MyChartPLUS Account Activation

This Tip Sheet provides an overview of different methods that can be used to activate a MyChartPLUS

account.

Instant Activation

Instant Activation offers patients the ability to activate a MyChartPLUS account instantly. An email is sent

to the patient containing a button that will take them directly to the MyChartPLUS signup page. The

patient must validate their identity using their date of birth when signing up from the instant activation

button. The button in the email expires after 24 hours.

Here is an example of an Instant Activation email:

If the patient selects the Sign up for MyChartPLUS button within 24 hours of the email being sent,

they are brought to a page where they are prompted to select a username, create a password, and verify

their identity using their date of birth:

If the patient click on the button after 24 hours, an activation code will pre-populate into the

MyChartPLUS signup page. This code will remain active for 45 days. The patient must validate their

identity using their date of birth and phone number when signing up with an activation code.

Sending an Instant Activation E-mail

Instant activation messages are automatically sent to patients without MyChartPLUS accounts for the

following events:

Appointment scheduling

Appointment check-in

Appointment check-out

ED visits, excluding those where the patient left without being seen

Registration and scheduling staff can manually send an instant activation email through the MyChart

Signup button in the toolbar in the Registration activity.

Clinical staff can send an instant activation message through the MyChartPLUS status icon in Storyboard.

The default email address on file in the patient’s demographics is where the instant activation email will be

sent. If the email is manually sent, staff must confirm the email address before clicking Send.

Self-Signup

Patients can create a MyChartPLUS account without an activation code using self-signup. The following

information is required for self-signup and must match or very closely match the information in the Epic

medical record:

First name

Last name

Address

DOB

Legal sex

Last four digits of SSN

Note: The entire SSN must be on file as staff cannot enter partial SSNs, but only the last four

digits need to match

Email address

Patients can access the self-signup screen by clicking the Sign Up Now button beneath “New User?” on

the MyChartPLUS website, mychartplus.org. On the next page, they will see a button to Sign Up Online

beneath “No Activation Code?”
