The medical devices industry anticipates marginal revenue growth in 2012Of respondents across the industry, 54% are

"more optimistic" about revenue growth

for their company over the next 12 months

as compared to the previous 12 months.

With the current volatile market situation,

the medical devices market is expected to

register different rates of growth between

the developed markets in the US and Western

Europe and the emerging markets of Asia-

Pacific and Latin American countries. However,

strong growth in emerging markets such as

India, China, and Brazil, has contributed to an

increase in revenue optimism.

The top three key expected changes for

2012 are the introduction of "new products

and services," business "expansion in current

market and abroad" and "improvement in

operational efficiency." Overall, the key

expected changes in business structure for

medical devices manufacturers in 2012 have

not changed considerably from those identified

in 2011.

Mergers and acquisitions in the industry is expected to increase in 2012Industry executives expect to see increased

levels of consolidation, with 63% of

respondents anticipating either a "significant

increase" or an "increase" in merger and

acquisition (M&A) activity over the next 12

months.

Incidentally, there has been no substantial

change in expectations of consolidation among

medical device companies in 2012 when

compared to the 2011 survey results. Finally,

high operating costs, rising competition, the

need to increase geographical presence in key

markets, stringent government regulations,

high R&D costs, increased time to develop

and introduce new technologies, business

competence, and pressure on bottom-line

performance are the key stimuli for switches

to inorganic growth.

China, India and Brazil are the most important emerging marketsRespondents identify that China, India and

Brazil will offer the highest opportunities

of growth among emerging markets. The

expansion of business activities in emerging

markets, together with stronger economic

growth than other regions of the world,

government funding and reforms, changing

consumer lifestyles, increasing penetration

of medical insurance products, and a rise in

disposable income, has increased the demand

for quality healthcare services. This, in turn, is

expected to increase the demand for medical

equipment and other support services.

The US, Singapore, Taiwan and Hong Kong,

South Korea, Germany, and Canada are

identified as the most important regions for

growth among developed regions.

Although concern over the implementation

of healthcare reforms starting from 2013

has reduced growth optimism in the US, the

growth prospects are still high amongst the

developed countries. With an aging Baby

Boomer population, high unmet medical needs

and increased incidence of lifestyle diseases

including cardiovascular diseases, diabetes,

hypertension, and obesity, the US medical

device industry continues to grow at a brisk

pace.

Companies face key challenges in‘market uncertainty’, ‘responding to pricing pressure’ and ‘regulatory change’ "Market uncertainty," "responding to pricing

pressure," and "regulatory change" are the

most immediate business concerns for the

industry. Additionally, "market uncertainty" and

"responding to pricing pressure" are considered

leading business concerns for respondents from

small and medium-sized companies, whereas

"regulatory change," and "cost containment"

are concerns expressed by respondents

from large companies. Furthermore, buyer

respondents across all regions rank "innovate

products" as the leading action suppliers

should take to secure business, while senior-

level executives identify the need to "innovate

products," "improve payment terms," and

"reduce prices" as key initiatives for suppliers

to secure buyers' business.

Average annual marketing budget of suppliers

is expected to remain stagnant in 2012 in

comparison to 2011 ICD Research's industry

survey revealed that on average, the

Moreover, in 2012, 72% of supplier respondents

anticipate an increase in marketing budgets,

as compared to 61% in 2011.

In 2012, the average size of the annual

marketing budget of global medical device

industry suppliers is US$1.7 million, as

compared with US$1.9 million in 2011.

Incidentally, the European debt crisis,

increased price transparency, and global

competition have led to neutral expectations

of annual marketing budgets. However, 76%

of companies have a marketing budget of

less than US$250,000 for the same period,

which implies that companies intend to keep

budgets low, spending less time and money on

marketing strategies.

"Online content sites", "email and newsletters" and "social media and networking sites" to dominate future investment "Online content sites," "email and newsletters,"

investment. Conversely, "radio," "newspapers,"

Additionally, "market intelligence research,"

"CRM systems" and "competitor intelligence

research" emerged as important areas of

investment among marketing and sales

solutions activities, in 2012.

considered chief success factors for marketing

agencies. Survey results indicate that critical

success factors have changed slightly since

2010 and 2011, and although the leading

critical factor of the "ability to target specific

audience niches" remained in pole position

throughout 2010–2012, the importance of

"flexibility in customizing services," and

"thorough reporting and analysis" has increased

substantially in the recent survey. Additionally,

customizing sales processes ensures conformity

with the customer buying process and

suppliers look for marketing agencies that

offer products and services which add value,

to attract customer attention.

NEW ORLEANS—“Healthcare is broken

not just when it comes to the

cost but in so many other ways,”

said National Coordinator of Health IT

Farzad Mostashari, MD, ScM, delivering a

keynote address at the Health Information

and Management Systems Society (HIMSS)

annual convention. “Healthcare is broken

as a system…as a lack of systems.”

That system is failing everyone in healthcare

and everyone we love, he said. More

important than errors of commission are

errors of emission—“the errors we never see.

Surgeons save lives, but we don’t know whose

lives we’re taking when we only control blood

pressure for half of the people who need it.”

Data and information can make the invisible

visible. “And then we can do something about

it. We can do quality improvement not just

retrospective accounting.”

To fix the system, incentives matter. “If you

pay people for the outcomes they’ll figure out

away to get the outcomes.” The species Homo

economicus is limited to CFOs of hospitals,

Mostashari joked. “They are a rare breed” that

operates purely on rational economics.

Professional ethos is at play as well, he said.

“It’s not just about the money.” He cited an

outcomes project during his time as public

health director in New York City, in which “we

said that we recognize that it takes more time

and effort to bring someone’s blood pressure

under control and here is some money to

offset the costs. It turns out, that works

better.”

Payment rules make a big difference too, he

said. He cited the Centers for Medicare &

Medicaid Services' Acting Adminstrator Marilyn

Tavenner who said without that new payment

and delivery models cannot succeed. “Fee for

service isn’t dead yet, but there are a lot of

plans for its demise.”

You need the incentives and the IT, he said.

“The ability to manage information is going

to be a part of doing business. In healthcare,

when things just happen based on standing

orders they get done more often. You don’t

need pop-up reminders every second saying

‘give the flu shot.’ Just give the flu shot!”

The economic system responds, Mostashari

said. “Things aren’t static.” For example, the

percentage of providers using computerized

physician order entry has increased from 65

percent to 85 percent and e-prescribing went

from 55 percent to 73 percent in one year.

Having the functionality and ability to do it is

the first step, he said. “What remains is the

will and leadership to use it well.”

Mostashari addressed complaints that various

measurement thresholds in the Meaningful Use

requirements are too hard. “You destroyed

those thresholds! And once you do it, you’re

not going to do it for every fifth patient.

You’re going to change the workflow and do it

for everybody.”

He acknowledged that Meaningful Use is not

easy, and meaningful users are earning their

incentive pay. Looking ahead to penalties

for not meeting Meaningful Use, he said “my

goal is for nobody to be penalized. We want

everybody to succeed on this.”

Don’t tell me healthcare can’t change, he said.

“It can. A lot of what seems to be resistance

that is just uncertainty.” Regional extension

centers were developed to “shrink the

change and provide a script.” How are these

organizations that didn’t even exist three

years ago doing? More than 130,00 providers

registered with a REC, 101,000 have gone

live on an EHR and more than one-quarter of

providers have demonstrated Meaningful Use.

Mostashari said he spent time in the HIMSS13

exhibition hall and saw many claims regarding

scalability. “You know what scales? Hard work,

determination, perseverance. Grit scales.”

He also visited the Interoperability Showcase

which first debuted at the HIMSS convention in

2006. “Best. Interop. Showcase. Ever,” was his

evaluation this year.

He also addressed recent studies that claim

that physicians are increasingly dissatisfied

with their EHR. “You can’t beat the data,”

he said. Some of those studies have a 1.3

percent response rate. The gold standard U.S.

physician survey is from the National Center

for Health Statistics, he said.

Going forward, “our shared challenge this

year is the exchange of information. Does

it get from where it is now to over there?

Interoperability is not the same thing at all.

Interoperability is when you get information

can you understand it, can you use it, can you

incorporate it into your own information?”

Despite the successes to date regarding

adoption of health IT, only 24 percent of

hospitals are exchanging clinical summaries

with outside hospitals.

“This is not where we want it to be. Twenty-

four percent is nothing to celebrate even if

it’s triple what it was three years ago. It’s not

good enough and we’re going to change this.”

more aware of the final criteria included in the

2014 edition of EHR standards. “I am shocked

at how many people who should know better

don’t even know what’s in there.” Many of

the complaints and suggestions people have

made over the years are included in the 2014

edition, so providers need to review it and be

aware of the new certification standards.

Mostashari said his office and the industry must

work on privacy and security issues. “That’s

one area where we intend to be forward

leaning. You said don’t regulate too soon and

don’t set in stone regulations before we know

what the problems are. We plan to do a lot

of education and a lot of listening. My hope

is that you will step up and do self-regulation

that we can endorse.”

He also cited the recent announcement from

CMS and ONC that the agencies intend to make

sure it is more profitable to share information

than to hoard it. “That’s a big deal. No one

should make a profit holding patient data

hostage,” he said to enthusiastic applause.

In the future, “the patient certainly is

going to be that blockbuster drug.” He

referenced studies indicating text messages

are as effective as pharmaceutical agents in

helping people quit smoking. “We are going

to need smart patients who access their

own information. They are going to push the

culture change that has to come.”

Imaging biomarkers have been developed

for use in early cancer diagnosis, staging

and restaging of disease and monitoring

the effects of therapeutic interventions.

In addition, biomarkers for evaluating

coronary function and perfusion are well-

established. Imaging biomarkers targeting

neurodegenerative diseases also are widely

used in the clinic.

18F-fluorodeoxyglucose will remain the most

important imaging biomarker in neurology

and oncology. However, with our increasing

understanding and knowledge of biology, there

is a need for refinement to better visualize

such processes as tumor neo-angiogenesis,

apoptosis, metastatic potential, receptor

status and many more.

For neurological applications, imaging

biomarkers that allow for a reliable detection

of plaques and/or tangles would greatly assist

in developing those drugs that might arrest

neurodegenerative processes. However, all

new imaging biomarkers need to undergo a

rigorous validation process from cells through

animal experiments to clinical trials. This

pathway is time-consuming and expensive, and

innovative approaches are needed to make

this process more efficient.

There are several important reasons for

accelerating the development of imaging

biomarkers. First, personalized medicine will

require markers that allow the prediction of

which therapeutic is likely to be effective in

which patient; second, in drug development,

imaging biomarkers could be used to

determine whether the therapeutic target is

present and whether the target is “hit” by the

drug. Finally, surrogate markers of treatment

responses such as FDG can be used to assess

therapeutic responses early and robustly

which, in turn, can lead to appropriate

changes in therapeutic approaches.

We will likely look back on this decade as the

decade of the biomarker—a time in which

medical researchers recognized opportunity,

challenged technology and themselves,

and developed and validated organ-specific

biomarkers that will better predict patient

outcomes.

Among the challenges are validation

and standardization procedures and the

establishment of true reference standards

for imaging biomarker performance. These

necessary housekeeping activities are very

expensive given that dozens, if not hundreds,

of biomarkers are being developed. Without

a close interaction between government

agencies and industry, the successful

translation of imaging biomarkers into the

clinic will not be possible.

Imaging biomarkers have their fair share

of opportunity and challenge, but great

successes have been achieved—in assisting in

diagnosing, staging and monitoring sarcoma,

metastatic breast cancer, lymphoma, head

and neck cancer and gastric cancer as well as

coronary artery disease, significant infection

and identifying Alzheimer’s disease prior to

the onset of problems as well as Huntington’s

disease.

Implantable medical device industry has

made remarkable progress in the past

30 years. From implantable cardiac

pacemakers, implantable atrial defibrillator,

cochlear implants, implantable electrode

(used for the treatment of Parkinson's

disease, epilepsy and other diseases),

implantable insulin pump, to all kinds of

plants into the stent, these products on the

market, soon to become the best-selling

products in the medical device market. To

be sure, implantable medical devices has

become an important branch of the medical

equipment.

In recent years, the research and development

of a new generation of implantable medical

devices developed in the United States and

other developed countries in full swing,

such as wireless heart monitor, real-time

blood glucose monitor, remote life monitor,

implantable cancer detection instrument.

These have been listed or are in clinical trials

of medical devices, smart, convenient shape

or use, can save the cost of medical care

and play better efficacy, even timely predict

disease.

"Scientific American" magazine recently

reported that the incidence of cancer in

countries around the world showed a rapid

upward trend. How to accurately judge the

efficacy and tumor progression after receiving

chemotherapy, radiotherapy or surgery, is a

major problem.

Massachusetts Institute of Technology

researchers new developed an implantable

These days, robot-assisted surgery is part of

the daily routine in many operating rooms.

High precision and gentle surgery speak in

favor of robotic surgery. However, high costs

not just for the equipment, but also for its

maintenance argue against it. Nevertheless,

this development sector has a promising

future.

The first surgery with a robot took place in

1985. With the PUMA 200, surgeons back then

operated on a brain tumor of a male patient.

Nowadays, surgical robots are being used in

many different disciplines. Whether it is in

neurology, cardiology or urology, wherever

highly accurate and often long surgeries are

required, robots are treasured assistants.

Doctor Ralf Westphal oversees several projects

in the field of robot-assisted surgery at the

Braunschweig University of Technology: “The

use of a robotic, tremor-reduced and high-

precision navigation system can contribute to

increasing accuracy and safety, in particular

in threshold areas, as is often the case in

neurosurgery.”

The pros and cons

Robots are also often used for milling of entry

points to position cochlear implants. Since the

surgery can become risky in critical areas such

as facial nerves, precision during surgery is

especially important, stresses Westphal. “The

combination of high-resolution 3D imaging,

computer-assisted surgery and robot-assisted

milling can contribute to increased precision

and safety in this area.”

Another advantage of robotic systems is the

chance to perform minimally invasive surgery.

Perhaps the best-known example of such a

robot is da Vinci. The system consists of a

console from which the surgeon operates the

device, three robotic arms and one patient-

side cart, which is equipped with a 3D camera.

Thanks to the flexible arms, the surgeon

is able to perform very precise and small

incisions on the tissue.

During an endoscopy of the nasal cavity,

surgical robots make it possible for the

surgeon to operate with both hands. This

way, the instruments can be controlled in

a more ergonomic fashion and the number

of required instrument changes reduced.

Disciplines requiring higher effort also benefit

from surgical robots. Physicians in the field of

orthopedics or trauma surgery are confronted

with bone fractures. Exact positioning of bone

fragments is an important criterion here. “It

is frequently impossible to muster up the

enormous effort and achieve high precision at

the same time during manual interventions.

Generally, this is not a problem for robots”,

Westphal points out.

Aside from numerous advantages however,

robotic systems can also have significant

downsides. Usually, frequently mentioned

shortcomings include very high costs. It is not

just the device that is expensive; oftentimes

you also need specialized technical staff

that is capable of operating the robot. The

maintenance of the systems also presents

hospitals with major financial burdens.

“Another point of criticism is the additional

time that is usually needed to properly set up

the robotic systems before or at the beginning

of surgery in the operating room”, Westphal

emphasizes. This is why the use of robots is

only worthwhile for surgeries that are time-

consuming anyway, or for those surgeries

where the increased precision of the systems

increases patient safety.

Radiation instead of incision Yet it is not just conventional surgery that

counts on robotic surgery. These technologies

are also in high demand for radiation therapy.

With the CyberKnife system for instance,

pinpoint and gentle radiation can be delivered

to tumor patients. ”The CyberKnife is a system

where the radiation source is positioned via a

robotic arm to aim photon beams directly at

the target region”, explains Doctor Alexander

Schlaefer of the Medical Robotics Group in our

MEDICA.de interview. Due to the flexibility

of the beam alignment, extremely focused

regions can be reached with a high dose of

Google Glass, the search giant's internet-connected glasses

prototype, is one of the most exciting

products to come around in a while. That's

because the implications are huge. Glass

will bring everyday activities closer to our

senses, and enhance our interactions with our

surroundings. We'll be more connected to the

Internet than we ever have before and we'll

be able to see useful images and data overlaid

onto the real world.

Granted, Glass is still in its early days. It

doesn't yet have its own cellular radio, so

it has to sync up with mobile phones via

Bluetooth to access Wi-Fi and 3G or 4G data

connections. As of now, Google Glass can do

things like record video, send text messages,

provide translations, and give directions.

But Google has been holding hackathons for

developers to come up with more ways to use

Glass. The search giant also recently held a

competition to give everyday people a chance

to get their hands on Glass and its API. Anyone

who wanted Glass had to write on either

Twitter or Google+ what they would do with

Glass and include the hashtag #ifihadglass.

Glass does raise some privacy concerns. And

before it really takes off, Google will need to

find a way to make it appeal to the masses.

As of now, Glass seems to appeal more to

men than women, according to a TechCrunch

analysis of #ifihadglass tweets and Google+

posts. It's also pretty expensive at $1,500. Still,

Google Glass has the potential to disrupt a lot

of industries, from Hollywood to air travel.

GPSConventional GPS is essentially already dead

because so many people have the same service

on their phones. But since many states ban the

use of cell phones while driving, Google Glass

will be a great alternative. Sure, you could

pull up the directions before you start driving

and even receive voice-based turn-by-turn

directions. But people still like to see where

they're going — and Glass does that.

HollywoodGoogle Glass will open up the doors to more

creative types of videography. Already, there

seems to be a push toward shooting movies

on iPhones and creating new ways for people

to consume and immerse themselves in video.

With Glass, we'll be able to record video from

an entirely new, first-person perspective.

When Google first announced Glass, for

example, Google co-founder Sergey Brin used

skydivers to demonstrate how people can now

capture video from anywhere, without needing

to physically hold a device.

Air travelJetBlue recently highlighted what it would

do with Google Glass. If JetBlue had Glass,

it "would continue to help the world view air

travel through a whole new lens," the airline

company wrote on Google+. A few of the

possibilities: real-time notifications for your

flight status, a map for available outlets in the

boarding area, taxi fare estimates for when

you're leaving the airport, real-time garage

capacity information, and directions to the

baggage claim area.

That means schools will need to determine the

best use cases for Glass in the classroom.

But Glass also has vast implications for

language learning since it can provide real-

time, text-based language translations of what

people are saying.
