2 PREFACEThis guide is written for all members of the clinical team who use or handle the whole system or parts of it. Before using the system you should read this guide carefully and acquaint yourself with the sys-tem.

2.1 Visual cuesThis guide uses the following visual cues to alert you to important information.

Warnings are used to alert the user to possible injury, death or other serious conse-quences associated with the use or misuse of the equipment.

Cautions are used to alert the user to the possibility of a problem with the device as-sociated with its use or misuse. Such problems include device malfunctions, device failure, damage to the device or damage to property.

3 INTRODUCTIONBrainlab offers a custom-made data transfer solution for almost every CT, MRI, X-Ray, SPECT and PET scanner. The PatXfer user guide helps you to understand the general procedure for converting patient data to Brainlab format.

Due to the wide variety of scanner types and data transfer media currently available, it is not possible to discuss every single data transfer type in detail. If you have any specific questions or problems relating to a particular scanner which is not presented in this manual, please contact Brainlab sup-port for assistance.

4 PATXFER DATA TRANSFER4.1 Basic instructions

4.1.1 How to use this manual

For optimum use of PatXfer, follow the directions in this manual while actually using the patient data transfer software. The manual describes each software function clearly.

Scan instructions Patient data is necessary before beginning PatXfer. To avoid initial problems, carefully read the scanning instructions in the Appendix.

Error messages For the user´s convenience, a list of explanations of possible error messages can be found in the Appendix. They are alphabetically sorted.

4.1.2 Starting PatXfer• Start the PatXfer software by double clicking on the start icon.

The initial screen The initial screen shows a title bar with the program name and software version number. In the mid-dle of the screen the Platform Selection dialog is displayed, which is the first page of the wizard-driven user interface. Every page contains one information line displaying environment specific mes-sages. Depending on your platform, the background screen may be omitted and only the Platform Selection dialog appears.

Shutting down the System

• To shut down the system, first exit PatXfer by pressing the Exit button.

• Then click on the shut-down function of the Windows or Windows NT computer, which is located in the start menu.

This allows the computer to save all changes to the working files and to properly close all applica-tions and services. The system will then prompt the user to turn off the computer.

To prevent possible data loss, never use the power switch on the computer to quit the System. Exit PatXfer first, and then click on the Windows/Windows NT shut-down function.

4.2 Using PatXfer

4.2.1 General

Wizard A wizard-driven user interface guides the user through the transfer process. During the image trans-fer, a reporting system provides up to date information about the status of the transfer. After PatXferhas loaded the images the user can select the images to be converted into the Brainlab file format. PatXfer guides the user through this process by gathering logical corresponding images in one ‘set’. Finally, PatXfer converts and saves the images in the default patient directory using the Brainlab file format.

Flow chart Shown below are all the steps of the PatXfer wizard. The steps within the solid boxes are mandatory. The steps in the dotted boxes are optional, depending on the selected platform.

<Back, Next> All wizard pages can be easily leafed through:

• Clicking Next> proceeds to the next wizard page.

• Clicking <Back returns to the previous page.

Help • The online help can be accessed on all wizard pages. Please use the question mark button ?, to start the help.

• The online help is not available for Alpha computers.

Exit All wizard pages include an Exit button.

• Use the Exit button or the Esc key to quit the program at any time during the process. You are prompted to confirm the selection before the application will close.

Image format On the initial page the platform can be changed according to the corresponding software. PatXferallows saving patient data in different formats as Standard Brainlab format and new, more advanced image formats according to additional Brainlab software.

Never select a platform if the appropriate planning software is not available, as the pa-tient data may then not be compatible with the available software, even if the transfer is successful.

4.2.3 Change platform• Click the Change button to open the available platform list. Please select one of the platforms,

depending the Brainlab module you use for the images.

About PatXfer Clicking the About button opens an information box displaying the version of PatXfer (including the build-number in brackets), the support library, and all available transfer modules. Furthermore, the Dongle license with the date and name of the online help manual file will be shown. If there be prob-lems with the image transfer, please forward the software version information to Brainlab support.

Modality • Select the scanner modality by clicking on one of the buttons:

- MR

- CT

- X-Ray

- Other (this includes PET, SPECT or special scanner data transfer)

Data Format If different scanners of the same modality are available (e.g. two CT scanners from different manu-facturers), select the appropriate scanner in the Data Format box. Click the small arrow on the right side to list all the additional scanner and media types.

Medium type Once the appropriate scanner has been selected, the transfer media can be loaded. The transfer media can be a tape, an optical disk or a floppy disk, depending on the data transfer.

Network transfer When using a network connection to transfer patient data to the Brainlab computer, a Brainlab Ser-vice Engineer provides specific instructions on-site. Prior to sending images from a CT/MR scanner make sure that the Brainlab computer has been turned on and has been connected to the network. An additionally installed program automatically stores the images in a local folder, which can be transferred later to the Brainlab format via PatXfer.

Default The most recently selected modality and scanner type is the default setting when PatXfer is restart-ed.

Transfer During the transfer, the set to which the current image is added is highlighted. The corresponding image is shown in the Set Preview area.

The display can be switched between the following:

• Preview displays the images which have already been transferred.

• Info provides information about the parameters for the highlighted image set (number of slices, scan date, image type, series number, study number, pixel number, pixel size and matrix size). During the transfer the number of slices is incremented every time a new image has been loaded.

• Comment displays the original comment from the scanner. Depending on the platform, it may be possible to add an additional description in the comment field.

Selecting the image set

All available image sets for the current patient are listed in the Image Set box.

• Click on the name of the set to view the corresponding information in the Set Preview/ Info/ Comment area.

• To select the image sets to be stored on the Brainlab system, click the small box on the left side of the set name (a tick will appear in the box). Only the selected image sets will be stored.

The image set including the most images is selected by default.

Maximum number of image sets

The maximum number of image sets mainly depends on the selected platform. The Brainlab stan-dard platform allows you to save the following number of sets in a single patient folder:

• two axial CT image sets

• one MRI image set of the three possible geometries: axial, coronal and sagittal

• three X-Ray image sets

• two PET/SPECT image sets of the three possible geometries: axial, coronal and sagittal

PATXFER DATA TRANSFER

To add new image sets to an existing patient folder, remember the platform-dependent number of allowed image sets for one patient folder. It may not be possible to add and store the sets to a specific existing folder.

Delete Img • A single image can be deleted with the Delete Img button.

• Use the slider in the Set Preview mode to select the image to be deleted.

• This button is not available during transfer for X-ray image sets and for all sets which contain only one image.

Stop • Press the Stop button to interrupt the transfer.

No further images are then be transferred. The transfer can be continued, but in this case not all images are available for the selected patient. To transfer all images, return to the previous page by clicking on the Back button and re-start the transfer.

Logfile • Press the Logfile button for more detailed information about each transferred image.

The logfile is displayed in an external viewer. It is updated during the transfer. Once an image set is saved and the transfer is finished the logfile.log file is moved into the patient folder.

• The logfile has to be closed manually.

Make sure to type the correct patient orientation in the scanner protocol. If a patient has already been scanned and the scanner operator used the wrong orientation, please call the Brainlab hotline to solve the problem.

4.2.8 Localizer Selection

Image set display As in the previously explained dialog boxes, all selected image sets are displayed one after another and all available slices can be viewed using the scrollbar.

Localizer for Set... If the patient was scanned using a localizer, select the localizer used from the list provided in the localizer combo box.

This box lists the three most recently used localizers in the top list. The list below holds all available localizers. The localizers are saved in the initialization file every time the system proceeds to the next page.

Always ensure that the correct localizer type has been selected. Otherwise the patient images are not able to be localized using additional Brainlab software and treatment planning is impossible.

Never choose a localizer, if the patient was scanned without a localizer.

In case of adding a new image set with a localizer to the existing patient folder, you must not have other localizeable image sets in this plan, which do not fit to the latest physical position of the head frame related to the patient skull. Otherwise different ste-reotactic coordinate origins are found corresponding to the patient anatomy, if all lo-calizeable image sets are localized and fused. This would lead to safety critical risks for the patient due to dangerous confusion about stereotactic planning and possible misinterpretation of planned stereotactic procedures.

Create a new patient folder

• Activate the radio button Create New Patient Folder to store image sets in a new patient folder.

• If necessary edit the patient name in the line Patient Name.

Add image set to existing patient folder

• Activate the radio button Add image set to existent patient folder to store the image sets in an already existing folder.

• Select the corresponding patient in the list Patient Name.

This is typically done when an additional set of MRI images of the same patient is transferred to an existing CT image set of the same patient.

To ensure that the correct patient has been selected, check the Patient ID. The list at the bottom provides set information of the selected patient (type of set (modality), set index, orientation and number of images, scan date).

Whenever new images are added to an existing patient folder and the patient’s name or ID numbers are not identical, a confirmation is required.

Normally patient name or ID should not change for different scanning modalities or examinations.

Due to platform restrictions, it may not be possible to add the selected sets to an ex-isting patient, as the number of supported image sets per patient folder is limited. In this case, a new patient should be created or the respective number of sets should be deselected.

Always use at least one folder for each patient. Incorrect usage could lead to patient injury, due to incorrect image referencing.

Finish • Click Finish to store the different sets.

To create a squared image matrix, the system performs interpolation if necessary and then saves the data. A progress bar mirrors the current position of the process.

Next Depending on the selected platform, PatXfer is closed and a new Brainlab application started. In this case the Finish button is labeled as Next.

After all images have been successfully transferred a box appears stating the end of the transfer process.

Restart If a platform is not available, PatXfer is restarted and by default the Patient Selection wizard page is displayed. This avoids time-consuming re-scanning of the patient directory and the user can easily proceed with the next patient. Of course the user can go back and choose another platform or trans-fer module.

If another restart wizard page is desired, ask the Brainlab support to configure the settings accord-ingly.

4.2.11 X-ray transfer

DICOM The DICOM X-ray transfer is the same as the standard CT and MRI transfer up to the Set Selectiondialog box. Due to the fact that no image orientation information (lateral or frontal) is available in the header, the orientation has to be manually defined.

Valid X-ray set To make the selection as easy as possible, one image after another is displayed until a full set has been created. A complete valid X-ray image set for a Brainlab system contains one lateral and one frontal image.

The line in the upper left corner shows the current set number, the number of the currently displayed image and the number of all available images in this set.

Lateral, Frontal projection

• Select Lateral or Frontal and confirm with Next.

The selected projection is disabled in the next screen, to avoid transferring the same projection twice.

Skip Image • Check Skip Image to skip inadequate images, this is the default setting.

When a projection has been assigned to the last image and not enough images have been selected to create a valid set, you are prompted to verify the selections. It is necessary to go back to the pre-vious pages and re-set the projection. Remember that exactly two images are needed for a valid X-ray set.

After selecting the second projection the set is complete, and the system proceeds to the next wizard page, which is usually the Localizer Selection.

Portrait, Landscape There are two window formats for displaying the scanned images from the scanner. Depending on the user´s preference or the inserted film, the format can be changed by pressing the corresponding button. For example, the format below is landscape. Thus, press the Portrait button.

Image order Please note the order of the projections. The first step is to set the lateral projection. This is indicated in the middle of the dialog box and also mentioned in the title bar.

Never scan a lateral projection if indicated that the a frontal image should be posi-tioned and vice versa, as localization and the use of the image set during treatment planning are not possible then.

Scan • After setting up the right film on the scanner unit, press the Scan button. A pre-scan of the entire scanner area is performed.

Brightness • If necessary use the Brightness scroll bar to adjust the brightness of the scanned image. The film must be re-scanned to see the changes.

Frame If the brightness is satisfactory, adjust the yellow frame to fit the area of the final image.

• To change the size of the frame, position the mouse cursor over one of the four handles. Then move the mouse while holding down the left mouse button. The rectangle directly changes the size according to this movement.

• To position the frame over the final scan area, position the mouse inside the borders of the square (not over the handles). Press the left mouse button and properly reposition the frame.

Final scan • To scan the adjusted area, press Next.

The scanner now performs the final scan. Please wait until the scanner stops and the next wizard page is displayed to scan the second image or to select a localizer.

4.2.12 Analyze transferThe Analyze transfer is mostly used to interface functional MRI data and is similar to other transfers regarding the configuration, patient selection, transfer and storage. The Brainlab Support Engineer does the specific configuration followed by additional user training.

The Analyze patient data has to be copied to the defined Analyze folder on the computer hard-disk. Usually the location is C:/Brainlab/Analyze but in case of doubt please contact the Brainlab Support Engineer. The data source may also be located on CD-ROM, floppy disk or on the network.

An Analyze patient image series contains two data files:

• The patientName.hdr file, which contains the image header with parameters related to the image volume

• The patientName.img file, which is the image file corresponding to the header file.

The file names of the *.hdr and *.img have to be the same. If one of the correspond-ing files is missing the patient data can't be transferred.

The file names of the *.hdr and *.img files represent the patient name and should contain the real patient name. The patient name used in PatXfer is constructed from these file names, so please make sure the file names contain the real patient name.

In the patient selection dialog the patient names listed are reconstructed from the An-alyze data file names. Make sure to choose the correct patient.

The Analyze transfer is the same as the standard CT and MRI transfer except for the Orientation Check dialog box.

Due to the fact that no image orientation information (head - feet/anterior - posterior) is available in the header, the orientation has to be manually defined. This is also stated in the dialog box, which is displayed when you click Next in the Image Set Selection dialog box.

The Analyze format does not include several required image parameters. Some of the volume parameters are default values and may be incorrect. Brainlab does not rely on the Analyze format information and the user has to verify, correct and accept the im-age parameters.

The Analyze format does not support angulation. Do not use Analyze images with an-gulation.

This dialog box allows you to verify and adjust the image labeling.

The image preview in PatXfer displays the slices as required by the radiologist (RPF). The patients left side has always to be on the left side of the screen!

• Use the Rotate buttons to rotate the complete image set in the respective (anterior - posterior or head - feet) direction.

• Use Reset to restore the initial image set orientation.

Please use the slider and verify all slices to see if the sorting order is correct.

The correct patient orientation has to be selected. Please use other image modalities and films from the selected patient to verify the correct image orientation of the image data. Only in case you are sure that the image labeling left/right, anterior/posterior and head/feet is correct you may confirm the correctness and proceed with the data trans-fer.

• Please verify the orientation labels before confirming it with the Next button.

All relevant information is written to a transfer specific logfile.

4.2.13 General information on functional MRI transferTo use functional MRI (fMRI) data you need to have at least two image series and to apply a partic-ular procedure:

• First a high-resolution T1 MRI (anatomical data) series has to be transferred using the standard MRI data transfer (DICOM or any other installed MRI transfer).

• The output of the fMRI image processing (SPM, AFNI or fMRI tools from scanner manufactur-ers) is transferred adequately and has to be stored to the same patient folder as the anatomical data.

fMRI series have to contain the functional structures as gray value overlay to one of the EPI series. It is required to have enough anatomical information (field of view, number of slices) to match the fMRI data later with the high resolution T1 MRI series.

The support.dll file can’t be found in the expected directory. It should be together with the PatXfer executable. Please verify the location and move this file the proper directory.

Can't load transfer DLL: %1

A transfer DLL was found, but could not be loaded. Please check the permission level of the spec-ified directory.

The transfer libraries were moved during the execution. Please undo the operation.

Can't process: incomplete image set!

An image set operation can’t proceed due to unavailable information.

Can't retrieve DLL information: %1

Tried to read information from the DLL, but there was no response.

Can't run follow-up application: %1

An attempt to start an external application was performed, but the application did not respond. It may not be found in the specified directory.

Can't run Notepad to show the logfile: %1

A transfer module requested a not existing wizard page. This indicates a version conflict between the main program PatXfer and the transfer DLL. Please check transfer version or contact Brainlab support!

No more images available.

One frontal and one lateral projection has to be selected. Please verify your selections!

No valid exported variable in %1 found! This is no valid Brainlab transfer DLL.

A DLL was loaded, but did not respond to a secret key. This could be a foreign DLL, which is in the same directory as the transfer modules (in this case, delete the foreign DLL), or the Transfer path is wrong.

No valid sets! Either no images were found, incomplete images were read or the images did not match the se-lected platform. Please check the transfer logfile or refer to the scanning instructions!

No valid transfer DLL available.

The dongle may not enable the desired transfer modules. Please contact Brainlab support to check the dongle settings.

5.6.2 JPEG Compression ErrorsThe error messages in this section are only a note as to where the error occurred in the decompres-sion routine. If one of these messages appears, contact Brainlab support or the scanner manufac-turer, as this indicates a format error in the compression code.

- Bad DRI header!

- Bad Huffman table!

- Error in SOF header!

- Error in SOS header!

- JPEG compression error! JPEG image data is corrupt!

- No Matrix specified (redefinition of line numbers (DNL) not supported)!

- Precision must be between 2 and 16 bits!

- Sampling factor must be 1!

- This is not a lossless JPEG image!

- Too many components for interleaved scan!

- Undefined predictor level!

- Unexpected end of image (EOI Marker)!

- Unexpected Marker encountered!

5.6.3 SCSI errorsMost of the SCSI errors result from improperly installed hardware or wrong media handling. Please refer to the hardware documentation if problem arise, or call Brainlab support if the problem cannot be solved.

Below is a definition of the error codes and corresponding comments in alphabetical order:

Bad SCSI port! The transfer queries the wrong SCSI interface. The installation of the device might not be correct, i.e. it might not be recognizable by the operating system or the initialization file may need to be configured.

Bad status! The SCSI device returned a bad status. Please check if the device is being used by another appli-cation.

Blank check! The medium being used seems to be blank or is the wrong device for that medium.

Can't access medium! Medium or SCSI error! Can’t read from medium!

The transfer did not receive any data from the device. Check the medium or device. Reboot the system, once the device has been turned on.

Illegal drive letter for MOD drive!

Every standard MOD drive is recognized by the operating system as a casual hard-disk drive. The OS assigns a drive letter for each found drive, so the drive letter can be verified in the ‘Explorer’. This drive letter may be incorrectly setup in our initialization file.

Illegal request! An illegal device operation was requested.

Illegal SCSI id for 12\” optical disk drive!

The SCSI ID is incorrectly setup in the initialization file.

Illegal SCSI id for film scanner!

The SCSI ID is incorrectly setup in the initialization file.

Illegal SCSI id for tape drive!

Medium error! The transfer library could not read the medium. It may be corrupt or is for another device. If a sec-ond drive is available, try that.

An error occurred while device communication.

Check hardware connections or the inserted medium!

SCSI reservation conflict!

During initialization of the SCSI device, in this case typically a film scanner, we reserve the device exclusively for our application. It seems that another application is using the device. Please close any other applications.

SCSI target device is busy!

The device may be in use by another process.

This will also occur if, for example, a tape is changed and the transfer is started before the tape could be mounted by the device. Please wait a while and try again.

SCSI target device is not ready!

The device may be in use by another process.

This also happens if you change, for example, a tape and you start the transfer before the tape could be mounted by he device. Please wait a while and try again.

Unexpected end of medium!

The transfer library tried to access a data block after the end of medium was reached.

Unexpected file mark encountered!

The transfer library tried to access a data block after the end of the file marker was reached. (Imagine a medium, which is partitioned in many parts.)

Unit attention! The SCSI device returned a unit attention code. Try again.

Unknown tape storage format!

The expected format did not match the read data.

Unspecified SCSI problem!

The SCSI device returned an undocumented error.

Unsupported media type!

Use only media of size 512 or 1024 bytes/sector!

The expected file system of the medium is not the one specified. Check the storage mode of the scanner.

Illegal image storage mode!

An unknown storage mode is being used. Please contact Brainlab support.

Illegal patient orientation!

The patient orientation is not supported by the chosen platform. Ask the scanner operator to change the orientation to true axial, coronal or sagittal images - depending on the scanner, or if newer planning software is owned which supports oblique images, change the platform.

Insufficient memory!

Close a few applications or restart the program.

Medium contains images with illegal image modality!

An unsupported image modality was encountered. Check with the scanner operator or ask our support engineers.

No usable images found! Please check import directory settings!

The specified import directory does not contain any images. Move the images to be transferred to the import directory or change the path setting in the initialization file.

Please use only uncompressed images!

The encountered compression code is unknown. Advise the scanner operator to store the images without compression.

The selected folder does already contain the maximum number of image sets!

An attempt to add new image sets to an existing patient folder, but it exceeded the maximum num-ber of supported image sets for the specified platform. Change the platform, if own a platform sup-porting more image sets is owned, cancel some selected sets or store as a new patient.

Unknown media format!

The medium was not recognized as a valid format. Check the medium or try the other side.

Unknown file format of image file!

The file format of the image is different from the manufacturer’s documentation. Try storing the data in a different format.
