•Requirements
• Full-time Member
• Parent or Legal guardian
• Child under 13 years old
Child Care Application Process for Member
•Member application
• On-line
• Download at www.naccrra.org
• Request by phone, fax, or mail
•Required member documents
• Birth certificate for each child need care
• Court issued document of legal custody, if member is legal guardian or foster parent
Child Care Application Process for Member Con’t
• Proof of salary and wages for four most recent pay periods for each household member (includes member income is in addition to living allowance
• Official proof of other types of income (SSI, military pay, child support, alimony)
• State rules apply
• Application must be signed by site Program Manager
Child Care Application Process for Provider• Provider application
• On-line
• Download at www.naccrra.org
• Request by phone, fax, or mail
• Required provider documents• License (copy of renewal documents if
license expires within 30 days)
• Child care provider’s social security card, if unlicensed
• In California, drivers license or non-drivers ID for unlicensed provider
Child Care Payments• Members receive child are attendance sheets
• Provider completes at the end of month and fax, email, or mail to NACCRRA
• Provider and member must sign attendance sheets
• Provider paid within 10 business days after receipt of attendance sheet
• Processing delayed if attendance sheet is incomplete, inaccurate, or missing parent and provider signatures
• Provider reimbursed up to the maximum rate allowed by state child care regulations
• Payments made directly to provider
Program Difficulties
• Member applications received without Program Manager’s signature
• Hard to get in touch with Member at program site
• Member and site email capability limited
• Providers with no email, fax, or copy capability
• Provider Social Security cards unreadable
• Provider W-9’s not signed
• Attendance sheets received without signatures
