2015-2016 STAAR 2ND QUARTER WINNERS FROM THE NURSE

The Phoenix Children's Hospital Breathmobile is a self contained

mobile asthma clinic, providing asthma identification, teaching,

treatment and follow up. They will be at Salk Elementary

school Feb. 23rd and March 23rd. Please call Susan Barnes

R.N. for a referral form and call 602-933-0348 for an

appointment.

On 1/11/16, the Fremont Self-Contained Autism Class attended

a Field Trip to the Textron Aviation Center located at Phoenix/

Gateway Airport. The students and teachers appreciate this

local company for giving us their time and allowing us to tour

their facility. THANK-YOU!!

It’s Parent Quality Service Survey time! Per state law, schools must survey all parents of their students annually. This survey

allows the parents to rate the quality of service received by their student’s school. The survey questions are the same as last

year. Parents are allowed the option of completing one survey per child or one survey per school, with instructions provided on

the survey instrument itself. Please remember to use a #2 pencil and do not use any staples on the survey. Surveys that have

been stapled cannot be scanned. All surveys are due back to Fremont Jr. High or Research & Evaluation by Friday, February 12,

2016

There are different ways to return the completed survey:

1.) Return survey to your student’s teacher.

2.) Bring survey to the drop-box located in the school office.

3.) Drop off or Mail to: Research and Evaluation 63 E. Main St. #101 Mesa, AZ 85201-7422
