I confirm that my child(ren) hold a current enrolment with COOSH Cambewarra.

I understand that half payment of fees must be made at time of placing this booking and that the remaining balance is to be paid by the first day of my child(ren)s attendance.

I understand my child(ren) must wear enclosed shoes daily (for safety reasons).

I understand I am to ensure my child(ren) bring lunch and a drink bottle daily.

I understand that inappropriate and/or violent behaviour exhibited by my child(ren) towards other children and/or Educators will NOT be tolerated. I understand that should an incident occur Educators will notify an ‘Authorised Contact’ to collect the child from COOSH/excursion venue immediately.

I understand incursion and excursion times indicated are approximate and may change.

I understand that transport will be by private bus unless stated otherwise.

I understand that the maximum number of children attending an excursion is 30 and number of staff is 4 unless stated otherwise.

I understand that the staff:child ratios for excursions will be based on 1:8.

I am aware that risk assessments have been prepared for each excursion and incursion and these are available from COOSH at the commencement of each day or as requested.
