Patient Observations

Observations documentation provides details of ward, patientname, DOB, hospital number, consultant, frequency ofobservations.

Patients receive twice daily obs of temp, pulse, blood pressure,resps, level of consciousness and O2 sats, unless otherwisedocumented.

Patients are assessed against MEWS in accordance with Trustpolicy.

24 hour cumulative balances are evident on all fluid balanceobservations.

Evidence of documentation of referrals to medical staff for patientswith MEWS of 4 or more.

Observations repeated within 30 minutes when 4 or more.

this ward? Do you feel involved in decisions about your treatment and care? * Have hospital staff been available to talk about any worries or

concerns you have ? * Do you get enough help from staff to eat your meals? Whilst you have been on this ward have you ever shared a sleeping

area with a member of the opposite sex? Do you think that hospital staff do everything they can to help control

your pain? When you use the call buzzer is it answered? Have staff talked to you about your discharge from hospital?
