Thomas J. Weisner Mayor of Aurora, 2005-2016

On October 30, Thomas J. Weisner retired, having served as Mayor of Aurora for nearly twelve years. During Mayor Weisner’s tenure, neighborhoods were re-engaged, crime was reduced, education supports were put into place, housing needs were addressed and numerous development projects were completed including RiverEdge Park and a beautiful downtown. All the fundamentals of a healthy community were strengthened under Mayor Weisner’s leadership.

Further, Mayor Weisner understood that access to affordable health care is important to everyone, not just those with little or no health insurance. Business owners need employees to be healthy and productive. Healthy students are better able to learn. Childhood immunizations help protect a whole community. Mayor Weisner’s efforts to support health care providers, including VNA Health Care, have contributed to a healthier community overall.

In short, among other things, Mayor Weisner’s legacy includes a path to a healthy life for thousands of residents of Aurora. Thank you!

In the 1850’s Florence Nightingale declared that good food, clean air and a sunny environment contribute mightily to a person’s well-being and their ability to heal and recover. She understood that a healthy environment, nutritious food and social interaction are fundamental to good health. Over 150 years later, a “wellness” revolution is in progress in the U.S.

As we know, “wellness” is a broad concept. In the field of healthcare, we approach it from various angles including, most recently, the “social determinants of health.” These determinants, as defined by the Centers for Disease Control, include economic stability, education, social and community context, neighborhood and the built environment, as well as health and healthcare.

Despite all of our efforts as individual health care providers, it takes government, business, social services and health care working together to create healthy communities. VNA is fortunate to work with very forward-thinking public servants across our service area. In fact, VNA’s tribute to Mayor Weisner of Aurora is in recognition that economic development, good schools, a well-supported social services network, public safety and affordable housing are fundamental to good health and wellness and his work to continually improve the foundation will have a lasting effect on the Aurora community.

Meanwhile, VNA is working to do its part by providing high quality medical and healthcare services to 65,000 unduplicated patients per year (and growing), regardless of the patient’s ability to pay for them. In addition, VNA registered nurses and social workers assist thousands of patients each year with housing, economic and social issues.

VNA Healthy Lifestyle Center & Wellness Kitchens are another strategy to help people along their path to a healthy life and were simply a dream when we purchased our first facility at 400 N. Highland Avenue in Aurora in 2006. The dream became a reality in 2014 with support from the Dunham Fund and City of Aurora when we created our first Healthy Lifestyle Center & Wellness Kitchen. Currently, VNA provides 65 wellness classes each month. Homemade soup, made right in the Wellness Kitchen by VNA volunteers, is hand-delivered to our homebound patients. With this success, VNA opened a Healthy Lifestyle Center & Wellness Kitchen in Romeoville and plans to open one in Elgin with community support. As often happens, one dream leads to another… Inspired by “Camerados” – a program established in northern England to combat social isolation, VNA will launch the “VNA Community Living Room Program,” in the coming year. Run by VNA volunteers, the program will offer a casual and supportive environment for community residents seeking social interaction and an opportunity to help others. After all, the path to a healthy life is paved with the help of each other!

• 81.7% of Kane County adults report one or more cardiovascular risks- being overweight, smoking, having high blood pressure or having high cholesterol • 63% of Kane County adults are overweight• 81% of all Kane County deaths are a result of chronic disease• 1 in 11 adults nationwide age 20 and up has diabetes

VNA created the Healthy Lifestyle & Wellness Kitchen with a vision to design a space extending a holistic approach to prevention and wellness programming.  This approach includes evidence-based group education classes which have proved to be a great success.  Patients quickly engaged in the format, often bringing family members. Many choose to continue attending classes once they have completed all of the self-care management program components. Classes are now offered without appointment, in English and Spanish and at a variety of convenient times including daytime, evening and Saturday. Wellness programs include Step by Step Diabetes, Step by Step Wellness, Step by Step Heart Health, Step by Step Weight Loss, and Step by Step Maternal Wellness.

Why Do We Need Wellness Programs?A multi-disciplinary Diabetes Resource Team was formed in 2015 to support the Step by Step Diabetes offerings and care for patients with diabetes across the organization.The team includes care coordination nurses, nurse practitioners, dieticians, clinical pharmacist, podiatrist, and community health support staff. Each team member brings their area of expertise and focus to providing care to patients with multiple, complex health issues.

The result of VNA’s focus on wellness? A significant impact on the health outcomes and improvements in healthy behaviors.

100% of recent wellness program participants established a healthy living goal and:

• 72% of participants reduced their blood pressure • 64% of participants reduced their weight• 100% of participants had an increase in physical activity • 88% of participants reported eating a healthier diet

around. I started attending the Wellness class and it has helped

me in so many ways. I love to cook but I learned how to practice

portion control and that helped me to start losing weight. This has

The US Department of Health Resources & Services Administration (HRSA) recently released the 2015 Uniform Data System (UDS) Reporting Measures, which rank and compare community health centers on multiple quality measures.

COMMUNITY ENGAGEMENT

Salad Bowl Garden Reaps Rewards Ten classrooms along with the football team participated in the “Salad Bowl” garden project at East Aurora High School. The project was a collaboration between VNA Health Care’s school-based Tomcat Health Center, East Aurora High School, School District 131, the Fox Valley Park District, and the University of Illinois Extension. A Kane County Health Department Fit for Kids Grant funded the garden project. The sowing, planting, maintaining and harvesting kept students involved and engaged over a period of seven months. The garden produced over 500 pounds of produce, enough for students to take food home to their families and supply the restaurant management classes with ingredients for healthy cooking.

“My honors geometry students created a layout of the garden, based on the planting guidelines. I have never seen my students so excited about something. Their enthusiasm led me to get involved too, and I came in over the summer to help with transplanting and weeding. Now I’m working with Sandra to coordinate students to help during the school day and after school.”

COMMUNITY ENGAGEMENT

- Stacy Conrad, East Aurora High School Mathematics Department

VNA Health Care hosted a local event in Aurora on August 5 for The Global Big Latch On. The Global Big Latch On takes place at registered locations, offering women a place to gather together to breastfeed and offer peer support to each other. At a specified time, thousands of breastfeeding women and their babies across the world participate in a synchronized breastfeeding event. VNA’s event hosted 34 women with 21 children latching on during the event.

“Breastfeeding provides many health, nutritional, economical and emotional benefits to mother and baby. Breastfeeding contributes to the normal growth and development of babies, and reduces the risk of infant morbidity and mortality, adult obesity, diabetes, cardiovascular disease, osteoporosis, and premenopausal breast cancer and ovarian cancer for both mom and baby.” - Rosalinda Saucedo, Director of Community Health

VNA Volunteer Cathy Beckman manages the ‘healthy soup kitchen’ project. Every Monday morning you will find Cathy making soup in the Wellness Kitchen at VNA’s Highland Avenue location. VNA nurses bring soup to patients during home visits. The gift of soup is another way to nurture our patients and provide them with a break from worrying about preparing a meal.

Will County Expansion Not only is Will County home to over 65,000 uninsured individuals, but in 2015 the ratio of primary health care physicians to population was 36% lower than the state average. VNA’s two new locations in Romeoville and Joliet make affordable, quality healthcare within reach of the men, women, and children who need it most. In November 2015, VNA opened its tenth health center at 2400 Glenwood Avenue in Joliet. In January 2016, VNA moved from its temporary Weber Road location in Romeoville to a newly built clinic at 160 N. Independence Boulevard. The 10,340 square foot clinic has three wings with 22 exam rooms.

Rosary High School, Aurora – Spike Out Cancer Volleyball TournamentRosary’s volleyball teams led by Coach Chandler and Mary Lou Kunold dedicated a game night in October to supporting cancer awareness and raising funds through a raffle, silent auction, and 50/50. The proceeds from this event support VNA’s Hospice and Give a Girl a Mammogram programs.

Mooseheart students and staff led by Bryan Miller, Coordinator of Recreation & Camping, and Cathy O’Brien, Mooseheart Community Nurse donated proceeds from an October Walk to support Give a Girl a Mammogram fundraising program.

The Salon at Carson’s, located in Northgate Shopping Center in Aurora hosts a ‘Cut for a Cure’ event every October. All charges collected at the event for hair services and a portion of all salon product sales are donated to VNA Health Care to support the Give a Girl a Mammogram program.

VNA receives Federal grants for Oral Health and Information Technology Enhancements

June, 2016 - $350,000 grant for Oral Health VNA received $350,000 to expand its oral health services. VNA will be able to add a fifth dental operatory to the dental clinic suite, located within VNA’s health center in Aurora. The grant allowed VNA to hire an additional dentist and a full-time hygienist.

September, 2016 - $147,423 grant for Information Technology EnhancementsVNA received $147,423 to support health information technology enhancements to improve efforts to share and use information in the electronic health record system to support better decisions, leading to better patient health outcomes. The grant will be used to replace hardware to maximize capacity to securely accept and transmit information internally as well as with external partners such as local hospitals and health plans.

Local funders provide critical support to VNA programsThe Community Foundation of the Fox River Valley awarded capital funding to VNA to support the purchase of the HealthStream Learning Center. The Dunham Fund supports community outreach to promote Medicaid enrollment. EFS Foundation and Grand Victoria Foundation support primary care and community outreach in Elgin. Aurora Rotary Club, INC Board NFP, and Naperville Exchange Club support the Healthy Families America Program. Aurora Township and Kendall County Board support primary care in Aurora. Fox Valley United Way supports Healthy Families America, Home Health and WIC. Kane County Health Department supports Fit for Kids programming and VNA’s Salad Bowl Garden at East Aurora High School.

This year’s fundraiser paired wine and cheese tastings to support the Aurora Community Heart Clinic at VNA and VNA’s wellness programs. Sommelier Rob Lehner led guests through a 20 minute introduction to wine tasting to kick off the evening. Guests enjoyed a ‘wines-around-the- world’ theme with tastings from France, South Africa, and Chile. Between tastings, bidding on the silent auction baskets became very competitive!
