Defects AFFECTING the level of control and particulars of any repairs or modifications required
1/ Contaminant dusts escape capture and can enter the operators breathing zone and workplace at thefollowing points:
a/ Cabinet Makers Shop - Bench extraction, when longer items are placed on their ends and the topsurface sanded.
Suitable repairs/modifications should be carried out to provide adequate Engineering Control.
Emergency Report and "Do Not Use" label issued to Trevor Hunt, at the time of inspection.
Defects NOT affecting the level of control and particulars of any repairs or modifications required
1/ Wadkin Table Saw 8. The level of control during the last part of cutting sheet materials is weak, with

dusts escaping capture to the rear / side of the hood. Suitable modifications to the top capture hoodshould be carried out to improve the level of control.
2/ Diplomat Belt Sander RHS Duct. The duct is split at the machine spigot.
3/ Router 5. Hevier dusts escape capture to the RHS of the capture hood and collect on the Router andfloor. Suitable modifications to the capture hood should be carried out to improve the level of control.
Other Observations
óR1/ Cross-reference with the last Thorough Examination and Test report
(E35223007741 - dated 20/10/2016)
óR2/ Test results recorded indicate no marked system performance deterioration since the last ThoroughExamination and Test.
óR3/ Condition at the Time of Test: Tests carried out with an average of 20 to 25 points open, as pernormal operating procedure. The following machines were witnessed in use: CNC Millenium Machine,Diplomat Belt Sander, Spindle Sander 15, Wadkin Table Saw 8, Spindle Moulder 14, Router 5, HorizontalSanders and Wall Saw.
óR4/ No air flow indicators are present at appropriate points on the system to indicate the system isfunctioning correctly. Reference should be made to HSG 258 regarding the installation of air flowindicators.
óR5/ The Effective Capture Distance of the Horizontal Sander No 7 Extract Hood is 0.75 m from thestopper block.
óR6/ The Effective Capture Distance of the Luna Horizontal Sander Extract Hood is 0.50 m from the face.
óR7/ The following parts of the system were not fully accessible;
a/ Fan and Motor Set due to mounting position within the filter enclosure. As a consequence the fan andmotor technical details could not be fully captured and should be provided along with the filter efficiencies.
b/ No safe accessible test points are available for the measurement of PSI and PSO. Suitable Test Holesshould be provided at the Filter Unit for the measurement of PSI and PSO across the primary filter.
It should be noted that inability to access the item(s) specified above has not affected the ability tocomplete an effective assessment of the Engineering Control provided by the system.

Comments/Notes 21/10/16 Urgent work required on 4 items. NOTE removed from service until fixed;. a/ CNC 4008 Machine b/ Cabinet Makers Shop - Bench extraction c/ De-nibbing Section LHS and RHS Benches when work pieces coated in white primer are sanded. d/ Wadkin Spindle Moulder 13 e/ Table Saw Refer to report for further details 24/10/16 As well as listed jobs Andy from Allianz said overall there has been a marked drop off in extraction performance. In trying to find reason for this we have checked ducting for blockages, checked all 3 motors are working, checked to see if bag shaker is working, all ok, however although bags are not holding sawdust and are all in place this doses not mean they are not "blinded" which would cause a significant fall in suction. Given that nobody can remember them ever being changed before it would be prudent to have them checked and cleaned if possible and or replaced. 31/10/16 Fumex Ltd. going to come and survey wood shop DCE and advise. 14/11/16 Contacted Fumex said they coming in morning 15/11/16 Fumex engineer called to assess problem, bags are shot. When he has identified make and model of DCE he will send quote and recommendations. 30/11/16 Still waiting for quote. 5/12/16 Contacted Fumex to check on progress, going to chase up and let me know? 21/12/16 No reply to date, will chase up in new year. 03/01/17 Fumex say they have received price for replacement bags and will be sending a quote soon.
2
11/01/17 Don't think this is the case, after speaking to them today, they are now asking for details of the extractor so they can locate replacement bags. Will keep you advised on any progress. 16/01/17 We couldn't find any details on M/C or on paper have advised Fumex, they are going to try and source bags without info. 06/02/17 Contacted Fumex again, going to get engineer who originally surveyed job to get in touch when he gets back in office. 13/02/17 Still waiting for some contact! 15/02/17 Rang today for progress, are going to ring back again! 15/02/17 Fumex rang, upshot can't help. Have sourced another firm, Scottex, seems promising, need to send them some details. 20/02/17 Scottex sent quote which seems quite reasonable, however this was only to supply bags, we need a replacement service as well. Have been in touch and am awaiting a reply. 22/02/17 Scottex came to view job and will send quote for total job, estimated to take a full day. 27/02/17 Quote accepted, Scottex will advise on date when new bags have arrived and arrange a date to fit, SF ok to fit in whenever they can come. 21/03/17 Job arranged for Fri 24/03/17. 03/04/17 Scottex came on Fri to change bags, on removing old bags it was found the shaker mechanism was broken and needed welding, so Trev called Star Weld back to take of it which they did. Scottex fitting bags on Monday now. 05/04/17 Found shaker frames broken in two more places, we removed frames and repaired. Scottex fitted remaining bags on Wednesday. Dust collector working ok and Scottex will come back in a month to check all ok. Biffa skip when returned had the back door broken, needs substantial repair. Mark has contacted Biffa.

At the moment we are using the one with the LEV attachment, but with the less good blade coverage. However, for the jobs currently being done this is less of an issue. ALthough the plan is still to upgrade the better blade protection with LEV attachment.

Defects AFFECTING the level of control and particulars of any repairs or modifications required
The level of control at the Cooksley Cross Cut Saw, the Band Saw and Wadkin Table Saw No 9 is poor. The contaminant escapes capture and enters the operators' breathing zone. Suitablerepairs/modifications should be carried out to provide adequate Engineering Control.
Defects NOT affecting the level of control and particulars of any repairs or modifications required
Several Duct Dampers are missing throughout the system and should be replaced.
Capture hoods are missing at Routers 4 and 5 and should be fitted in order to improve the effectivecapture.
Other Observations
Dusts are collecting in the ductwork to the Millennium CNC machine and should be cleared.
The Effective Capture Distance of the Horizontal Sander No 7 Extract Hood is 0.50 m from the stopperblock.
