MEMBERSHIP: Upon approval of my application by Saint Andrews Club Director I will be entitled to all of the privileges of said membership.

NEW MEMBER INITIATION FEE: I agree to pay Saint Andrews Club the sum of $1,000 (plus applicable taxes) as a non-refundable initiation fee. Contact Club Director for details.

PAYMENT OF DUES/GOLF PACKAGE: I agree to pay all fees and dues as defined in the membership packet. Club rules require that member’s accounts must be paid in full within 30 days of the statement date. Delinquent accounts may be posted and are subject to a 1.5% �nance charge per month (annual percentage rate of 18%).

PAYMENT OF MONTHLY CHARGES: I agree to pay all applicable monthly charges by the 15th of each month with the CREDIT CARD on file with management. Accounts 30 days past due may be subject to suspended charging privileges.

RULES AND REGULATIONS: I agree to be bound by and comply with all the rules and regulations established by Saint Andrews Club management.I agree to acquaint myself with and abide by all club/golf course rules and regulations. I have particularly been made aware that misconduct by myself, my family and/or guests may become the basis for suspension or expulsion. I also agree to give written notice of my resignation. If I resign or am terminated between January 1 and June 30 of any year, I understand that I am obligated for food and beverage minimums through June 30 of that year. If I resign or am terminated between July 1 and December 31 I understand that I am obligated for food and beverage minimums through December 31 of that year. I also understand that if I resign or am terminated after April 1 of any year I am obligated for the full year’s dues of that year. I understand that by signing this application I am entering into a legal contract and will be bound by the terms and conditions stated.

ACCEPTANCE OF APPLICATIONS: I understand that this application for membership is subject to approval by Saint Andrews Club management. The Club management has the right at its sole discretion to accept or reject this application. This application will become fully binding upon approval.

NEW MEMBER CHARGE REQUIREMENT: I agree to place my credit card on file for the purpose of paying all monthly charges.
