLOWED inside the Embassy III. AFTER EMBASSY INTERVIEW - If you have companion with cell phone, text FPI at 0917-8975223 and your Program Counselor the OUTCOME of your interview. - You are not required to go back to FPI Office if your visa has been approved. Fill up the Visa Intervi ew De-Briefing Form and email it to FPI at  [email protected]  within the same day of interview - IF YOU RECEIVE A BLUE, OR GREEN, OR YELLOW PAPER FROM THE CONSUL, REPORT IMMEDIATELY TO FPI FOR COUNSELING IV. DELIVERY OF PASSPORT WITH VISA TO FPI BY 2GO COURIER - FPI will text you with instructions to pick up your passport PROVIDED you have DEPOSITED THE NECESSARY AIRFARE INITIAL DEPOSIT with the designated Travel Agen cy. Bank details of the Trave l Agency and instructions where to deposit this will be ANNOUNCED. - GET CLEARANCE TO CLAIM PASSPORT at Rm 204:  Present deposit slips of payments made to FPI (Profiling Fee, Program Fee, Housing Deposits-if required) and Ticket Payments to Travel Agency for validation purposes.   • Work and Travel Program

    FPI WAT FORM TRAVEL ARRANGEMENT and WAIVER FORM

    1. I understand that I need to get my roundtrip, point to point airline ticket from FPIs accredited travel agency. I am directly responsible in making my APPROVED point to point bookings ( Ex: Manila to Indianapolis; then Indianapolis to Manila) with the said travel agency and communicate with them regarding my other travel plans, total airfare, US taxes and Philippine Travel tax. FPI will NOT get involved in the booking and quotation process to avoid confusion. 2. I will ensure that my departure and return dates shall be within the time frame specified by my host organization and approved in writing by my Dean or school authorities and should not exceed 20 days after my DS2019 end date. 3. After getting my visa, I shall coordinate immediately with the Travel Agency my flight details and the total cost of my airfares and deposit payments directly to the travel agency. I will submit my deposit slip to the travel agency and to FPI as proof that I paid total cost of my airfare plus Philippine travel tax.. 4. I also understand that my passport, visa and DS 2019 shall be released by FPI only after I paid at least $1,000.00 or 75% of my ticket cost (whichever is applicable) to the designated Travel Agency as down payment for my airfare. I do understand that I need my passport, visa and DS-2019 to attend the Orientation at the Commission on Filipinos Overseas (CFO) as required by the Philippine Government prior to my departure to the US. 5. I understand that my final confirmed plane ticket shall be released only after attending the FPI Pre-Departure Orientation with one parent or legal guardian as mandated by the US State Department and after showing proof of full payments of airfares and Philippine travel tax. 6. I fully understand that any changes in my itinerary and/or flight schedules after my plane ticket has been issued is subject to appropriate airline penalties. 7. I will email my arrival information to my host organization at least two weeks prior to my arrival in the US. If there is no airport pick up provided by my host company, I will proceed to my housing or host company location following the arrival instructions that I should ask from FPI and / or my host company. 8. I fully understand that in order to change my RETURN DATE while Im aready in the US, I MUST arrange it directly with the travel agency with prior written approval of my school and FPI. I also understand that the AIRLINE will charge re-bookings / re-issuance fees / airfare differences that should be settled first before a new ticket is issued. I understood and agree to comply with all the above statements.
