• Piper Alpha – July 1986• Insured loss of $3.4b dollars and the loss of 167 lives with on 61 survivors.• The platform was responsible for 10% of the North Sea production• The key cause of the accident was a series of connected failures. The

Swiss Cheese Model.• The initial cause of the disaster was the failure of a pump that had been

temporarily sealed by hand. When the gas flowed, the pump failed and the explosion consumed the platform.

• Human error and lost paperwork had caused the pump to be used the and a integrated safety system together with training could have prevented this disaster.

• Texas City – March 2005• BP Refinery explosion that killed 15 workers and injured 170 other.• OSHA fined BP a record amount of money and increased this when BP

did not implement the required changes.• 437,000 barrels of oil is the capacity of the refinery• Again, the swiss cheese model of failures took place.• Maintenance failure led to a leading of oil and this pooled on the

ground.• The ignition was caused by a pickup truck engine that was left

running.• $87m dollars was the fine and the damage to BP reputation was even

larger.• And then Macondo happened

• Macondo – DeepWater Horizon Oil Spill – 2010• 11 people were killed and 4.9m barrels of oil were discharged into The

Gulf of Mexico.• Methane Gas from the well rushed to the platform and ignited.• Blowout preventer failed. Maintenance failures did not pick up the

failed battery.• The motive of profit was over riding the legal obligation to work safely

and the disaster was again trigger by a series of small failures leading the biggest oil spill in history.
