Risk assessment form

Production title:

Location address:

Area of consideration / concern Procedures / actions to be taken /

regulations to comply withElectrical Equipment (wires) Wire lying around, need to keep out of

way and make sure they are not a hazard. So no one trips over any. Also have to careful of liquids, therefore no liquids will be allowed near any equipment and wires will be safely hidden and out of the way.

Long Hours (fatigue – Also long travel period 2 hours)

Provide drinks and food so that actors/actresses do not feel de-motivated. Also make sure there is a toilet available. The club has a toilet and it is central to a shopping area with food places for meal times.

Camera position (Deck area is high) As we cannot use a ladder for health and safety purposes and the deck area is high. It makes it difficult to shoot at a high angle. However there is a stage provided and a few steps which will help this problem. Therefore we are not putting anyone’s health at risk.

Lighting Equipment (i.e – laser) Laser lights and certain lighting equipment cannot be operated for more than 15seconds at a go as it can cause health problems. To make sure no one is at risk there will be a lighting technician at the premise to operate the lights.

Makeup (allergic reactions) Due to skin problems – using makeup will be difficult in case any skin allergies occur. Therefore we will carry a first aid kit as well as getting the person to sign and state on a document that we are allowed to apply makeup on their face.
