ANNUAL REPORT INTRODUCTION & INSTRUCTIONS Pursuant to Tennessee Code Annotated (Tenn. Code Ann.) §45-18-109 1 , Tenn. Code Ann. §45-17- 110 2 , Tenn. Code Ann. §45-12-109 3 and Tenn. Code Ann. §45-15-106 4 , each license issued pursuant to these chapters shall expire on December 31 st . Each license may be renewed for the ensuing twelve-month period upon application by the license holder showing continued compliance with the requirements of each Act 5 and the payment of the nonrefundable supervision fee. A licensee making timely and complete application for renewal of its license may continue to operate under its existing license until its application is approved or denied. The completed renewal application and the payment of the annual supervision fee must be sent to the department on or before December 31, but no earlier than November 1, of each year. IMPORTANT: In order to renew your current License(s) you must file, submit and upload the following three (3) items per License type. Note, License Transitions, Licensure Renewal Applications and Financial Statements must be filed and submitted via NMLS; the Annual Report Form must be uploaded via the Annual Report Upload Center. All three (3) items must be filed, submitted and uploaded on or before December 31, 2018 in order for your current License(s) to be renewed for 2019. Check Casher Licensee: Licensees are required to transition their license(s) to NMLS by December 31, 2018. When the transition is approved, the license will also be renewed for calendar year 2019. If a Licensee has NOT yet requested transition, the Licensee must complete Steps 1, 2 and 3 below. If a Licensee requested transition prior to November 1, 2018, the Licensee must complete Steps 2 and 3. More information about NMLS can be found on the NMLS Resource Center or see TN Check Casher License Transition Checklist for more information regarding the Transition. 1. ANNUAL REPORT INTRODUCTION & INSTRUCTIONS

Pursuant to Tennessee Code Annotated (Tenn. Code Ann.) §45-18-1091, Tenn. Code Ann. §45-17-1102, Tenn. Code Ann. §45-12-1093 and Tenn. Code Ann. §45-15-1064, each license issued pursuant to these chapters shall expire on December 31st. Each license may be renewed for the ensuing twelve-month period upon application by the license holder showing continued compliance with the requirements of each Act5 and the payment of the nonrefundable supervision fee. A licensee making timely and complete application for renewal of its license may continue to operate under its existing license until its application is approved or denied. IMPORTANT: In order to renew your current License(s) you must file, submit and upload the following three (3) items per License type. • Upload the Financial Statements in NMLS. Financial Statements are uploaded separately under

the Filing tab and Financial Statement submenu link. See the Financial Statement Quick Guide for instructions.

3. Complete the Annual Report Form electronically, then save and upload your file according to

the instructions under the Annual Report Upload Center. The Annual Report Form includes a Financial Statement Questionnaire and an Analysis of the Business Activities conducted for the period January 1, 2017 through December 31, 2017. Information for the Financial Statement Questionnaire must be pulled directly from the Licensee’s Financial Statements prepared in accordance with generally accepted accounting principles.

Note: An additional step has been added for uploading the Annual Report. As you will see when you access the upload center, a security code is required in order to complete the upload process. The security code is Cp458a!#. In addition, the upload center will only be available between the hours of 7:00 AM and 7:00 PM Central Standard Time.

Deferred Presentment Services Licensee:

1. File Licensure Renewal Applications via NMLS. In addition to the Supervision Fee of $625 for

each license, note that the NMLS will charge a processing fee of $100 for the company headquarters (MU1) and a processing fee of $20 for each company branch (MU3). In order to initiate the Licensure Renewal process, licensees should access the NMLS Resource Center at http://mortgage.nationwidelicensingsystem.org and “click” the GREEN BUTTON marked ANNUAL RENEWAL. The filing deadline is December 31, 2018.

2. Submit Financial Statements, including, but not limited to, a Balance Sheet, Statement of Income or Loss, for the immediately preceding fiscal year end (December 31, 2017), prepared in accordance with generally accepted accounting principles (GAAP) via NMLS.
