4. Midterm Lesson Plan

Cooperating Teacher/Mentor Teacher Evaluation 2 (end of experience/semester) Students give Cooperating/Mentor Teacher hard-copy of ST Evaluation (download/print from Taskstream); Students SCAN and ATTACH completed form; and give hard-copy to College Supervisor NOTE: College/Clinical Faculty Supervisor INPUTS cooperating teacher ratings into Taskstream Observation Form4 – Calculated Score (Points)

11. Attendance Sheet 2 (end of experience/semester completed):
