Alfred Taubman Medical Research Institute 2018 – 2020 Scholar, Forbes Institute for Cancer Discovery CLINICAL INTERESTS My surgical practice can be divided into five areas of focus: (1) orbital disorders, such as thyroid orbitopathy, vascular anomalies and orbital tumors; (2) pediatric oculoplastic surgery, such as congenital ptosis, blepharophimosis syndrome, coloboma, and congenital anophthalmia; (3) periocular cancer treatment and reconstructive surgery; (4) lacrimal disorders; and (5) eyelid disorders, such as involutional ptosis, ectropion, entropion, facial palsy and eyelid retraction, etc. In all five areas, my practice is distributed evenly between tertiary care referrals and highly complex quaternary care referrals. I perform >700 major surgeries per year, including >175 orbital surgeries. RESEARCH INTERESTS The PI/PD’s funded research encompasses basic, translational, clinical and device research, bringing together multiple forms of healthcare research in an innovative fashion that builds on his training and practice as a surgeon-scientist. The PI/PD is the Lead Investigator of a 5-year investigator-initiated prospective clinical trial (cancer) with significant basic and translational components to assess the utility of hedgehog pathway inhibitors in saving vision in cases of advanced periocular and orbital basal cell carcinoma. The trial brings together clinical, histologic, genomic and molecular marker analysis to develop advanced tools for treating personalized care for locally aggressive basal cell carcinoma. PI/PD’s basic research projects utilize zebrafish to study tumor metastasis and heterogeneity, as well as stem cell reprogramming in an adult zebrafish model. Utilizing zebrafish embryo xenografts, the lab is studying epithelial/mesenchymal transitions during the metastatic process, as well as characterizing signaling and epigenetic pathways that regulate the metastatic process. Ultimately the xenograft model will be used as a tool for drug development targeting the metastatic process. Using a zebrafish adult regeneration model, we are characterizing the biology of cellular reprogramming and dedifferentiation. We find remarkable similarities between genomic reprogramming during tissue regeneration and the behavior of cancer stem cells in oncogenesis. The PI/PD’s translational/pre-clinical research project is focused on the role of nuclear receptors in regulating the inflammatory response of orbital fibroblasts in thyroid eye disease. The PI/PD is also actively involved in device development, with one patent issues and one provisional patent filed, and with Coulter Innovation Program funding for developing and bringing to market a novel lacrimal drainage device. Amount: $850,000 (Genentech, Inc., 29% indirect cost recovery) and $95,315 (University of Michigan) The goal of this investigator-initiated prospective clinical trial, which will be run by the PI through the Clinical Trials Office at the UM Cancer Center, is to characterize the role of hedgehog inhibition in the treatment of periocular basal cell carcinoma. The clinical trial will be accompanied by a translational science investigation of the response of tumors to hedgehog inhibition and analysis of disease predictors, with the goal of improving treatment and identifying new treatment targets. Enrollment began in June 2015. Title: Molecular Pathways of Cancer Cell Stemness and Metastasis PI: Alon Kahana, MD, PhD Forbes Cancer Research Institute Period: 04/01/2018 – 06/30/2020 Amount: $120,000 This project utilizes both human and zebrafish studies to elucidate the molecular mechanisms underlying the metastatic process, using a zebrafish xenograft model of breast cancer as well as chromosome conformation capture (Hi-C) and phenotypic analysis. Alfred Taubman Medical Research Institute Period: 4/1/2017 - 12/31/2019 Amount: $500,000 Cancer is a disease of unregulated cell division and migration, which leads to metastasis, organ damage, and the sequelae of malignant tumors (including death). The paradigm that has dominated cancer research over the past 30 years is that cancer is a genetic disease driven by mutations and subsequent selection of malignant cell clones. At the same time, there is growing appreciation that the most important cancer cells harbor unique traits that transcend specific mutations, with features more similar to embryonic stem cells - i.e. cancer stem cells. These are the cells that ultimately metastasize and kill our patients. Our group’s Taubman Grand Challenge Project aims to fight cancer by (1) characterizing key traits of cancer stem cells that can be targeted therapeutically, and (2) developing a unique implantable technology that will attract cancer stem cells during the act of metastasis, providing critical information about these cells, facilitating early detection of metastatic events, and potentially thwarting the metastatic process. Bohnsack, MD, PhD Agency: NIH/NEI K08 EY022912 Period: 12/1/2012 – 11/30/2017 The research goal of this career development mentored research project is to study the biology of the ocular neural crest in a zebrafish model as a means of identifying and characterizing developmental pathways involved in the formation of the anterior segment of the eye, with a focus on congenital glaucoma. This is a mentored clinician-scientist award, and Dr. Kahana provides direct mentoring and material support to the PI. PRIOR GRANT SUPPORT Title: A Zebrafish model of extraocular muscle regeneration. Period 1/1/2016-12/31/2016 The goal of this clinical project is to test whether use of a newly-invented stent reduces tearing in patients while it stents open their lacrimal system. Through an active collaboration a biomedical engineer, we have now developed a lacrimal stent that has a slit opening strategically located to drain tears into the lumen. Such a stent could revolutionize the use of stents in the treatment of symptomatic, debilitating epiphora. Title: A zebrafish model for studying orbital development and disease.
