    PERTAMBANGAN MINYAK DAN GAS BUMI DAN PENGUSAHAAN SUMBERDAYA

    Memimbang : bahwa dalam rangka kelancaran pelaksanaan pemeriksaaan keselamatan kerja atas instalasi, peralatan dan teknik yang dipergunakan dalam usaha pertambangan minyak dan gas bumi dan pengusahaan sumberdaya panasbumi dan sebagai pelaksanaan lebih lanjut Peraturan Menteri Pertambangan dan Energi Nomor 06 P/ 0746/ M.PE/1991 tanggal 19 Nopember 1991, dianggap perlu menerapkan Pedoman dan Tatacara Pemeriksaan Keselamatan Kerja atas Instalasi, Peralatan dan Teknik Yang Dipergunakan Dalam Usaha Pertambangan Minyak dan Gas Bumi Dan Pengusahaan Sumberdaya Panasbumi dalam suatu Keputusan Direktur Jenderal Minyak dan Gas Bumi ; Mengingat : 1.     REGARDING

    INSPECTION GUIDELINES AND PROCEDURES FOR OCCUPATIONAL SAFETY

    OF THE PLANT, EQUIPMENT AND ENGINEERING EMPLOYED IN THE MINING OPERATIONS OF OIL AND GAS AS WELL AS

    EXPLOITATION OF GEOTHERMAL RESOURCES

    THE DIRECTOR GENERAL OF

    Considering : that in the framework of smooth implementation of the inspection for occupational safety of the plant, equipment and engineering employed in the mining operations of Oil and Gas as well as exploitation of geothermal resources and as further implementation of the Regulation of the Minister of Mines and Energy Number 06 P/ 0746/ M.PE/1991 dated 19th November 1991, it is deemed necessary to stipulate Inspection Guidelines and Procedures for Occupational Safety of the Plant, Equipment and Engineering employed in the Mining Operations of Oil and Gas as well as Exploitation of Geothermal Resources in a Decree of the Director General of Oil and Gas;

    MEMUTUSKAN :

    Menetapkan : KEPUTUSAN DIREKTUR JENDERAL MINYAK DAN GAS BUMI TENTANG PEDOMAN DAN TATACARA PEMERIKSAAN KESELAMATAN KERJA ATAS INSTALASI, PERALATAN DAN TEKNIK YANG DIPERGUNAKAN DALAM USAHA PERTAMBANGAN MINYAK DAN GAS BUMI DAN PENGUSAHAAN SUMBERDAYA PANASBUMI.

    Pasal 1 (1) Segala definisi atau pengertian yang dipergunakan

    dalam Keputusan ini adalah definisi atau pengertian sebagaimana dimaksud dalam Pasal 1 Keputusan Menteri Pertambangan dan Energi Nomor 06P/0746/M.PE/1991 tanggal 19 Nopembner 1991

    (2) Selain sebagaimana telah ditetapkan pada ayat (1),

    dalam Keputusan ini yang dimaksud dengan :

    DECIDED :

    To draw up : DECREE OF THE DIRECTOR GENERAL OF OIL AND GAS REGARDING INSPECTION GUIDELINES AND PROCEDURES FOR OCCUPATIONAL SAFETY OF THE PLANT, EQUIPMENT AND ENGINEERING EMPLOYED IN THE MINING OPERATIONS OF OIL AND GAS AS WELL AS EXPLOITATION OF GEOTHERMAL RESOURCES.

    Article 1

    (1) Any definitions or terminologies used in this Decree shall be definitions or terminologies as referred to in Article 1 of the Decree of the Minister of Mines and Energy Number 6P/0746/M.PE/1991 dated 19th November 1991.

    (2) In addition to those as already provided in

    paragraph (1), in this Decree, meant by :

Sertifikat Kelayakan Penggunaan, adalah persetujuan yang diberikan Direktur Jenderal atas operasi atau penggunaan Instalasi dan Peralatan setelah dilakukan pemeriksaan Keselamatan Kerja, yang berupa Sertifikat Kelayakan Penggunaan Instalasi (SKPI) dan Sertifikat Kelayakan Penggunaan Peralatan (SKPP);

    b. Direktorat Jenderal, adalah Direktorat Jenderal Minyak dan Gas Bumi;

    c. Direktur, adalah Direktur Teknik Pertambangan Minyak dan Gas Bumi;

    d. Direktorat Teknik, adalah Direktorat Teknik Pertambangan Minyak dan Gas Bumi;

    e. Kepala Pelaksana Inspeksi Tambang, adalah Kepala Inspeksi Tambang Minyak dan Gas Bumi;

    f. Pelaksana Inspeksi Tambang, adalah pejabat Direktorat Jenderal yang diangkat oleh Direktur Jenderal

    g. Perusahaan, adalah Pertamina, Kontraktor Kontrak Production Sharing, Technical Assistance, Joint Operating Body, dan mitra kerja Pertamina lainnya dalam operasi Minyak dan Gas Bumi dan Kontraktor Kontrak Operasi Bersama Pengusahaan Sumberdaya Panasbumi;

    h. Perusahaan Jasa adalah Perusahaan Jasa Inspeksi Teknik dalam bidang pertambangan minyak dan gas bumi dan pengusahaan sumberdaya panasbumi yang telah mendapatkan penunjukan Direktur Jenderal.

    Pasal 2 (1) Terhadap Instalasi dan Peralatan dalam operasi

    pertambangan minyak dan gas bumi wajib dilaksanakan pemeriksaan Keselamatan Kerja.

    (2) Pemeriksaan Keselamatan Kerja sebagaimana dimaksud pada ayat (1) dilakukan terhadap Instalasi dan Peralatan yang : a. akan dipasang atau didirikan; b. sedang dipasang atau didirikan; c. telah dipasang atau didirikan.

    a. Worthiness Operation Certificate shall be approval given by the Director General of the operation or use of the Plant and Equipment after being carried out an Inspection for Occupational Safety in the form of a Worthiness of Plant Installation Certificate (SKPI) and Worthiness of Equipment Operation Certificate (SKPP);

    b. Directorate General shall be the Directorate

    General of Oil and Gas; c. Director shall be the Director of Oil and Gas

    Mining Engineering;

    d. Engineering Directorate shall be the Directorate of Oil and Gas Mining Engineering;

    e. Mine Head Inspector shall be the Oil and Gas

    Mine Inspection Head; f. Mine Inspector shall be an officer of the

    Directorate General appointed by the Director General;

    g. Company shall be Pertamina, Production

    Sharing Contractor. Technical Assistance, Joint Operating Body and another work partner of Pertamina in the Oil and Gas operations and Joint Operation Contract Contractor in the Exploitation of Geothermal Resources;

    h. Service Company shall be an Technical

    Inspection Service Company in the fields of Oil and Gas mining and exploitation of geothermal resources already having an appointment from the Director General.

    Article 2

    (1) Of the Plant and Equipment in the mining operations of Oil and Gas, shall be carried out an inspection for Occupational Safety.

    (2) The Inspection for Occupational Safety as referred

    to in paragraph (1) shall be carried out of the Plant and Equipment which : a. will be installed or erected; b. is being installed or erected; c. has been installed or erected.

  • (3) Terhadap peralatan yang dibuat berdasarkan

    pesanan dan bukan merupakan produksi masal, pemeriksaan Keselamatan Kerja dapat dilakukan ditempat pembuatan peralatan.

    Pasal 3 Instalasi dan Peralatan yang wajib dilaksanakan pemeriksaan Keselamatan Kerja adalah sebagaimana termasksud dalam Lampiran I Keputusan ini.

    Pasal 4 (1) Pemeriksaan Keselamatan Kerja atas Instalasi dan

    Peralatan sebagaiman dimaksud dalam Pasal 2 dilaksanakan oleh Pelaksana Inspeksi Tambang dan atau oleh Perusahaan Jasa.

    (2) Tatacara Pemeriksaan Keselamatan Kerja atas Instalasi dan Peralatan adalah sebagaimana termaksud dalam Lampiran II Keputusan ini.

    Pasal 5 (1) Terhadap Instalasi dan Peralatan yang telah

    dilaksanakan pemeriksaan Keselamatan Kerja diberikan Sertifikat Kelayakan Penggunaan Instalasi (SKPI) dan Sertifikat Kelayakan Penggunaan Peralatan (SKPP) oleh Direktur Jenderal

    (2) Sertifikat Kelayakan Penggunaan Instalasi (SKPI) dan Sertifikat Kelayakan Penggunaan Peralatan (SKPP) sebagaimana dimaksud pada ayat (1) berlaku selama jangka waktu 5 (lima) tahun untuk SKPI dan 3 (tiga) tahun untuk SKPP, atau kurang dari jangka waktu tersebut di atas apabila Instalasi dan Peralatan tersebut mengalami perubahan atau diragukan kemampuannya.

    (3) Setalah selesainya jangka waktu sebagaimana dimaksud pada ayat (2), terhadap Instalasi dan Peralatan wajib dilakukan pemeriksaan Keselamatan Kerja sebagaimana dimaksud dalam Pasal 2.

    Pasal 6

    Setelah dilaksanakan evaluasi terhadap teknik yang akan dipergunakan sebagaimana termaksud dalam Lampiran I huruf C, Direktur Jenderal Cq. Direktur memberikan pengesahan.

    (3) Of the equipment manufactured based on an order

    and not mass production, the inspection for Occupational Safety can be carried out in the place where the equipment is manufactured.

    Article 3 The Plant and Equipment of which the inspection for Occupational Safety shall be carried out shall be as referred to in Attachment 1 to this Decree.

    Article 4

    (1) The Inspection for Occupational Safety of the Plant and Equipment as referred to in Article 2 shall be carried out by the Mine Inspector and / or by the Service Company.

    (2) The Inspection procedures for Occupational Safety

    of the Plant and Equipment shall be as referred to in Attachment II to this Decree.

    Article 5

    (1) For the Plant and Equipment of which the

    inspection for Occupational Safety has been carried out shall be issued a Worthiness of Plant Installation Certificate (SKPI) and Worthiness of Equipment Operation Certificate (SKPP) by the Director General.

    (2) The Worthiness of Plant Installation Certificate

    (SKPI) and Worthiness of Equipment Operation Certificate (SKPP) as referred to in paragraph (1) shall be valid for a period of 5 (five) years for SKPI and 3 (three) years for SKPP, or less than the periods as mentioned above if the Plant and Equipment has a modification or its ability is doubted.

    (3) After the periods as referred to in paragraph (2)

    have ended, of the Plant and Equipment shall be carried out an inspection for Occupational Safety as referred to in Article 2.

    Article 6

    After a technical evaluation which will be employed as referred to in Attachment I letter C has been carried out, the Director General Cq. Director shall issue ratification.

  • Pasal 7

    Besarnya biaya pemeriksaan Keselamatan Kerja atas Instalasi dan Peralatan yang dilaksanakan oleh Perusahaan Jasa ditetapkan berdasarkan mekanisme pasar tenaga kerja bidang minyak dan gas bumi.

    Pasal 8 Dalam jangka waktu 6 (enam) bulan sejak berlakunya Keputusan ini, Perusaan yang telah beroperasi atau telah menggunakan Instalasi dan Peralatan dan belum melakukan ketentuan dan Keputusan ini wajib mengajukan permohonan pemeriksaan Keselamatan Kerja atas Instalasi, Peralatan dan Teknik yang dipergunakan kepada Direktorat Jenderal cq. Direktorat Teknik.

    Pasal 9 Apabila terdapat kekurangan-kekurangan pada Instalasi atau Peralatan setelah diadakan pemeriksaan Keselamatan Kerja, maka dalam waktu yang ditetapkan oleh Direktur, Perusahaan wajib mengadakan perbaikan atau perubahan sehingga Instalasi dan Peralatan tersebut memenuhi hal-hal yang disyaratkan oleh peraturan perundang-undangan yang berlaku.

    Pasal 10 Keputusan ini berlaku pada tanggal ditetapkan.

    Article 7

    The amount of the inspection cost for Occupational Safety of the Plant and Equipment carried out by the Service Company shall be fixed based on the mechanism of the labour (manpower) market in the fields of petroleum and Gas.

    Article 8

    In a period of 6 (six) months since the effectiveness of this Decree, the Company which has been operating or employing the Plant and Equipment and has not yet implemented the provisions of this Decree shall be under an obligation to submit an application for an inspection for Occupational Safety of the Plant, Equipment and Engineering employed to the Directorate General cq. Engineering Directorate.

    Article 9

    In case of shortcomings in the Plant or Equipment after being carried out an inspection for Occupational Safety, then in a period as stipulated by the Director, the Company shall be under an obligation to make repairs or modifications so that the Plant and Equipment will satisfy the matters required by the prevailing legislative regulations.

    Article 10 This Decree shall come into effect as of its stipulation date.

    1. Instalasi Ekplorasi dan Eksploitasi : a. Instalasi pemboran; b. Instalasi produksi; c. Instalasi pengumpulan d. Instalasi lainnya yang terkait dengan

    kegiatan Eskplorasi dan Eksploitasi

    2. Instalasi Pemurnian dan Pengolahan : a. Instalasi Pemurnian dan Pengolahan; b. Pembongkaran dan Pemuatan; c. Instalasi lainnya yang terkait dengan

    kegiatan Pemurnian dan Pengolahan baik langsung maupun tidak langsung berhubungan dengan kegiatan termaksud.

    3. Instalasi Penimbunan dan Pemasaran yang

    dimaksud adalah : a. Instalasi Seafed Depot; b. Instalasi Inland Depot; c. Instalasi Depot Pengisian Pesawat Udara

    (DPPU); d. Instalasi Transit Terminal; e. Instalasi Stasiun Pengisian Bahan Bakar

    Umum (SPBU) dan Stasiun Pengisian Bahan Bakar Gas (SPBG);

    f. Instalasi lainnya yang terkait dengan kegiatan penimbunan dan pemasaran

    B. Peralatan

    1. Katup Pengaman yaitu peralatan yang berguna untuk melindungi peralatan dan fasilitas yang terkait meliputi :

    a. Safety Vale; b. Relief Valve; c. Safety Relief Valve; d. Thermal Relief Valve; e. Pilot Operated Safety Valve; f. Vacuum Relief Valve.

    PLANT AND EQUIPMENT SUBJECT TO THE INSPECTION FOR OCCUPATIONAL SAFETY

    A. Plant 1. Exploration and Exploitation Plant : a. Drilling plant; b. Production plant; c. Gathering plant

    d. Other plant related to the Exploration and Exploitation activities

    2. Refinement and Treatment Plant : a. Refinement and Treatment Plant; b. Unloading and Loading; c. Other plant related to the Refinement and

    Treatment activities, either directly or indirectly related to the activities in question.

    3. Storage and Marketing Plant, that is :

    a. Seafed Depot Plant; b. Inland Depot Plant; c. Aircraft Fuelling Depot (DPPU) Plant; d. Transit Terminal Plant; e. Public Fuelling Station (SPBU) and Gas

    Fuelling Station (SPBG) Plant; f. Other plant related to the storage and

    marketing activities

    B. Equipment

    1. Safety Valves, that is equipment useful to protect the related equipment and facilities comprising :

    a. Safety Valve; b. Relief Valve; c. Safety Relief Valve; d. Thermal Relief Valve; e. Pilot Operated Safety Valve; f. Vacuum Relief Valve.

  • 2. Bejana Tekan dan sejenisnya yaitu peralatan

    yang bekerja dengan tekanan kerja didalam peralatan melebihi Atm tekanan lebih (gauge), atau bejana vakum dengan tekanan kerja didalam peralatan kurang dari 1 Atm absolut.

    3. Pesawat Angkat yaitu peralatan untuk

    memindahkan, mengangkat barang secara vertikal dan atau horizontal dalam jarak, antara lain meliputi : a. Pesawat Angkat Bergerak; b. Pesawat Angkat Tetap; c. Pesawat Angkat di atas kepala.

    4. Peralatan Listrik yaitu peralatan yang

    membangkit, mendistribusi dan mengendalikan sistem tenaga listrik meliputi : a. Unit Power Generator; b. Unit Power Transformer; c. Unit Switcher; d. Unit Motor Control Center.

    5. Peralatan Putar yaitu peralatan yang berfungsi

    memindahkan atau memampatkan minyak, gas serta panasbumi meliputi : a. Unit Kompresor; b. Unit Pompa.

    6. Pipa Penyalur yaitu bentang pipa berikut

    fasilitas-fasilitas terkait yang digunakan untuk mengalirkan dan menyalurkan minyak dan gas bumi serta panasbumi.

    C. Teknik Yang Dipergunakan

    Teknik yang dipergunakan yaitu tata cara atau prosedur yang akan dipergunakan dalam operasi pertambangan minyak dan gas bumi antara lain meliputi : 1. Spesifikasi Prosedur Las (WPS) dan Rekaman

    Kualifikasi Prosedur (PQR); 2. Catatan Unjuk Kerja (Sertifikat) Juru Las /

    Operator Las; 3. Prosedur Uji Tekan Pipa Penyalur; 4. Prosedur Uji Beban Pesawat Angkat; 5. Prosedur Reparasi, Modifikasi dan Alterasi.

    2. Pressure Vessel and the like, that is equipment

    working with a work pressure within the equipment of more than more pressure Atm (gauge), or vacuum vessel with a work pressure within the equipment of less than 1 absolute Atm.

    3. Lifting Equipment / Crane, that is equipment

    to remove, lift up goods vertically and / or horizontally at a distance, among others including : a. Mobile Cranes; b. Stationary Cranes; c. Overhead Cranes.

    4. Electrical equipment, that is equipment

    generating, distributing and controlling the electrical power system comprising :

    a. Power Generator Unit; b. Power Transformer Unit;

    c. Switcher Unit; d. Motor Control Center Unit.

    5. Rotary Equipment, that is equipment

    functioning to remove or compress petroleum, Gas and geothermal comprising :

    a. Compressor Unit; b. Pump Unit.

    6. Pipeline, that is a range of pipe together with

    related facilities used to transmit and distribute Oil and Gas as well as geothermal.

    C. Engineering Employed

    The engineering employed, that is procedures which will be employed in the mining operations of Oil and Gas, among others includes : 1. Welding Procedures Specifications (WPS) and

    Procedure Qualification Record (PQR); 2. Performance Record (Certificate) of the

    Welder / Welding Operator; 3. Pressure Test Procedures for Pipeline; 4. Load Test Procedures; 5. Procedures for Repairs, Modification and

    Alteration.

    meliputi penilaian perencanaan dengan melaksanakan penelaahan terhadap data :

    limbah; b. Alat ukur dan metering; c. Sistem perpipaan; d. Sistem intrumentasi; e. Piranti pengaman; f. Tangki timbun; g. Sistem pencegahan dan penanggulangan

    kebakaran; h. Sistem pencegahan dan pemantauan

    pencemaran lingkungan; i. Peralatan lain; j. Diagram alir proses dan diagram pipa &

    instrumen; k. Gambar tata letak peralatan; l. Pekerjaan sipil; m. Klasifikasi daerah berbahaya (hazardous area

    classification); n. Electrical one line diagram; o. Gambar tata letak pentanahan (grounding lay-

    out); p. Diagram cause & effect / SAFE Chart; q. Prosedur kerja pengoperasian peralatan; r. Prosedur evakuasi darurat; s. Surat persetujuan atau rekomendasi dari

    instansi yang berwenang untuk program Pengolahan dan Pemantauan Lindungan Lingkungan.

    B. Pemeriksaan fisik.

    Pemeriksaan fisik meliputi : 1. Pemeriksaan Instalasi untuk memastikan

    sesuai tidaknya Instalasi dengan perencanaan, spesifikasi, prosedur pembuatan dan pemasangan.

    INSPECTION PROCEDURES FOR OCCUPATIONAL SAFETY OF THE PLANT

    AND EQUIPMENT

    1. INSPECTION FOR OCCUPATIONAL SAFETY OF THE PLANT

    A. The Inspection for Occupational Safety of the

    Plant includes an evaluation of the designs by reviewing of the data on :

    a. Process, utilities and waste treatment

    equipment; b. Measuring and metering devices; c. Piping system; d. Instrumentation system; e. Safety appliances; f. Storage tank; g. Fire prevention and control system; h. Environmental pollution monitoring and

    prevention system; i. Other equipment; j. Process flow diagram and pipe & instrument

    diagram; k. Equipment Layout drawing; l. Civil works; m. Hazardous area classification; n. Electrical one line diagram; o. Grounding layout; p. Cause & effect diagram / SAFE Chart; q. Operation Procedures of the equipment; r. Emergency evacuation procedures; s. Approval letter or recommendation from the

    competent authorities for the Environmental Protection Monitoring and Processing program.

    B. Physical Inspection The physical inspection includes :

    1. Inspection of the Plant to ensure whether or not the Plant complies with the designs, specifications, manufacturing and installation procedures.

  • 2. Memeriksa Kelengkapan Mutu Peralatan, Teknik Keselamatan Kerja dan Lindungan Lingkungan meliputi :

    a. Alat Pemadam kebakaran; b. Alat deteksi api, panas, asap dan gas

    berbahaya; c. Alat perlengkapan penyelamatan dan

    pelindung perorangan; d. Sistem komunikasi; e. Sistem kontrol dan penghentian darurat

    (emergency shutdown system); f. Pemeriksaan tangga, lampu, bordes dan

    handrail; g. Flaring; h. Rock muffler i. Peralatan pencegahan dan penanggulangan

    pencemaran lingkungan; j. Sistem untuk meniadakan bahaya listrik

    statis dan arus listrik lainnya; k. Tanda-tanda keselamatan kerja, antara

    lain; tanda peringatan, larangan, kode merah dan tanda lainnya.

    3. Memeriksa Kelengkapan Sertifikat Kelayakan

    Penggunaan Peralatan, Sertifikat Kelayakan Konstruksi Platform serta Ijin Penggunaan dari Instansi teknis terkait antara lain :

    a. Katup pengaman; b. Bejana Tekan dan peralatan lain

    sejenisnya; c. Peralatan Berputar (unit pompa dan unit

    kompresor); d. Peralatan Listrik; e. Pesawat Angkat f. Pipa Penyalur g. Struktur Anjungan Lepas Pantai; h. Ketel Uap, Bejana Uap, Alat bantu

    Navigasi, Alat Ukur & Metering, Alat Timbang, Helideck, Zat Radio Aktif, Bahan Peledak, dan lain-lain;

    i. Peralatan penyelamatan dan perlindungan perorangan

    4. Khusus Instalasi pemboran wajib dilakukan

    pemeriksaan berkala sesuai dengan Pedoman Pelaksanaan Operasi Pemboran Darat dan Lepas Pantai Yang Aman di Indonesia (KK-01-DMJ).

    5. Pemeriksaan PreCommissioning,

    Commissioning dan Pengujian.

    2. Inspection for the completeness of Quality of the Equipment, Occupational Safety Techniques and Environmental Protection including :

    a. Fire Fighting Equipment; b. Fire, heat, smoke and hazardous gas

    detectors; c. Personal protection and safety equipment; d. Communication system; e. Emergency shutdown and control system; f. Inspection of stairs, lights, flight of steps

    and handrails; g. Flaring;

    h. Rock muffler; i. Environmental pollution prevention and

    control equipment; j. System to eliminate the hazards of static

    electricity and other electric currents; k. Signs of occupational safety i.e : sign of

    warning, prohibition, red code and other signs.

    3. Inspection for the Completeness of the Worthiness of Equipment Operation Certificate, Worthiness of Platform Construction Certificate as well as Operation License from the related technical agencies, i.e : a. Safety Valve; b. Pressure Vessel and other similar

    equipment; c. Rotary Equipment (pump unit and

    compressor unit); d. Electrical equipment; e. Lifting Equipment; f. Pipeline; g. Offshore Platform Structure; h. Steam Boiler, Steam Vessel, Navigation

    Aids, Measuring & Metering Devices, Weighing Equipment, Helideck, Radioactive Substance, Explosives, etc;

    i. Individual protection and safety equipment.

    4. The drilling plant shall specially be subject to a

    periodic inspection pursuant to the Implementation Guidelines of Safe Onshore and Offshore Drilling Operations in Indonesia (KK-01-DMJ).

    5. Pre-Commissioning, Commissioning and

    Testing.

  • C. Pelaporan Menyusun seluruh hasil pemeriksaan teknis berbentuk dokumen untuk diserahkan ke Direktur Direktorat Teknik Pertambangan Minyak dan Gas Bumi sebagai bahan evaluasi untuk penerbitan Sertifikat Kelayakan Penggunaan Instalasi.

    II. PEMERIKSAAN KESELAMATAN KERJA

    ATAS PERALATAN : A. PEMERIKSAAN KESELAMATAN KERJA

    KATUP PENGAMAN

    Pemeriksaan teknis katup pengaman meliputi :

    a. Penelaahan Dokumen - P & ID (Piping & Instrument Diagram) - Dokumen katup pengaman - Plat nama - Tipe katup pengaman :

    * Konvensional * Pilot * Balance

    - Sertifikat kelayakan Penggunaan test bench;

    - Rekaman Kalibrasi pressure gauge / indicator;

    - Perhitungan rekanan uji cold diff untuk menentukan tekanan buka nyata.

    b. Pelaksanaan Pengujian - Uji tekanan buka - Uji bocor

    c. Rekaman Hasil Uji

    Rekaman hasil uji diisi berdasarkan dokumen katup pengaman, data plat nama serta data pengujian sebagai bahan evaluasi penerbitan Sertifikat Kelayakan Penggunaan Peralatan.

    d. Penyegelan Penyegelan katup pengaman dilakukan oleh Pelaksana Inspeksi Tambang pada pengatur katup pengaman yang telah diuji dengan hasil baik dengan menggunakan Segel Migas.

    C. Reporting

    To prepare all the technical inspection results in the form of documents to be submitted to the Director of the Directorate of Petroleum and Gas Mining Engineering as evaluation material for the issue of the Worthiness of Plant Operation Certificate.

    II. INSPECTION FOR OCCUPATIONAL

    SAFETY OF THE EQUIPMENT : A. INSPECTION FOR OCCUPATIONAL

    SAFETY OF THE SAFETY VALVE The technical inspection of the safety valve includes : a. Review of Documents

    - P & ID (Piping & Instrument Diagram) - Safety valve document - Name plate - Types of safety valve : * Conventional * Pilot * Balance - Worthiness Certificate of Test Bench; - Indicator / pressure gauge calibration

    record; - Cold diff test record calculation to stipulate

    the real open pressure.

    b. Test Implementation - Open pressure test - Leak test

    c. Test Result Record

    The test result record (form) should be completed based on the documents of safety valve, name plate data as well as test data as evaluation material of the issue of the Worthiness of Equipment Operation Certificate.

    d. Sealing The safety valve sealing should be carried out by the Mine Inspector on the safety valve regulator already tested with good results by using the Migas (Directorate General of Oil and Gas) Seal.

  • B. PEMERIKSAAN KESELAMATAN KERJA BEJANA TEKAN DAN SEJENISNYA Pemeriksaan Teknis bejana tekan dan sejenisnya dibedakan menjadi 3 (tiga) bagian :

    1. Bejana tekan dan sejenisnya yang baru

    dan atau akan dipasang. 2. Bejana tekan dan sejenisnya yang lama

    dan atau telah dipasang. 3. Bejana tekan dan sejenisnya yang

    mengalami reparasi / alterasi / modifikasi..

    B.1. Pemeriksaan teknis bejana dan sejenisnya

    yang baru dan atau akan dipasang.

    a. Penelaahan data yang meliputi : - Gambar konstruksi dan perhitungan

    bejana tekan; - Spesifikasi material yang digunakan; - Spesifikasi prosedur las dan rekaman

    kualifikasi prosedur serta catatan kualifikasi (sertifikat) unjuk kerja juru / operator las;

    - Prosedur uji tidak merusak dan

    kualifikasi personilnya; - Prosedur reparasi - Prosedur perlakuan panas pasca las (bila

    disyaratkan); - Prosedur uji tekan;

    b. Pemeriksaan fisik yang meliputi :

    - Verifikasi sistem pengendalian mutu pemanufaktur;

    - Identifikasi material yang akan digunakan;

    - Verifikasi pembentukan bejana; - Verifikasi pemasangan sambungan; - Verfifikasi persiapan pengelasan; - Pemeriksaan hasil las serta verifikasi

    hasil uji tanpa merusak; - Pengukuran dimensi bejana dan

    pemeriksaan internal; - Uji bocor dan uji tekan; - Verifikasi formulir pemanufaktur dan

    plat nama nejana.

    B. INSPECTION FOR OCCUPATIONAL SAFETY OF THE PRESSURE VESSEL AND THE LIKE

    The Technical Inspection of the pressure vessel and the like is distinguished into 3 (three) parts :

    1. Pressure vessel and the like which are new

    and / or will be installed. 2. Pressure vessel and the like which are

    existing and / or have been installed. 3. Pressure vessel and the like which have

    been repaired / altered / modified

    B.1. Technical inspection of the pressure vessel and the like which are new and / or will be installed.

    a. Review of data comprising :

    - Pressure vessel calculation and construction drawing;

    - Specifications of the materials which will be used;

    - Specifications on welding procedures and procedure qualification record (form) as well as performance qualification record (certificate) of the welder / welding operator;

    - Procedures for non destruction test and qualifications of its personnel;

    - Procedures for repairs; - Post weld heat treatment procedures (if

    required); - Pressure test procedures.

    b. Physical inspection comprising :

    - Verification of the manufacturers quality control system;

    - Identification of the materials which will be used;

    - Vessel forming verification; - Joint installation verification; - Welding preparation verification; - Inspection of the welding results as

    well as verification of the non destruction test results;

    - Vessels dimension measurement and internal inspection;

    - Leak test and pressure test; - Verification of the manufacturers form

    and vessels name plate.

  • c. Uji tekan - Pemeriksaan rekaman kalibrasi alat

    pengukur dan alat pencatat tekanan, termasuk daerah jangkau yang diijinkan untuk tekanan uji yang bersangkutan, dimana skala penunjukan maksimum peralatan termaksud 1,5 4 kali tekanan uji.

    - Uji tekan dilaksanakan dengan menaikkan tekanan secara bertahap dan dicatat dengan alat pencatat tekanan sampai tekanan uji yang telah ditentukan kemudian tekanan ditahan minimum 2 (dua) jam dan setelah itu tekanan dikeluarkan secara bertahap.

    d. Pelaporan Menyusun seluruh hasil pemeriksaan teknis berbentuk dokumen untuk diserahkan ke Direktur Direktorat Teknik Pertambangan Minyak dan Gas Bumi sebagai bahan evaluasi untuk penerbitan Sertifikat Penggunaan Peralatan.

    B.2. Pemeriksaan teknis bejana tekan dan sejenisnya yang lama dan atau telah dipasang.

    a. Penelaahan data yang meliputi :

    - Sertifikat kelayakan penggunaan peralatan yang akan diperpanjang masa berlakunya;

    - Catatan riwayat penggunaan bejana; - Gambar bangunan bejana (as built

    drawing) - Salinan plat nama atau marka keras

    lainnya; - Prosedur kerja aman melakukan

    pemeriksaan

    b. Pemeriksaan fisik meliputui : - Pemeriksaan visual kondisi bejana; - Verifikasi hasil pengukuran terakhir

    tebal bejana yang dilakukan oleh Perusahaan;

    - Melakukan pengukuran tebal bejana; - Uji tidak merusak (bila diperlukan); - Verifikasi hasil perhitungn sisa umur

    bejana.

    c. Pressure test - Calibration record inspection of the

    pressure gauge and pressure recorder including the allowed range area for the relevant pressure test where the maximum reading scale of the relevant equipment is 1.5 - 4 times test pressure.

    - The pressure test is carried out by increasing the pressure step by step and is recorded by the pressure recording device until reaching the test pressure already stipulated, and then the pressure is retained for minimum 2 (two) hours and after that, the pressure is released out step by step

    d. Reporting

    To prepare all the technical inspection results in the form of documents to be submitted to the Director of the Directorate of Petroleum and Gas Mining Engineering as evaluation material for the issue of the Worthiness of Equipment Operation Certificate.

    B.2. Technical inspection of the pressure vessel and the like which are existing and / or have been installed. a. Review of data comprising :

    - Worthiness of equipment operation certificate of which the validity period will be extended;

    - History record of the vessel operation;

    - As built drawing of the vessel; - Copy of the name plate or other hard

    marks; - Safe work procedures to carry out

    the inspection.

    b. Physical inspection comprising : - Visual inspection of the vessels

    condition; - Verification of the last measurement

    results of the vessels thickness carried out by the Company;

    - To measure the vessels thickness; - Non-destruction Test (if required); - Verification of the calculation results

    of the remaining vessels life.

    B.3. Pemeriksaan teknis bejana tekan dan

    sejenisnya yang mengalami reparasi / alterasi / modifikasi.

    a. Penelaahan data yang meliputi :

    - Sertifikasi Kelayakan Penggunaan Peralatan yang akan diterbitkan dan atau diperpanjang masa berlakunya;

    - Catatan riwayat penggunaan bejana - Gambar bangunan dan konstruksi

    bejana; - Salinan plat nama atau marka keras

    lainnya; - Prosedur kerja aman melakukan

    pemeriksaan; - Prosedur reparasi / alterasi / modifikasi; - perhitungan reparasi / alterasi /

    modifikasi; - Spesifikasi material bahan pengganti; - Spesifikasi prosedur las dan rekaman

    kualifikasi prosedur serta catatan kualifikasi (sertifikat) unjuk kerja juru / operator las;

    - Prosedur uji tidak merusak dan

    kualifikasi personilnya; - Prosedur perlakuan panas pasca las (bila

    dsyaratkan).

    b. Pemeriksaan fisik meliputui : - Verifikasi sistem pengendalian mutu

    pemanufaktir; - Identifikasi material yang akan

    digunakan; - Verifikasi pembentukan bejana; - Verifikasi pemasangan sambungan; - Verifikasi persiapan pengelasan; - Pemeriksaan hasil las serta verifikasi uji

    tanpa merusak; - Pengukuran dimensi bejana; - Uji bocor dan uji tekan (bila diperlukan)

    c. Reporting To prepare all the technical inspection results in the form of documents to be submitted to the Director of the Directorate of Oil and Gas Mining Engineering as evaluation material for the issue of the Worthiness of Equipment Operation Certificate.

    B.3. Technical inspection of the pressure vessel and the like which have been repaired / altered / modified.

    a. Analysis of data comprising :

    - Worthiness of equipment operation certificate which will be issued, or of which the validity period will be extended;

    - History record of the vessel operation; - As built drawing and construction of

    the vessel; - Copy of the name plate or other hard

    marks; - Safe work procedures to carry out the

    inspection. - Procedures for repaird / alteration /

    modification; - Calculation of repairs / alteration /

    modification; - Specifications of the substitute

    materials; - Specifications on welding procedures

    and procedure qualification record as well as performance qualification record (certificate) of the welder / welding operator;

    - Procedures for non destructive test and qualifications of its personnel;

    - Post weld heat treatment procedures (if required).

    b. Physical inspection comprising :

    - Verification of the manufacturers quality control system;

    - Identification of the materials which will be used;

    C. PEMERIKSAAN KESELAMATAN KERJA

    PESAWAT ANGKAT

    Pemeriksaan teknis pesawat angkat meliputi :

    a. Penelaahan Dokumen Spesifikasi perencanaan dan tabel beban Perhitungan perencanaan Gambar perencanaan Material yang digunakan Spesifikasi sambungan Sumber tenaga yang digunakan Petunjuk operasi (Operasi Manual)

    b. Pemeriksaan fisik Pemeriksaan sambungan-sambungan Pemeriksaan kait pemegang beban Pemeriksaan bagian yang bergerak serta

    berputar Pemeriksaan sistem hidrolik / pneumatik Pemeriksaan kondisi tali baja (wire rope)

    termasuk ikatan simpul dan jumlah lilitan

    Verifikasi fungsi indikator-indikator Verifikasi dimensi boom Verifikasi counter weight Pemeriksaan persiapan pengujian

    - Persiapan tempat pengujian - Prosedur pengujian - Persiapan alat uji antara lain :

    C. INSPECTION FOR OCCUPATIONAL SAFETY OF THE LIFTING EQUIPMENT /CRANE The technical inspection of the lifting equipment consists of : a. Review of Documents

    b. Physical Inspection

    Inspection of joints Inspection of load holding hook Inspection of moving as well as rotating

    parts Inspection of hydraulic / pneumatic system Inspection of the condition of wire rope

    Verification of functions of the indicators Verification of the booms dimension Verification of counter weight Test preparation inspection

    - Test location preparation - Test procedures - Preparation of Test equipment among

  • c. Pelaksanaan pengujian Uji fungsi tanpa beban Uji beban dengan prosedur uji beban yang

    telah mendapat pengesahan Direktur

    d. Pemeriksaan setelah pengujian Pemeriksaan atas seluruh bagian-baigan yang terbebani dan bergerak ataupun yang diam untuk mengetahui kemungkinan adanya deformasi, retak, longgar pada baut dan atau cacat-cacat lainnya.

    e. Pemeriksaan khusus

    - Apabila pesawat angkat mengalami kerusakan atau diragukan kemampuannya.

    - Apabila diadakan perbaikan atau perubahan prinsipil pada pesawat angkat.

    f. Rekaman hasil uji Rekaman hasil uji diisi berdasarkan atas data pesawat angkat dan hasil uji.

    g. Pelaporan Menyusun seluruh hasil pemeriksaan teknis berbentuk dokumen untuk diserahkan ke Direktur Direktorat Teknik Pertambangan Minyak dan Gas Bumi sebagai bahan evaluasi untuk penerbitkan Sertifikat Kelayakan Penggunaan Peralatan.

    D. PEMERIKSAAN KESELAMATAN KERJA UNIT POMPA, UNIT KOMPRESOR DAN PENGGERAKNYA.

    Pemeriksaan teknis unit pompa, unit kompresor dan penggeraknya meliputi :

    a. Penilaian terhadap rancang bangun unit

    Pompa, unit Kompresor dan penggeraknya dengan melaksanakan penelaahan data : - Spesifikasi perencanaan; - Gambar dan perhitungan perencanaan; - Spesifikasi material yang digunakan; - Spesifikasi sambungan yang digunakan; - Spesifikasi komponen utama dan

    pendukung serta piranti pengaman yang digunakan;

    - Pedoman pengoperasian dan pemeliharaannya.

    b. Penilaian fabrikasi dengan melaksanakan penelaahan laporan inspeksi yang dilakukan oleh Pihak Ketiga yang independen di pabrik pembuat yang terdiri dari antara lain :

    c. Test implementation Functional test with no load Load test under load test procedures already

    ratified by the Director

    d. Inspection after test Inspection of all loaded and rotating or stationary parts to find out the possibility of any deformation, crack, lessen in bolts and / or other defects.

    e. Special inspection - If the lifting equipment is damaged or its

    ability is doubted. - If principal repairs or alterations are made

    on the lifting equipment.

    f. Test result form The test result form should be completed based on the lifting equipment data and test results.

    g. Reporting

    To prepare all the technical inspection results in the form of documents to be submitted to the Director of the Directorate of Oil and Gas Mining Engineering as evaluation material for the issue of the Worthiness of Equipment Operation Certificate.

    D. INSPECTION FOR OCCUPATIONAL

    SAFETY OF THE PUMP UNIT, COMPRESSOR UNIT AND THEIR DRIVE

    The technical inspection of the pump unit, compressor unit and their drive includes :

    a. Evaluation of the design of the Pump Unit,

    Compressor Unit and their drive by performing a data analysis of : - Design specifications; - Design calculation and drawing; - Specifications of the materials used; - Specifications of the joints used; - Specifications of the main and supporting

    components as well as safety appliances used;

    - Their operating and maintenance Manuals.

    b. Evaluation of fabrication by making an analysis of the report on inspection carried out by the independent third party in the manufacturers factory consisting among others of :

    c. Pemeriksaan fisik, yaitu pemeriksaan mengikuti proes pemasangan unit Pompa / unit Kompresor dengan penggeraknya dilokasi pemasangan dengan melakukan : - Identifikasi peralatan dan komponen; - Pemeriksaan kondisi struktur pondasi - Pemeriksaan kelengkapan alat ukur dan

    piranti pengaman; - Pemeriksaan umum kondisi pemasangan

    unit Pompa / unit Kompresor dan penggeraknya;

    - Uji mekanikal, kelistrikan dan instrumen; - Uji coba piranti pengaman - Pemeriksaan pelaksanaan start up; - Uji kemampuan.

    d. Pelaporan

    Menyusun seluruh hasil teknis berbentuk dokcumen untuk untuk diserahkan ke Direktur Direktorat Teknik Pertambangan Minyak dan Gas Bumi sebagai bahan evaluasi untuk penerbitan Sertifikat Kelayakan Penggunaan Peralatan.

    E. PEMERIKSAAN KESELAMATAN KERJA PIPA PENYALUR

    Pemeriksaan teknis pipa penyalur meliputi : a. Penelaahan Dokumen

    - Umum Lokasi Jadwal penyelesaian pembangunan Spesifikasi perencanaan Spesifikasi prosedur las dan rekaman

    kualifikasi prosedur serta catatan kualifikasi (sertifikat) unjuk kerja juru / operator las

    - Identification of materials - Verification of instrument pipes - Verification of the test of main components - Balance test results - Alignment inspection results of the

    coupling and shaft of the pump / compressor with their drive;

    - Results of the factory acceptance test already carried out in the manufacturers factory;

    - Inspection of the driving system.

    c. Physical inspection, that is an inspection following the installation process of the Pump unit / Compressor unit with their drive in the installation location by performing : - Identification of equipment and

    components; - Inspection of the foundation structures

    condition - Inspection for completeness of the

    measuring device and safety appliances - General inspection of the installation

    condition of the Pump unit / Compressor unit with their drive;

    - Mechanical, electrical and instrument test; - Safety device trial test - Inspection of the start up implementation - Performance test.

    d. Reporting

    To prepare (compile) all the technical inspection results in the form of documents to be submitted to the Director of the Directorate of Petroleum and Gas Mining Engineering as evaluation material for the issue of the Worthiness of Equipment Operation Certificate.

    E. INSPECTION FOR OCCUPATIONAL

    SAFETY OF THE PIPELINE.

    The technical inspection of the pipeline includes : a. Review of Documents

    - General Location Construction completion schedule Design specifications Specifications on welding procedures

    and qualification record of procedures as well as performance qualification certificate of the welder / welding operator

    maksimum dan minimum (khusus pipa penyalur lepas pantai)

    * Sudut kemiringan * Kedalaman perparitan * Jarak bentangan maksimum yang

    tersangga * Right of Way (ROW).

    Penelaahan metode pelengkungan pipa di

    lokasi meliputi : * Diameter pipa * Ketebalan pipa * Radius lengkung pipa

    Penelaahan beban pipa penyalur

    * Beban operasi antara lain berat sendiri, tekanan internal dan eksternal, ekspansi panas, daya apung dan lain-lain.

    *Beban lingkungan antara lain gelombang, arus, angin, gempa dan lain-lain.

    * Beban pemasangan antara lain buckling, residual stress yang terjadi saat instalasi pipa.

    Penelaahan perhitungan kekuatan pipa

    penyalur antara lain : * Tegangan yang terjadi pada pipa

    penyalur, buckling on buttom stability (khusus pipa penyalur lepas pantai)

    * Perhitungan penyangga pipa penyalur *Perhitungan tekanan uji yang akan

    dilakukan

    Penelaahan perhitungan pengendalian korosi antara lain : * Karakteristik korosi * Metode pengendalian korosi * Spesifikasi dan dimensi coating * Kebutuhan pengendalian korosi yang

    diperlukan

    Penelaahan pada spesifikasi terkait dan sitem intrumentasi antara lain :

    Procedures for repairs Specifications of the materials used Procedures for operation and

    maintenance Cleaning and drying procedures Safety device data

    - Design

    Review of the pipeline route map, i.e :

    * Depth of the max. and min. pipeline location (especially offshore pipeline)

    * Slope angle * Depth of ditches * Maximum range distance supported * Right of Way (ROW).

    Review of the pipe curving method in

    the location consisting of : * Diameter of the pipe * Thickness of the pipe * Curving radius of the pipe

    Review of the pipelines load

    * Operational load i.e own weight, internal and external pressure, heat expansion, floating capacity etc

    * Surrounding load i.e wave, current, wind, earthquake, etc.

    * Installation load i.e buckling, residual stress taking place at the time of the pipe installation

    Review of the pipelines strength calculation, i.e : *Tension taking place on the pipeline,

    buckling on bottom stability (especially offshore pipeline)

    * Calculation of the pipelines support * Calculation of the test pressure which

    will be carried out

    Review of the corrosion control calculation, i.e : * Characteristics of corrosion * Corrosion control method *Coating specifications and dimensions * Corrosion control requirement

    untuk memastikan klasifikasi area, tipe konstruksi dan jarak aman yang tersedia;

    - Pemeriksaan persiapan pengelasan dan kesamaan sumbu serta kelurusan sambungan pipa;

    - Pemeriksaan sistem penyangga dan perparitan;

    - Penelaahan hasil uji tidak merusak; - Kondisi dan rekaman hasil uji sistem

    pengendalian korosi serta pemberat pipa (khusus pipa penyalur lepas pantai);

    - Pemeriksaan pelaksanaan pembersihan dan pengeringan dalam pipa penyalur;

    - Pemeriksaan kelengkapan piranti pengaman.

    c. Pemeriksaan pelaksanaan uji tekan.

    - Review of the non destructive test results; - Condition and record of the test results of

    corrosion control system and pipe weight (especially offshore pipeline);

    - Inspection of the cleaning and drying implementation in the pipeline;

    - Inspection for completeness of the safety devices.

    c. Inspection of the pressure test implementation,

    Inspection for completeness of the test equipment consisting of :

    - Dead weight tester (DWT) - Pressure gauge - Temperature gauge - Pressure recorder - Temperature recorder - Pump

  • Alat pengukur dan pencatat tersebut harus dikalibrasi sebelum digunakan, dan skala penunjukkan maksimumnya adalah 1,5 4 kali tekanan uji.

    d. Uji tekan Uji tekan dilaksanakan dengan menaikkan tekanan secara bertahap sampai dengan tekanan uji dan ditahan minimum selama 24 (dua puluh empat) jam untuk pipa penyalur di darat, minimum 8 (delapan) jam untuk pipa penyalur di lepas pantai dan minimum 2 (dua) jam untuk pipa penyalur uap panasbumi.

    e. Pelaporan

    Menyusun seluruh hasil teknis berbentuk dokcumen untuk untuk diserahkan ke Direktur Direktorat Teknik Pertambangan Minyak dan Gas Bumi sebagai bahan evaluasi untuk penerbitan Sertifikat Kelayakan Penggunaan Peralatan.

    PEMERIKSAAN KESELAMATAN KERJA UNIT POWER GENERATOR, UNIT TRANSFORMER, UNIT SWITCHERGEAR DAN UNIT MOTOR CONTROL CENTER

    Pemeriksaan teknis peralatan listrik meliputi :

    a. Penelaahan Dokumen Standar yang digunakan; Spesifikasi unit peralatan listrik / data

    sheet (Generator, Transformer, Switchgear dan Motor Control Center);

    Gambar perencanaan; Spesifikasi perencanaan & konstruksi Spesifikasi material; Perhitungan perencanaan (bila

    disyaratkan); Prosedur pengoperasian & pemeliharaan; Klasifikasi area; Piranti pengaman; Klas isolasi

    b. Pemeriksaan fisik meliputi : Pemeriksaan peralatan dan komponennya Verifikasi plat nama Pemeriksaan umum kondisi pemasangan di

    lokasi Pemeriksaan lay out peralatan dan area

    klasifikasi

    Those gauges and recorders shall be calibrated before being employed and the maximum reading scale shall be 1.5 - 4 times test pressure.

    d. Pressure Test The pressure test is carried out by increasing the pressure in phases until reaching the test pressure, and is retained for minimum 24 (twenty four) hours for the onshore pipeline, minimum 8 (eight) hours for the offshore pipeline and minimum 2 (two) hours for the geothermal pipeline.

    e. Reporting

    To prepare (compile) all the technical inspection results in the form of documents to be submitted to the Director of the Directorate of Petroleum and Gas Mining Engineering as evaluation material for the issue of the Worthiness of Equipment Operation Certificate.

    F. INSPECTION FOR OCCUPATIONAL

    SAFETY OF THE POWER GENERATOR UNIT, TRANSFORMER UNIT, SWITCH GEAR UNIT AND MOTOR CONTROL CENTER UNIT

    The technical inspection of the electrical

    equipment consists of : a. Review of Documents Standard applied; Specifications of the electrical equipment

    unit / data sheet (Generator, Transformer, Switch Gear and Motor Control Center);

    Design drawing; Construction & design specifications; Specifications of materials; Design calculation (if required); Procedures for operation & maintenance; Area classification; Safety applieances; Insulation grade.

    b. The physical inspection comprises :

    Inspection of equipment and its components

    Name plate verification General inspection of the installation

    condition in the location Inspection of the equipment layout and area
