A variety of risk factors converge on the artery to promote atherogenesis in individuals with type 2 diabetes:Skeletal muscles may be resistant to insulin action, which decreases the utilisation of glucose and free fatty acids, causing hyperglycaemia and increased levels of circulating free fatty acids. In the presence of insulin resistance, the pancreas initially attempts to compensate by producing more insulin, leading to hyperinsulinaemia, itself a risk factor for arteriopathy. A high burden of abdominal fat presents the liver with elevated levels of free fatty acids through the portal circulation. This excess of free fatty acids will drive the overproduction of TG-rich lipoprotein particles, including VLDL. A reciprocal decrease in HDL accompanies the hypertriglyceridaemia that is characteristic of the type 2 diabetes state. In addition to the increase in fasting TGs, patients with diabetes may have an accentuated response to dietary fat, yielding an exaggerated postprandial lipaemia, indicated by the creamy supernatant over the plasma in the test tube. The adipocyte can also release pro-inflammatory cytokines such as TNF-alpha, which not only have direct effects on vascular wall cells that can promote atherogenesis, but can also elicit the production of acute-phase reactants by the liver, including CRP (an independent indicator of cardiovascular risk and of incident diabetes), increased fibrinogen (a substrate for increased thrombosis), and an increase in the inhibitor of fibrinolysis, PAI-1. The metabolic syndrome often includes hypertension, a well-known promoter of atherogenesis. Genetic factors may play into susceptibility to type 2 diabetes and atherosclerosis as well. Finally, the formation of advanced glycation end-products from glycated macromolecules, depicted by the green glucose molecules modifying the polypeptide chain, can engage the receptor for advanced glycation end-products (RAGE) and compound the inflammatory stimuli encountered by the arterial wall in patients with type 2 diabetes.

Mensaje clave: sin embargo, los frmacos inhibidores del cotransportador de sodio y glucosa tipo 2 actan a nivel renal, incrementando la glucosuria, mediante un mecanismo de accin independiente de la insulina y potencialmente complementario a las acciones del resto de grupos teraputicos para la DM2.
A pesar de que su existencia es conocida desde finales del siglo XIX, recientemente se ha incorporado al tratamiento de la DMt2 una nueva clase teraputica, los inhibidores del co-transportador sodio-glucosa del tbulo renal (inhibidores SGLT2 o gliflozinas) que, como primera caracterstica diferencial debemos subrayar que tienen un efecto anti-hiperglucemiante que es totalmente independiente de insulina, al actuar en el rin potenciando la aparicin de glucosa en orina. As mantienen su efecto sea cual sea la capacidad secretoria residual que mantenga la persona diabtica que, como ahora, va reducindose conforme avanza la historia natural de la enfermedad (Adaptado de DeFronzo RA. Los frmacos inhibidores del cotransportador SGLT-2 reducen la reabsorcin de glucosa en el tbulo proximal, incrementan la excrecin renal de glucosa, disminuyen los niveles de glucemia (sin provocar hipoglucemia) y reducen el peso corporal al provocar una prdida calrica de unas 300 Kcal/da, sin riesgo de hipoglucemia.Al ser un mecanismo independiente de la insulina, la inhibicin del SGLT-2 induce descensos de glucemia en todos los pacientes con diabetes tipo 2, con independencia del grado de evolucin de la enfermedad.

79Este grfico representa una estructura primaria (secuencia de aminocidos) y otra secundaria (puentes disulfuro) de la molcula de insulina.La insulina consta de dos cadenas: la A que contiene 21 aminocidos y la B con 30. Se trata de una protena de peso molecular bajo (unos 6 Kdalton). Las dos cadenas se encuentran unidas por dos puentes disulfuro, adems de existir otro puente intracatenario en la cadena A. La presencia en integridad de estos puentes es imprescindible para la accin biolgica de la insulina.
